Pregnancies in Gaucher disease: a 5-year study.

OBJECTIVE: The study was undertaken to investigate the outcome of pregnancies in Gaucher disease, particularly in enzyme-treated women. STUDY DESIGN: A retrospective study was performed of pregnant women evaluated at a referral clinic. RESULTS: There were 43 (17 treated, 26 untreated) women with 66 pregnancies (23 treated, 43 untreated). The live birth rate was 78.3% among treated, 86.0% among untreated. One treated woman had three spontaneous abortions; 3 untreated women had one each. Four pregnancies in each group had postpartum bleeding, 7 requiring transfusions. Postpartum infections wer

Antibiotic regimens for endometritis after delivery.

BACKGROUND: Postpartum endometritis, which is more common after cesarean section, occurs when vaginal organisms invade the endometrial cavity during labor and birth. Antibiotic treatment is warranted. OBJECTIVES: The effect of different antibiotic regimens for the treatment of postpartum endometritis on failure of therapy and complications was systematically reviewed. SEARCH STRATEGY: We searched the Cochrane Pregnancy and Childbirth Group's trials register (30 January 2004). SELECTION CRITERIA: Randomized trials of different antibiotic regimens for postpartum endometritis, after cesarean s

Antibiotic prophylaxis for operative vaginal delivery.

BACKGROUND: Vacuum and forceps assisted vaginal deliveries are reported to increase the incidence of postpartum infections and maternal readmission to hospital compared to spontaneous vaginal delivery. Prophylactic antibiotics are prescribed to prevent these infections. However, the benefit of antibiotic prophylaxis for operative vaginal deliveries is still unclear. OBJECTIVES: To assess the effectiveness and safety of antibiotic prophylaxis in reducing infectious puerperal morbidities in women undergoing operative vaginal deliveries including vacuum and/or forceps deliveries. SEARCH STRATEG

[Bacterial vaginitis: general overview].

Bacteria are the most frequently detected agents in women, clinically complaining of vaginal discharge. The studies have shown that the vaginal microflora of women with bacterial vaginitis have altered from Lactobacillus spp. to various anaerobic bacteria. Gardnerella vaginalis is found in vaginal flora of women with bacterial vaginitis as well as in healthy women, while anaerobic bacteria such as Mobiluncus and Prevotella are the causative agents for bacterial vaginosis. For the laboratory diagnosis of bacterial vaginitis, direct microscopy is one of the most commonly used methods, and for th

Dr John Clarke: licentiate in midwifery of the Royal College of Physicians of London.

John Clarke was one of ten licentiates in midwifery created by the College of Physicians in the late eighteenth century to regularise the growing involvement of medical men in obstetrics. He was an excellent clinician and a popular teacher. His publications included original observations about puerperal fever, nausea and other complications of pregnancy and the management of labour. He opposed the attitude of the College that prevented its Fellows from practising midwifery and he stridently criticised the College's neglect of the diseases of children. Part one of his book on the diseases of ch

Postpartal endomyometritis in a case of unknown tertian malaria.

A 28-year-old woman developed puerperal endomyometritis and tertian malaria simultaneously. She delivered her child by vacuum extraction during week 41 of pregnancy in September 1994. The peripartal period was uneventful. Nine days post partum the patient was readmitted to hospital with fever and pain in the area of the episiotomy. On day 13 post partum a hysterectomy was performed because of suspected abscess-forming endomyometritis. Two days after the hysterectomy the patient developed septic temperatures, which persisted for 10 days. Tertian malaria due to Plasmodium vivax was found to be t

[Upper genital tract infections in women infected with HIV: epidemiologic, diagnostic and therapeutic aspects].

Fifty seven women hospitalized in the gynecological department of Bichat-Claude Bernard hospital in Paris for pelvic inflammatory disease (PID) proven by laparoscopy from sept. 1992 to nov. 1994 had a serological diagnosis of HIV infection. Three of them (5,2%) were positive (HIV+). Previous publications have shown from 4% to 17% in the USA and as much as 32% in Abidjan. However this prevalence is arising each year. PID in HIV+ seems to have more often a poor symptomatology but require more surgical treatments. Postpartum endometritis in HIV+ were also studied. From 1989 to 1994, we observed 1

[Analysis of lochial acid-base equilibrium in an uncomplicated course of the postpartum period and in endometritis].

Lochial acid-base status was analyzed in 45 puerperants in whom the postpartum period ran an uncomplicated course or was complicated by endometritis. The normal course of the puerperium is associated with the development of metabolic acidosis in the uterine cavity, and this acidosis is completely compensated for at the expense of gas alkalosis. In slight endometritis metabolic acidosis augments, and grave endometritis is associated with the development of marked acidosis at the expense of the development of tissue hypoxia in the presence of metabolic acidosis.

Postpartum urinary retention.

BACKGROUND: Urinary retention is a common and frustrating complication in women during the immediate postpartum period. Physiologic changes in the bladder that occur during pregnancy predispose patients to develop symptomatic retention of urine during the first hours to days after delivery. METHODS: The incidence and characteristics of postpartum urinary retention were researched through a literature review and are illustrated by a case report. RESULTS AND CONCLUSIONS: Postpartum urinary retention has a reported incidence ranging from 1.7 to 17.9 percent. Factors associated with postpartum u

Early online.

BACKGROUND: Caesarean section is a life-saving procedure which is associated with high rates of maternal and neonatal complications. It has been estimated that globally, 29.7 million births occur by caesarean section annually. The risk of postpartum infection is estimated to be five to ten times higher compared to normal vaginal delivery. Pregnancy-related sepsis was listed as a top six cause of maternal mortality in the South African Saving Mothers report between 2017 and 2019. Multiple trials have been conducted in an attempt to optimise administration of prophylactic antibiotics in an effor

Azithromycin-based Extended-Spectrum Antibiotic Prophylaxis for Cesarean: Role of Placental Colonization with Genital Ureaplasmas and Mycoplasmas.

OBJECTIVE: To explore whether the effect of azithromycin (AZI) on postcesarean infections varied by the presence/absence of genital mycoplasmataceae placental colonization. STUDY DESIGN: This was a single-center substudy of multicenter double-blind C/SOAP (Cesarean Section Optimal Antibiotic Prophylaxis) trial of women randomized to AZI or placebo (+cefazolin) antibiotic prophylaxis at cesarean. Chorioamnion/placenta specimens were tested for genital mycoplasmataceae colonization by polymerase chain reaction. Primary outcome was a composite of endometritis, wound infection, or other infection
