We report a case of an extensive intermittent sigmoidorectal intussusception due to an adenocarcinoma of the rectum in an adult. Abdominal multislice computed tomography with two-dimensional multiplanar reformatted images and three-dimensional volume rendering rapidly established the diagnosis preoperatively. The intussusception was confirmed by surgery.

A 58-year-old woman was initially externally diagnosed in 1983 with a presacral mass, which was subsequently treated as an abscess. Thereafter, the patient experienced recurrent perianal fistulas. The authors report one case of perforation of rectal with peritonitis during a barium enema administration in a patient suffering from an adeno-carcinome of colon, and died after 48 hours with collapsus. After a brief review of the literature, the authors expose pathogeny and clinical signs of this relatively rare accidients. The conclusions repose on watchful precautions according to indications and technical way of barium enema examination in patients specially exposed to perforation accidents.

Local recurrence rates for colorectal carcinoma occur in the range from 20% to 35%. For this reason, postoperative radiotherapy was evaluated as a measure to achieve local disease control in the pelvis.
