Improving Sleep Quality for Persons on Hemodialysis in Iraq.

Insufficient and poor sleep quality significantly impacts the health and quality of life of persons with end-stage renal failure (ESRF) on hemodialysis. This study explored the effectiveness of teaching patients on hemodialysis, Benson's relaxation technique. Seventy-one persons on hemodialysis in Iraq were randomly assigned to either the experimental or the control group. The experimental group received training and encouragement to use the Benson relaxation technique. The Pittsburgh Sleep Quality Index (PSQI) score was collected for all the patients before and after the intervention. After 8

Curvilinear relations between insight-, cognition-, and skills-oriented technique use and outcome across treatments, or Goldilocks and the three psychotherapies.

OBJECTIVE: A "Goldilocks" effect is when "just right" conditions are present for a phenomenon to occur. In psychotherapy research, the Goldilocks effect is when moderate intervention levels (not very low or high) correlate with more improvement than very high or very low levels. Finding curvilinear relations requires the wider range of what a specific intervention can take, which can be seen when examining technique and outcome across different treatments rather than within a single therapy orientation. METHOD: For 151 patients with panic disorder (66% female, 33% racial/ethnic minority) in a

Randomized controlled trial investigating potential effects of relaxation on mitochondrial function in immune cells: A pilot experiment.

This study aimed to investigate the effect of a relaxation response induced by hypnosis on the mitochondrial energy production of immune cells compared to an everyday relaxing situation. Chronically stressed individuals (88% women) with at least moderate suggestibility were randomized to a hypnosis (20 min relaxation hypnosis; n = 20) or a control condition (20 min documentary; n = 22). Before and after intervention, peripheral blood was collected. The primary outcomes were mitochondrial respiration and density in immune cells measured by high-resolution respirometry and citrate synthase activ

Comparison of sedative effects of medetomidine-midazolam, acepromazine-butorphanol and midazolam-butorphanol in dogs.

Sedative effects of medetomidine at 20 micrograms/kg and midazolam at 0.3 mg/kg (MM), acepromazine at 0.05 mg/kg and butorphanol at 0.2 mg/kg (AB), and midazolam at 0.1 mg/kg and butorphanol at 0.2 mg/kg (MB) were compared in dogs. All dogs given MM were laterally recumbent within 11 min of the administration of the drugs, and this combination also induced deep sedation accompanied by analgesia and muscle relaxation, and strongly depressed arousal reactions to external stimuli. However, undesirable effects such as bradycardia and decreased respiration were also observed in dogs given MM. Six o

Lucid dreaming as a treatment for recurrent nightmares.

BACKGROUND: Lucid dreams occur when a person becomes aware that he or she is dreaming while still in the dream state. Previous reports on the use of lucid dreaming in the treatment of nightmares do not contain adequate baseline data, follow-up data, or both. METHODS: A treatment of recurrent nightmares incorporating progressive muscle relaxation, guided imagery, and lucid dream induction is presented for 2 case studies. Three other cases were treated with lucid dream induction alone. The duration of the nightmares ranged from once every few days to once every few months. RESULTS: The procedu

Prediction of response to biofeedback-assisted relaxation in hypertensives: development of a Hypertensive Predictor Profile (HYPP).

A group of hypertensive patients who participated in a biofeedback-assisted relaxation program were divided into treatment successes and treatment failures based on the change in their blood pressure. Multiple regression analysis was used to characterize the successes and failures and to develop a hypertensive predictor profile. Hypertensives most likely to lower their blood pressure with biofeedback-assisted relaxation are those in whom there is evidence of autonomic overactivity, for example, cool hands, high heart rates, and evidence of a chronic response to stress, such as high anxiety sco

Self-control of psychophysiologic response to motion stress: using biofeedback to treat airsickness.

Investigators of the Neuropsychiatry Branch, Clinical Sciences Division, U.S. Air Force School of Aerospace Medicine (USAFSAM), provided biofeedback-moderated behavioral treatment to 53 fliers grounded for chronic, severe motion sickness, and followed each flier for 2 years after treatment completion. Success was defined as returning to and maintaining satisfactory operational flying status. Of these, 42 fliers (79%) met this criterion; 3 (6%) were partially successful, and 8 (15%) were subsequently grounded for recurrent airsickness. Follow-on studies will investigate psychophysiological mech

Relation between outcome and the patient's initial experience of the therapist and therapeutic conditions in four treatment methods for phobic disorders.

One hundred and three phobic women not suitable for insight-oriented psychotherapy were randomized to either therapist-directed prolonged exposure therapy (PE), supportive therapy of dynamic type (ST), relaxation therapy (R), or basal therapy (B). Immediately after seeing their therapist for history taking and beginning of treatment, the patients were asked to report their experience of the therapist, of themselves in relation to the therapist, and of the therapy (Experience of Therapy Inventory, ETI). The patients were informed that the therapists would not see the ETI. The patients were rate…

'Cool and quiet' therapy for malignant hyperthermia following severe traumatic brain injury: A preliminary clinical approach.

Malignant hyperthermia increases mortality and disability in patients with brain trauma. A clinical treatment for malignant hyperthermia following severe traumatic brain injury, termed 'cool and quiet' therapy by the authors of the current study, was investigated. Between June 2003 and June 2013, 110 consecutive patients with malignant hyperthermia following severe traumatic brain injury were treated using mild hypothermia (35-36°C) associated with small doses of sedative and muscle relaxant. Physiological parameters and intracranial pressure were monitored, and the patients slowly rewarmed fo

A functional coupling of TRPA1 and TMEM16A channels regulates endothelium-dependent hyperpolarization-mediated vasorelaxation of mesenteric resistance arteries in healthy and septic mice.

Vascular endothelial dysfunction drives sepsis progression and multi-organ failure. Although TRPA1 and TMEM16A are expressed in the vasculature, their roles in arteriolar vasorelaxation remain unclear. The vasorelaxant effects of TRPA1 and TMEM16A on the second-order branch of mesenteric resistance arteries were investigated in healthy and septic mice using a Mulvany-style wire myograph. Fluorescent Ca2+ assays were employed to assess cytosolic Ca2+ ([Ca2+]i) in vascular endothelial cells (VECs). A mouse sepsis model was constructed through the method of cecal ligation and puncture (CLP). Both

Downregulation of Ca2+ signalling proteins in cardiac hypertrophy.

Calcium (Ca2+) signaling plays an essential role in several functions of cardiac myocytes. Transient rises and reductions of cytosolic Ca2+, permitted by the sarcoplasmic reticulum Ca2+ ATPase and other proteins, control each cycle of contraction and relaxation. Prolonged rises of cytosolic Ca2+ are involved in transcriptional activation, including the hypertrophy program. Furthermore, activation of transcriptional pathways produced by excitation of membrane receptors and involving Protein Kinases C and D, calcineurin, mitogen-activated protein kinases and glycogen synthase kinase 3b, generate

Bitter taste receptors on airway smooth muscle bronchodilate by localized calcium signaling and reverse obstruction.

Bitter taste receptors (TAS2Rs) on the tongue probably evolved to evoke signals for avoiding ingestion of plant toxins. We found expression of TAS2Rs on human airway smooth muscle (ASM) and considered these to be avoidance receptors for inhalants that, when activated, lead to ASM contraction and bronchospasm. TAS2R agonists such as saccharin, chloroquine and denatonium evoked increased intracellular calcium ([Ca²(+)](i)) in ASM in a Gβγ-, phospholipase Cβ (PLCβ)- and inositol trisphosphate (IP₃) receptor-dependent manner, which would be expected to evoke contraction. Paradoxically, bitter tast

Manometric characterization of rectal dysfunction following radical hysterectomy.

Bladder dysfunction thought to be due to partial denervation has been described following radical hysterectomy. Some patients experience acute and chronic rectal dysfunction characterized by difficulty with defecation and loss of defecatory urge. To define this abnormality, anorectal pressure profiles were examined in 15 patients with Stage I carcinoma of the cervix before and after radical hysterectomy. Profiles were done using standard anorectal manometry with a water-infused system. In all patients preoperative manometric profiles were normal; postoperative studies were abnormal in all pati

Stretching-Positioned Extracorporeal Shock Wave Therapy Yields Superior Short-Term Efficacy for Post-Stroke Upper-Limb Spasticity: A Retrospective Comparative Study.

BACKGROUND Extracorporeal shock wave therapy (ESWT) reduces post-stroke spasticity, but the optimal muscle position during application is unclear. We hypothesized that ESWT applied under muscle stretch would yield superior anti-spastic effects. This study aimed to compare the short-term efficacy of ESWT on the biceps brachii under stretching versus relaxation postures. MATERIAL AND METHODS This retrospective comparative study analyzed data from 96 stroke patients categorized into Group A (ESWT under stretching, n=48) or Group B (ESWT under relaxation, n=48). Outcome measures included the Modif

Biaxial Stress Relaxation of Vaginal Tissue in Pubertal Gilts.

Pelvic organ prolapse (POP) is a condition characterized by displacement of the vagina from its normal anatomical position leading to symptoms such as incontinence, physical discomfort, and poor self-image. Conservative treatment has shown limited success and surgical procedures, including the use of mesh, often lead to severe complications. To improve the current treatment methods for prolapse, the viscoelastic properties of vaginal tissue need to be characterized. We determined the biaxial stress relaxation response of vaginal tissue isolated from healthy pubertal gilts. Square specimens (n

Understanding User Reactions and Interactions With an Internet-Based Intervention for Tinnitus Self-Management: Mixed-Methods Evaluation.

Purpose Internet-based interventions have the potential to reduce the disparity in access to psychological therapy that people with tinnitus currently experience. One example is the Tinnitus E-Programme, which, although freely available online, has not yet been formally evaluated. The purpose of this study was to evaluate past, current, and new users' reactions and interactions with the Tinnitus E-Programme. Method Study 1 used an online survey to gather past and current users' reactions to, and interactions with, the intervention (n = 27). Study 2 used interviews and a relaxation log to asses
