Idiopathic thrombocytopenic purpura (ITP) is a disorder characterized by increased platelet destruction in the setting of normal megakaryopoiesis. Approximately 20% of patients with ITP are refractory to corticosteroids and splenectomy. Recently, pulse high-dose dexamethasone was reported to be effective in the treatment of chronic ITP in adult patients. Ovarian Hyperstimulation syndrome (OHSS) is a rare condition in patients with hypogonadotropic hypogonadism. Two patients with hypogonadotropic hypogonadism are reported, a rare case of severe OHSS and a case of prevented OHSS via gonadotropin-releasing hormone (GnRH) agonist trigger, respectively. The first case was a 31-year-old patient. In vitro fertilization (IVF) treatment was performed three times but the patient never developed OHSS. An 8-year-old girl having alopecia areata which progressed to alopecia totalis in 1½ years showed an incomplete regrowth of the hair when treated with our standard regime of 1 mg betamethasone per day orally in spite of the treatment having been continued for 8 months. Severe mononeuritis multiplex in systemic lupus erythematosus (SLE) is quite rare and almost always accompanied by evidence of active disease in other organs, although occasionally it may be the presenting feature of the disease. We describe here two patients with SLE who presented a severe mononeuritis multiplex secondary to a necrotizing vasculitis which respond successfully to therapy with intravenous cyclophosphamide, and review the literature of similar cases.

We describe a patient with fulminant adult onset Still's disease and multiple organ involvement. The acute flare persisted despite high doses of prednisone but responded to megadose intravenous pulse methylprednisolone therapy. This treatment modality may be useful in temporizing severe multisystem exacerbations of adult Still's disease.

Our patient presented with widespread airspace consolidation. He was a steroid-dependent asthmatic receiving moderate doses of corticosteroid therapy. Open lung biopsy showed allergic angiitis of Churg and Strauss syndrome. The patient continued to deteriorate on high doses of prednisone. He was subsequently given four pulses of intravenous methylprednisolone with dramatic clearing noted on x-ray film and resolution of his shunt.

A patient with sick sinus syndrome and resistant paroxysmal atrial flutter was treated with amiodarone and implanted with a Medtronic 7005 DDD pacemaker. Following discontinuation of amiodarone, an episode of sustained atrial flutter was converted to sinus rhythm by noninvasive rapid atrial pacing in the outpatient clinic. This case illustrates one of the many capabilities for arrhythmic intervention using currently available implanted pacemakers.
