The authors report a case of a 58 year old male presenting with bilateral ureteral compression by peri-aneurysmal retroperitoneal fibrosis. A ureterolysis and prosthetic replacement of the abdominal aorta gave good results. Eighteen months postoperatively, proteinuria and an edema of the lower limbs revealed an extramembranous glomerulitis, for which no cause could be found. Was this rare association fortuitous, or does it result from the action of some unknown antigen?

We report 4 patients with surgically proved idiopathic retroperitoneal fibrosis. Retroperitoneal fibrosis was diagnosed correctly in 3 of the patients by computerized tomography. On the computerized tomogram, retroperitoneal fibrosis demonstrated a characteristic soft tissue mass enveloping the vessels and ureters. In 2 patients, the follow-up computerized tomogram revealed spontaneous resolution of the mass in the retroperitoneal space. Computerized tomography is one of the most useful methods in the preoperative evaluation and postoperative follow-up of this disease.

Immediately after ureterolysis in the treatment of idiopathic retroperitoneal fibrosis, we covered the ureter with a large epiploon, forming a sheath with omentum covering the entire ureter from the renal pelvis to the juxtavesical area of the same. This technique revitalizes the ischemic ureter resulting from ureterolysis and protects against the reinclusion of the ureter mobilized inside the sclerolipomatous mass.

A patient with advanced renal failure and bilateral exophthalmos was found to have urinary obstruction caused by retroperitoneal fibrosis, as well as bilateral orbital pseudotumors. Pseudotumor of the orbit, especially when bilateral, has been associated with a wide variety of systemic conditions. Although the association of pseudotumor and retroperitoneal fibrosis is uncommon, any patient with bilateral orbital pseudotumor and renal insufficiency should be examined for the possible presence of a treatable urinary obstruction.

A case of avascular necrosis of the head of the femur is reported. The patient is known to suffer from retroperitoneal fibrosis. The possibility of an association between these two conditions is discussed.

We present a case of idiopathic retroperitoneal fibrosis (RPF) in a female patient of 45 years, obese (BMI = 39 kg/sqm), hypertensive since 2005, with diabetes mellitus treated with diet and diabetes insipidus in whom, during a routine control, the following has been found: serum creatinine 1.74 mg/dl, and an inflammatory syndrome associated with fever.
