Radiation treatment is widely used in pediatric cancer patients. Much attention should be paid to preserve the function and structure of normal organs and tissues adjacent to tumor. To achieve this goal, optimal treatment modality should be chosen and closely followed. Certain peculiarities of irradiation of children, particularly, infants should be considered. These include the use of medication sleep. Drug combinations to be used to prepare patients for radiotherapy are discussed.

Reconstruction of the thoracic wall is often necessary after cancer surgery of the breast or irradiation. The resulting large defects often cause an instable thoracic wall. To reconstruct stability, we can use fascia or dura mater. The large defects can be closed either by latissimus flap, or, if the thoracodorsal artery is destroyed, which is often the case by rotating flaps according to the technique of Schrudde or Bohmert, in very rare cases by distant flaps from the neighbouring upper arm.

Six cases are presented: two degenerated and four necrotic X-ray dermatitis. Four patients had received X-ray therapy for benign lesions. All suffered long standing X-ray dermatitis lasting up to 60 years. The princip of treatment is a complete excision of the diseased skin and the covering of the defect with flaps. Such severe cases which need a complex treatment, must be handled with caution. X-ray therapy is a powerful but dangerous tool; its dangers are often known late; it should be reserved for severe conditions.

Radiation ulcera are a permanent source of danger. Their treatment may be long and difficult. Surgical competence is the clue to treatment. Conservative treatment is justified only in the beginning and for short periods. The goal of surgery is excision of damaged tissue and plastic cover with healthy skin. The technique will have to be chosen according to findings and localisation.
