Technique of combined heart-lung transplantation.

Heart-lung transplantation was, for many years, conceptualized as a possible treatment for patients with combined end-stage cardiac and pulmonary disease. As experience grew with heart transplantation, particularly in the 1970s, the difficulties of performing the orthotopic operation in patients with fixed pulmonary hypertension became apparent. This further impetus for combined heart-lung transplantation led to successful animal experiments in the late 1970s, and the first successful heart-lung transplant operation was performed in 1981. There has been significant evolution in the operative t

The prevalence of superior vena cava anomalies as detected in cardiac implantable electronic device recipients at a tertiary cardiology centre over a 12-year period.

INTRODUCTION: The vast majority of cardiac implantable electronic device (CIED) recipients require transvenous lead insertion, which may be hindered by the presence of venous anomalies. The aim of this study was to determine the prevalence and variations of persistent left superior vena cava (PLSVC) and to conduct subsequent outpatient follow-up in terms of device function and the clinical condition of the recipients using data from CIED placement procedures conducted over a 12-year period. METHODS: The study population included patients undergoing first-time transvenous implantation of cardi

Use of Pedicled Trapezius Myocutaneous Flap for Posterior Skull Reconstruction.

BACKGROUND: Soft-tissue defects in posterior skull can be challenging for reconstruction. If related to tumor resection, these wound beds are generally irradiated and can be difficult from a recipient-vessel perspective for a free tissue transfer. Locoregional flaps might prove to be important reconstructive option in such patients. There is a very limited data on the usage of pedicled trapezius myocutaneous flaps for such defects. METHODS: The authors reviewed existing study for usage of trapezius flap for posterior skull repair and used pedicled trapezius myocutaneous flaps based on the des

Hypothermic Liver Machine Perfusion With EKPS-1 Solution vs Aqix RS-I Solution: In Vivo Feasibility Study in a Pig Transplantation Model.

OBJECTIVE: Hypothermic machine perfusion (HMP) is superior to simple cold storage for kidney preservation. We previous observed in a porcine liver transplantation model increased tumor necrosis factor-alpha (TNF-alpha) production eventually leading to poor recipient survival after HMP using standard kidney perfusion solution (KPS-1) compared with simple cold storage. We compared two solutions for HMP preservation of the liver: enriched KPS-1 (EKPS-1) and Aqix RS-I. METHODS: Pig livers were obtained after cold flushing with histidine-tryptophan-ketoglutarate solution. Subsequently, the livers

Management of Venoarterial Extracorporeal Membrane Oxygenation Cannulation-Associated Groin Wound Complications With Muscle Flaps at a High-Acuity Cardiac Referral Center.

BACKGROUND: The insertion of large bore cannulas into the femoral vessels for venous-arterial extracorporeal membrane oxygenation (VA-ECMO) administration has been associated with significant acute and chronic wound complications in patients with significant medical and surgical comorbidities, including vessel exposure and lymphocele development. In this series, we report our experience using muscle flap reconstruction in the management of post-ECMO groin wounds, with particular emphasis on groin lymphocele. METHODS: VA-ECMO patients at a high-acuity cardiac referral center who developed groi

Selective versus non-selective laser photocoagulation of placental vessels in twin-to-twin transfusion syndrome.

OBJECTIVE: We have recently described a surgical technique for the treatment of twin-to-twin transfusion syndrome (TTTS) that allows precise identification of vascular anastomoses (selective laser photocoagulation of communicating vessels, or S-LPCV). The purpose of this study was to compare S-LPCV with the previous non-selective technique (NS-LPCV) that targeted all vessels crossing the dividing membrane. MATERIALS AND METHODS: Patients with TTTS were treated with NS-LPCV from May 1994 to June 1997 and with S-LPCV from July 1997 to December 1999. TTTS was defined as polyhydramnios of > or =

Thrombotic complications associated with stem cell transplantation.

Thrombotic complications are common in stem cell transplantation (SCT) recipients and endothelial cell injury is a dominant contributing factor to the hemostatic impairments. Endothelial cells line the vascular bed and each vascular bed has a unique structural and functional properties. Therefore, understanding of these properties may hold important clues to site-specific diagnostics and therapeutics. The two most common thrombotic manifestations related to SCT, veno-occlusive disease (VOD) and thrombotic microangiopathy (TMA), are characterized by small vessel thrombosis in the microcirculati

Survival of composite chondrocutaneous grafts by vessel implantation: a study in the rabbit ear model.

Composite chondrocutaneous graft reconstruction or reattachment has limited applicability, is traditionally restricted to small segmental losses, and is dependent on the status of the recipient bed and graft periphery for successful revascularization. Surgical enhancement of composite graft survival was experimentally investigated in the rabbit ear model through transposition and appositional placement of an adjacent vascular pedicle. Fluorescein-derived surface-survival determinations, microangiographic vessel-counting methods, and histologic analysis were used to study the effects of vascula

Factor 5 and Factor 2 heterozygous positivity and complications in living donor liver transplant donors.

OBJECTIVE: Factor 2 and Factor 5 mutations are among the most common procoagulant genetic disorders and are routinely evaluated in donor preparation. Homozygous mutations are contraindicated for surgery, but heterozygous mutations cannot be said to be an impediment. We aimed to investigate the effect of heterozygous gene mutation of F2 and/or F5 on complications. METHODS: In our study, 210 living liver donors were examined. The available data of Factor 2 and 5 heterozygous positive donors were evaluated in terms of 21 donor patients and 30 liver recipients. The heterozygous positive group and

Early experience with donation after circulatory death heart transplantation using normothermic regional perfusion in the United States.

OBJECTIVE: This pilot study sought to evaluate the feasibility of our donation after circulatory death (DCD) heart transplantation protocol using cardiopulmonary bypass (CPB) for normothermic regional reperfusion (NRP). METHODS: Suitable local DCD candidates were transferred to our institution. Life support was withdrawn in the operating room (OR). On declaration of circulatory death, sternotomy was performed, and the aortic arch vessels were ligated. CPB was initiated with left ventricular venting. The heart was reperfused, with correction of any metabolic abnormalities. CPB was weaned, and

Long-term outcomes of drug-eluting stents versus bare-metal stents in saphenous vein graft disease: results from the Prairie "Real World" Stent Registry.

OBJECTIVES: This study was designed to compare long-term clinical outcomes of drug-eluting stents (DES) versus bare metal stents (BMS) in patients with saphenous vein graft (SVG) disease in the "real world." BACKGROUND: The safety and efficacy of DES versus BMS in SVG remains uncertain due to contradictory reports of either lower revascularization rates with DES; or clinical equivalence to BMS; or even an excess of clinical events associated with DES use. METHODS: We identified consecutive patients who underwent stent placement within a de novo SVG lesion between May 1, 2003 and July 31, 200

[Concepts and clinical practice of functional reconstruction of complex wounds].

The repair of complex wounds has shifted from simple wound closure toward functional reconstruction. Nevertheless, a systematic exposition regarding their definition, the principles of functional reconstruction, and future directions is currently lacking. Based on a literature review and comprehensive analysis, this article proposes that the definition of complex wounds should incorporate four dimensions including abnormal healing timeline, involvement of anatomical structures, coexistence of multiple risk factors, and complex management requirements. Furthermore, the core concept of functiona

Innovative Reuse of Breast Expander Capsules for Parietal Pleura Repair in DIEP Flap Surgeries.

The deep inferior epigastric artery perforator (DIEP) flap is a very popular method for autologous breast reconstruction due to its aesthetic benefits and muscle preservation. However, preparation of recipient vessels during this procedure can lead to complications, including injury to the parietal pleura, resulting in a possible post-operative pneumothorax. Current literature on parietal reconstruction in such cases is limited, with a variety of treatment methods reported. We present an innovative approach using an autologous breast expander capsule for reconstruction of a parietal pleura def…

The impact of an anomalous third segment of the vertebral artery on bypass surgery: a case report and literature review.

The horizontal part of the third segment (V3) of the vertebral artery (VA) is a critical anastomotic site for bypass procedures involving either donor or recipient vessels. It is rare for the V3 segment to deviate from its typical course of passing through the atlanto-transverse foramen. V3 anomaly encountered in occipital artery (OA)-V3 bypass surgery has not been previously reported. Here, we present a case involving a patient undergoing bypass surgery due to recurrent post-stent occlusion at the first segment (V1) of the left VA. During the operation, it was noted that the V3 horizontal seg…
