Sudden visual loss in pseudotumor cerebri due to central retinal artery occlusion.

A patient with a 16-month history of papilledema from pseudotumor cerebri had sudden loss of vision in one eye because of a central retinal artery occlusion (CRAO). The CRAO was transient and visual acuity improved from light perception to 6/7.5 OS in long-term follow-up. A visual field abnormality and color-vision deficit have persisted. Both eyes demonstrated peripapillary neovascular membranes. The eye with CRAO had extensive chorioretinal folds extending from the disc through the macula. Thorough investigations failed to demonstrate a coagulation abnormality, hyperviscosity, or source of e…

Salmon-patch hemorrhages after central retinal artery occlusion in sickle cell disease.

In two patients with sickle cell disease (one hemoglobin SC and one hemoglobin SS), central retinal artery occlusions developed. In one case, the occlusion followed a retrobulbar injection of lidocaine hydrochloride. Although the central retinal artery reperfused in each patient, many secondary peripheral retinal arteriolar occlusions remained. During the subsequent days, multiple salmon-patch hemorrhages developed in the distribution of these occluded arterioles. In one patient, the salmon-patch hemorrhages evolved into atrophic schisis cavities. These unusual cases allowed us to document the…

The c-wave of the human electroretinogram in central retinal artery occlusion.

The pigment epithelium-receptor complex, which is supplied from the choroidal circulation, is the main source of the c-wave of the ERG. Occlusion of the central retinal artery (OCRA), which causes serious damage to the inner retinal layers, should therefore not primarily be expected to affect the c-wave amplitude. Nevertheless, conflicting reports of increased, decreased and unchanged c-waves have appeared in the literature. In the present study on four patients with OCRA the affected eyes showed diminished b- and c-waves. On the other hand, both the a-wave and the trough between the b- and c-…

Combined Central Retinal Artery Occlusion (CRAO) and Central Retinal Vein Occlusion (CRVO) in a Celiac Disease Patient: A Case Report.

Celiac disease (CD) is a digestive disorder caused by an abnormal immune reaction to gluten, leading to severe malabsorption syndrome. Central retinal vein occlusion (CRVO) was reported in a couple of cases worldwide in patients with this disease entity. Herein, we are reporting a rare case of combined central retinal vein and artery occlusion in a young female celiac disease patient presented with a counting finger vision at six feet and improved to 20/60 vision after conservative management.

Retinal OCT findings in acute central retinal artery occlusion of varying severity at different disease stages - a retrospective, observational study.

PURPOSE: To study the optical coherence tomography (OCT) changes in eyes with acute central retinal artery occlusion (CRAO) of different severity and at different disease stages. METHODS: The study included acute CRAO cases of < 7 days duration, imaged on OCT at various time points. Based on the OCT findings at presentation, cases were classified into three severity groups: mild, moderate, and severe. OCT scans were evaluated and classified into four-time intervals based on symptom duration. RESULTS: There were 39 eyes from 38 patients with acute CRAO who underwent 96 OCT scans. At presentat…

Similarities and differences in systemic risk factors for retinal artery occlusion and retinal vein occlusion: A nationwide case-control study.

BACKGROUND: To investigate the relationship between risk factors for retinal artery occlusion (RAO) and retinal vein occlusion (RVO) and thereby identify similarities and differences between the two types of retinal vascular occlusions. METHODS: In this case-control study, 5708 patients with RAO were included and matched with three patients with RVO each. The patients with RVO were matched on sex and age at index date. All patients, personal information, diagnoses, and prescriptions were obtained from the Danish nationwide registries. Adjusted conditional logistic regression was used to inves…

Branch retinal artery occlusion and non-ischemic central retinal vein occlusion due to hyperhomocysteinemia in a 14-year-old child.

A 14-year-old girl presented with sudden, painless loss of vision in the left eye. Complete ophthalmologic examination including fluorescein angiography revealed an impending central vein occlusion and a branch retinal artery occlusion inferotemporally. One month later, there was a non-ischemic central retinal vein occlusion of the same eye. Systemic evaluation led to the diagnosis of hyperhomocysteinemia. This case report underscores the importance of excluding hyperhomocysteinemia in vascular occlusive disease.

Retinal capillary rarefaction is associated with arterial and kidney damage in hypertension.

Microvascular disease and rarefaction are key pathological hallmarks of hypertension. The retina uniquely allows direct, non-invasive investigation of the microvasculature. Recently developed optical coherence tomography angiography now allows investigation of the fine retinal capillaries, which may provide a superior marker of overall vascular damage. This was a prospective cross-sectional study to collect retinal capillary density data on 300 normal eyes from 150 hypertensive adults, and to investigate possible associations with other organ damage markers. The average age of participants was

[Successful Systemic Lysis Therapy in Acute Retinal Arterial Occlusions].

BACKGROUND: Retinal artery occlusion leads to dramatic and irreversible vision loss. There is currently no evidence-based standard therapy. According to the German guidelines on retinal artery occlusions, intravenous fibrinolysis therapy can be performed up to a time window of 4 h 30 min. METHODS: Two patients were treated accordingly. RESULTS: In patient 1, systemic lysis therapy was used in branch retinal artery occlusion (BRAO) of the inferior temporal retinal artery with macular involvement 4 h 15 min after symptom onset. Immediately after the therapy, the patient reported significant im…

Angiographic features and disease outcomes of symptomatic retinal arterial macroaneurysms.

PURPOSE: Symptomatic retinal arterial macroaneurysms (RAM) are primarily investigated by fundus fluorescein angiography after presenting with visual disturbance. The natural history includes spontaneous regression and occasionally occlusion of the arteriole distal to the aneurysm. RAM may be managed conservatively. Interventional treatment options include focal argon laser photocoagulation, Nd:YAG laser hyaloidotomy, and pars plana vitrectomy. The purpose of this study was to elicit the rates of distal vessel occlusion and aneurysm thrombosis in RAM at presentation, and their relevance to the
