A 43-year-old male presented to the emergency department due to progressive weakness of the left hand and wrist following blunt trauma to the elbow around three weeks prior to evaluation. He was initially evaluated at the time of injury with radiographs of the elbow and forearm, which were reported to be normal. Due to worsening weakness, he sought care at additional emergency departments and with a neurologist who performed a CT, repeat radiographs, and eventually an MRI of the elbow, all of which revealed no acute findings. At our facility, point-of-care ultrasound evaluation of the elbow de

Moebius syndrome is a rare congenital disorder of unknown etiology. It is characterized by non-progressive paralysis of the facial and abducens cranial nerves due to atrophy of their nuclei, often accompanied by malformations in the upper and lower limbs. This report aims to highlight the critical role of hospital dentistry in managing a 6-year-old patient with Moebius syndrome. The patient was on mechanical ventilation via tracheostomy following a cardiorespiratory arrest at birth and has been hospitalized in a long-term care institution. Feeding and medication were administered via gastrosto

Despite its increasing prevalence, nonalcoholic fatty liver disease (NAFLD) remains under-diagnosed in primary care. Timely diagnosis is critical, as NAFLD can progress to nonalcoholic steatohepatitis, fibrosis, cirrhosis, hepatocellular carcinoma, and death; furthermore, NAFLD is also a risk factor linked to cardiometabolic outcomes. Identifying patients with NAFLD, and particularly those at risk of advanced fibrosis, is important so that healthcare practitioners can optimize care delivery in an effort to prevent disease progression. This review debates the practical issues that primary care

INTRODUCTION: Iliopsoas abscess complicating chronic periprosthetic joint infection (PJI) after hip arthroplasty is rare but potentially fatal. Because the iliopsoas bursa communicates with the hip joint, infection can track in either direction, delaying diagnosis and sustaining sepsis and treatment failure. Recognizing this presentation early is what allows the underlying prosthetic infection to be identified.
CASE REPORT: An 88-year-old man with a previously well-functioning right total hip arthroplasty presented with a persistent inflammatory syndrome. Contrast-enhanced computed tomography

INTRODUCTION: Superior Vena Cava Syndrome (SVCS) is a rare but serious oncologic emergency in pediatric patients, most commonly caused by mediastinal masses such as lymphomas or leukemias. This condition results from the obstruction of the superior vena cava (SVC), leading to impaired venous return and respiratory and cardiovascular complications, progressive exacerbation in a short period, and an extremely high fatality rate. We report the case of a 12-year-old boy with SVCS caused by a mediastinal mass.
MAIN SYMPTOMS/FINDINGS: The patient presented with progressive dyspnea, orthopnea, and s

Ignatzschineria indica is a rare Gram-negative bacterium increasingly recognized as a potential human pathogen associated with wound myiasis and severe bloodstream infections. Here we report a fatal case of I. indica bacteremia in a 63-year-old woman admitted to the intensive care unit with septic shock. The patient had arterial hypertension and liver cirrhosis and was found after prolonged immobilization in severely neglected conditions. Clinical examination revealed extensive pressure ulcers complicated by maggot infestation and a left pertrochanteric femoral fracture. Blood cultures became

Background: Central pontine myelinolysis is a rare condition that can result in debilitating symptoms. Recent evidence suggests a shift in hypothesized prognosis for individuals with this diagnosis and implies a valuable role for rehabilitation to address prevention of secondary complications and functional recovery. This paper describes a 30 year-old female with central pontine myelinolysis presenting with incomplete locked-in syndrome and chronicles her rehabilitative journey at a comprehensive post-acute rehabilitation center. We present discipline specific interventions and discuss the int

Background: Assessing and Listening to Individual Goals and Needs (ALIGN) is a palliative care social work intervention that aims to improve delivery of goal-concordant care for hospitalized older adults with cancer discharged to skilled nursing facilities. Objective: Explore processes through which ALIGN may improve delivery of goal-concordant care to substantiate the conceptual model grounding the intervention and to inform mechanistic hypotheses of how the intervention might be effective. Design: A process evaluation triangulating findings from patient and caregiver interviews with a matrix

Snakebites are a critical health issue in remote regions of the Amazon basin. I report a case of Bothrops bilineatus envenomation in a 29-y-old biologist conducting fieldwork in Parque Nacional del Manu, Peru. Following significant logistic delays-including an initial suboptimal antivenom dose and limited access to additional vials-the patient developed progressive edema, necrosis, coagulopathy, and signs of compartment syndrome. Management involved 2 partial doses of antivenom (1 at 1600 and the other at 2100), a subsequent transfer to a tertiary care center, antibiotics, analgesia, and event

A 37-year-old woman with a medical history of myasthenia gravis resulting in progressive respiratory failure requiring continuous mechanical ventilation via tracheostomy, as well as multiple cardiac arrests leading to severe anoxic brain injury, was brought to the hospital from a nursing home because of difficulties with ventilation and oxygenation. On presentation to the ED, the patient was found to be agitated and tachypneic on a ventilator, generating low tidal volumes despite elevated peak airway pressures. Before the current presentation, the patient had been mechanically ventilated at a

This case report chronicles a 47-year-old male with no known past medical history, who presented to the emergency department with a chief complaint of progressive dyspnea and lower extremity edema. The patient was previously healthy until he contracted COVID-19 approximately six months prior to the date of presentation. He made a full recovery two weeks later. However, in the ensuing months, he progressively declined with worsening shortness of breath and lower extremity edema. On outpatient cardiology evaluation, he was found to have cardiomegaly on chest radiograph and sinus tachycardia on e

The authors present a unique case of immune-mediated necrotizing myopathy in a 60-year-old female who presented with aphasia, tremor, and progressive muscular weakness. To the best of our knowledge, the presentation of aphasia and tremor has not been previously reported in immune-mediated necrotizing myopathy. This rare presentation may mimic a variety of other central nervous system and myopathic disorders. Delays in prompt diagnosis and treatment of immune-mediated necrotizing myopathy can be detrimental to the physical and mental well-being of the patient and additionally leads to inefficie

A 51-year-old man presented with a 3 month history of progressive right visual loss. On examination, the only abnormal findings were a right visual acuity of 4/10 and a swollen right optic disc. The patient had previously undergone MRI that had been reported as being normal and a trial of corticosteroids under the care of the referring ophthalmologist. An extensive battery of blood tests was normal apart from a rapid plasmatic reagin titre of 1:64. ELISA revealed elevated levels of total and immunoglobulin M antibodies against Treponema pallidum. Cerebrospinal fluid analysis revealed a mild le

BACKGROUND: The progressive loss of function affects the quality of life of the ALS patient. Quality of life depends on a spectrum of factors. Available instruments for assessment include questionnaires developed for generic illness-related disabilities and ALS-specific questionnaires that focus on function.
METHODS: Comparisons were made between two ALS-specific questionnaires (ALS Functional Rating Scale and SIP/ALS-19) and a generic instrument (Short Form-12) given to 1,513 patients from the (North American) ALS Patient Care Database.
RESULTS: The SIP/ALS-19 correlates well with the ALS F

BACKGROUND: Two thirds of patients with primary hyperparathyroidism (PHPT) go untreated, risking progressive osteoporosis, fractures, and nephrolithiasis. Low patient motivation for surgery has been identified as a significant factor limiting surgical referral and treatment. We sought to better understand what patient and clinician-level factors drive low patient motivation to identify targetable interventions.
STUDY DESIGN: Using a qualitative phenomenological framework, we conducted semi-structured interviews with 19 primary care providers (PCPs), 14 patients who underwent parathyroidectomy

Tuberculosis (TB) is a global public health concern and a leading cause of death. Its persistence occurs mainly because barriers in the care cascade are not being fully addressed. Healthcare professionals and scientists have been addressing treatment challenges such as abandonment and irregular drug intake via strategies such as directly observing treatment and singular therapeutic projects to improve adherence. However, while protocols and guidelines advocate these strategies, their implementation requires a broader approach from healthcare teams. This article examines the importance of such

Hematopoietic stem cell transplants are increasingly being performed in attempt to halt the progression of juvenile metachromatic leukodystrophy, which is a rare neurodegenerative disease. Children who are diagnosed with metachromatic leukodystrophy are not commonly cared for by nurses who specialize in pediatric stem cell transplants. This article provides nurses with insight about this disease and serves as a guide for nursing care of this patient population during hematopoietic stem cell transplant. The case study highlights the complexities of care of this population while illustrating man
