Differential Patterns and Outcomes of 20.6 Million Cardiovascular Emergency Department Encounters for Men and Women in the United States.

Background We describe sex-differential disease patterns and outcomes of >20.6 million cardiovascular emergency department encounters in the United States. Methods and Results We analyzed primary cardiovascular encounters from the Nationwide Emergency Department Sample between 2016 and 2018. We grouped cardiovascular diagnoses into 15 disease categories. The sample included 48.7% women; median age was 67 (interquartile range, 54-78) years. Men had greater overall baseline comorbidity burden; however, women had higher rates of obesity, hypertension, and cerebrovascular disease. For women, the m

[Coronary artery surgery after 70 years: an analysis of the risk factors of operative mortality].

The risk factors of operative mortality after coronary bypass surgery in patients over 70 years of age were studied in a consecutive series of 109 patients operated in our department between January 1990 and June 1992. The anginal pain was classified stage III or IV in 92 cases. Seventy-nine patients had triple vessel disease, 36 patients had left main stem stenosis and 57 had previous myocardial infarction. Twenty-six patients had ejection fractions of less than 50% and 6 were less than 30%. The average number of bypass grafts was 2.35. Associated procedures included 9 endarteriectomies of th

[Aortic Valve Replacement for Active Infective Endocarditis with Multiple Cerebral Infarctions and Subarachnoid Hemorrhage:Report of a Case].

We report a case of successful surgical treatment of infected aortic valve endocarditis with intractable heart failure, multiple cerebral infarction, and subarachnoid hemorrhage. A 46-year-old man who had fever for two weeks and subsequent left hemiplegia was admitted to our hospital. Brain computed tomography (CT) and magnetic resonance imaging (MRI) revealed multiple cerebral infarction and subarachnoid hemorrhage. Echocardiography showed severe aortic regurgitation with a huge mobile vegetation. Despite medical treatment, chest X-ray revealed progressive pulmonary congestion. We performed a

A Case of Post Myocardial Infarction Papillary Muscle Rupture.

Papillary muscle rupture is a rare, life-threatening post myocardial infarction mechanical complication. Without surgical intervention, prognosis is very poor. Clinicians need to recognize this complication early, as prompt therapy is crucial. We present a case of inferior ST elevation myocardial infarction complicated by posteromedial papillary muscle rupture resulting in severe acute mitral regurgitation (flail anterior mitral leaflet), acute pulmonary edema and cardiogenic shock. In our patient, a new mitral regurgitation murmur suggested this mechanical complication. Complete disruption of

Scorpion bite, a sting to the heart!

Scorpion bites are common in India. Usually, these bites are harmless but sometimes have serious clinical sequelae, including death. We report herein a case of scorpion bite with electrocardiographic abnormalities simulating early myocardial infarction. Pulmonary edema and congestive heart failure accompanied these electrocardiographic changes as well as serum cardiac markers. The etiology of cardiovascular manifestations in severe scorpion sting is related to venom effect on sympathetic nervous system and the adrenal secretion of catecholamines as well as to the toxic effect of the venom on t

Charcot and vascular Parkinsonism.

Jean-Martin Charcot (1825-1893), recognized as the founder of Neurology and the first formal teacher of nervous system diseases, died on August 16, 1893, from acute pulmonary edema secondary to myocardial infarction. In his last years, there were several descriptions of his gait and posture disorders, suggesting the diagnosis of "lower-half parkinsonism" due to cerebrovascular disease.

Coronary artery compression three months after transcatheter pulmonary valve implantation.

We report a case of a 29-year-old man who developed exercised-induced myocardial infarction 3 months post Melody valve implantation. We introduce the concept of ruling out dynamic coronary artery compression by simulating transcatheter pulmonary valve implant while increasing cardiac output and thus aortic dimensions in the catheterization laboratory. © 2014 Wiley Periodicals, Inc.

Difficult extubation: calming the sympathetic storm.

Tracheal extubation can evoke an equally strong haemodyamic stress response as tracheal intubation. We present a patient with myocardial infarction who repeatedly failed tracheal extubation. He developed acute pulmonary oedema following each attempt at tracheal extubation due to sympathetic overactivity. A change of approach with extubation under propofol sedation followed by continued sympatholysis with dexmedetomidine infusion allowed successful extubation.

The clinical utility of transesophageal echocardiography in ischemic papillary muscle rupture.

Ischemic papillary muscle rupture is a potentially lethal complication of myocardial infarction. Acute mitral regurgitation, shock with pulmonary edema, may ensue as its result. Transthoracic echocardiography is the initial noninvasive tool used to confirm the diagnosis. Transesophageal echocardiography has overcome some of the limitations of transthoracic imaging, permitting timely surgical management of these critically ill patients.

Prospective, open-label, uncontrolled pilot study to study safety and efficacy of sildenafil in systemic sclerosis-related pulmonary artery hypertension and cutaneous vascular complications.

Pulmonary artery hypertension (PAH) remains the leading cause of morbidity and mortality in systemic sclerosis, while Raynaud's phenomenon and digital ulcers significantly add to the morbidity in systemic sclerosis (SSc). This study was undertaken to evaluate the role of sildenafil in PAH, Raynaud's phenomenon, and digital ulcers in systemic sclerosis patients. A prospective, open-label, uncontrolled pilot study was done at a tertiary care centre in India to study the safety and efficacy of oral sildenafil in PAH, Raynaud's phenomenon, digital infarcts, and ulcers in SSc. Seventeen patients fu

Differential diagnoses of acute ground-glass opacity in chest computed tomography: pictorial essay.

Ground-glass opacity is a very frequent and unspecified finding in chest computed tomography. Therefore, it admits a wide range of differential diagnoses in the acute context, from viral pneumonias such as influenza virus, coronavirus disease 2019 and cytomegalovirus and even non-infectious lesions, such as vaping, pulmonary infarction, alveolar hemorrhage and pulmonary edema. For this diagnostic differentiation, ground glass must be correlated with other findings in imaging tests, with laboratory tests and with the patients' clinical condition. In the context of a pandemic, it is extremely im…

[The lung in heart diseases].

The effects of "hypocirculation" and "hypercirculation" of the lungs are small. Hypocirculation has an influence of the ventilation/perfusion ratio, and can thus contribute to hypocapnia. In the early stages, hypercirculation--in particular via a left-to-right shung, leads to an increase in diffusion capacity; after a course of many years, a "counter-situation" occurs. Progressive pulmonary hypertension, as is exemplified for mitral stenosis, leads to measurable restrictive and obstructive impairment of function, and possible to unspecific hyper-reaction, as also, over the long-term, to a dimi

Rostral cerebellar arterial infarct in two cats.

A 10-year-old female neutered domestic shorthair (DSH) cat and a 6-year-old female neutered Siamese cat were presented following a peracute onset of decerebellate rigidity and a cerebellar vestibular syndrome, respectively. In both cats, physical examination and routine blood tests were unremarkable, as was routine analysis of cerebrospinal fluid obtained from the DSH cat. Based on the magnetic resonance imaging (MRI) features - focal wedge-shaped lesion in the cerebellum characterised by hyperintensity in T2-weighted, T2( *)-gradient echo and fluid attenuated inversion recovery (FLAIR) images

Association of Vegetation Size With Embolic Risk in Patients With Infective Endocarditis: A Systematic Review and Meta-analysis.

IMPORTANCE: Infective endocarditis is a life-threating condition with annual mortality of as much as 40% and is associated with embolic events in as many as 80% of cases. These embolic events have notable prognostic implications and have been linked to increased length of stay in intensive care units and mortality. A vegetation size greater than 10 mm has often been suggested as an optimal cutoff to estimate the risk of embolism, but the evidence is based largely on small observational studies. OBJECTIVE: To study the association of vegetation size greater than 10 mm with embolic events using

Unrepaired Truncus Arteriosus Type 1 With Eisenmenger Syndrome and Recurrent Embolic Strokes: An Adult Case Report.

BACKGROUND In adults, non-corrected truncus arteriosus type 1 with Eisenmenger syndrome is a rare congenital heart defect characterized by a single large artery from the heart that directs blood flow to the lungs, leading to irreversible pulmonary hypertension and a reversal of blood flow. This report describes the case of a 44-year-old woman with congenital Eisenmenger syndrome and unrepaired truncus arteriosus type 1 presenting with recurrent thromboembolic cerebral infarcts. CASE REPORT A 44-year-old woman presented with a 2-month history of recurrent neurological deficits, including aphasi

Antiphospholipid syndrome leading to paradoxical embolus and stroke via secundum atrial septal defect.

We have presented a case of a 22-year-old man, presenting with cerebral infarct, subsequently found to have antiphospholipid syndrome (APS), deep venous thrombosis, pulmonary embolism and atrial septal defect (ASD), thereby confirming the presence of infarct due to paradoxical embolism in this patient. The importance of ASD in the patients of APS, resulting in paradoxical embolism is debatable, with recent studies undermining its importance. We have demonstrated that it does indeed happen. This would have implications in the risk assessment and management of ASD in such patients. This case rep

Pulmonary edema localized in the right upper lobe accompanying mitral regurgitation.

Focal patterns of pulmonary edema are confusing and often mistaken for the more common causes of focal lung disease, pneumonia, infarction, or aspiration. The authors report four cases of right upper lobe edema secondary to mitral regurgitation. The pathogenesis believed to be responsible for this condition is the vector of blood flow from the left ventricle to left atrium, which may be targeted at the right superior pulmonary vein, locally accentuating the forces for edema formation in the right upper lobe. Pulmonary edema accompanying mitral regurgitation should be suspected whenever right u

Multiple Thromboembolic Cerebral Infarctions from the Aorta in a Patient with Churg-Strauss Syndrome.

BACKGROUND: Ischemic stroke is a rare complication of Churg-Strauss syndrome (CSS) and its pathogenesis has not been well clarified yet. We report a case of cerebral infarction in a patient with CSS due to embolism from a thrombus on the wall of the aorta. CASE: A 39-year-old man had multiple cerebral infarctions with symptoms of mild left hemiparesis and reduced vision. He was clinically diagnosed to have CSS based on remarkable eosinophilia, history of asthma, sinusitis, pulmonary infiltrates, and histologically proven extravascular eosinophilic infiltrates in the specimen of gastric mucosa

Carotid plaque vulnerability and circle of Willis anatomy predict ipsilateral brain infarcts and long-term mortality in carotid endarterectomy patients.

Cerebral infarcts in carotid artery disease arise from a complex interplay between plaque vulnerability, extracranial vascular burden, intracranial collateral capacity. Although each of these factors has been studied individually, their combined impact on infarct presence, infarct pattern, and long-term outcomes in real-world carotid endarterectomy (CEA) populations remains insufficiently understood. This study aimed to determine how carotid plaque morphology, bilateral carotid stenosis severity, circle of Willis (CW) anatomy jointly influence ipsilateral brain infarcts and overall mortality.
