Scalp Dysesthesia: Emerging Pathophysiologic Insights and Therapeutic Implications.

Scalp dysesthesia is a poorly understood condition characterized by chronic scalp pruritus and pain in the absence of underlying cutaneous disease. Delayed diagnosis, limited clinician awareness, and inconsistent therapeutic responses continue to complicate management. This scoping review aimed to synthesize current evidence regarding the clinical presentation, diagnosis, and treatment of scalp dysesthesia. A comprehensive review of the literature was conducted, with data extracted from 20 studies describing 59 patients, the majority of whom were middle-aged and older women. Common comorbiditi…

Uncommon non-traumatic scalp entities: a pictorial review.

Non-traumatic emergent scalp abnormalities are uncommon and may be easily overlooked on routine neuroimaging. However, by nature of its superficial anatomic location, the scalp is frequently the first site of physical abnormality noted by patients. Although scalp pathology is rarely the primary indication for obtaining cross-sectional imaging, the rich vascular supply to the scalp means that this region is commonly linked to disease processes in the skull, intracranial vasculature, and more distant anatomical structures. In this pictorial essay, we present a unique spectrum of scalp lesions en…

YOLOv7-based automated detection platform for scalp lesions.

OBJECTIVE: In dermatological research, the focus on scalp and skin health has intensified, particularly regarding prevalent conditions like dandruff and erythema. This study aimed to utilize YOLOv7 model to develop an automated detection web-based system for these specific scalp lesions. METHODS: Utilizing a dataset of 2200 clinical images, the model's accuracy and robustness were assessed. The raw images were initially preprocessed by the Roboflow tool. We then trained and evaluated the YOLOv7 model, comparing its performance with several baseline models including YOLOv5, YOLOF, and the sing…

Infantile Scalp Hemangiomas: Surgical Management and Lessons Learned.

Infantile hemangiomas (IH) are common benign vascular tumors in pediatric patients, often found on the scalp. While most IH naturally regress without intervention, surgical excision becomes necessary when severe anatomic or physiological complications arise. This review examines previously published case studies on pediatric scalp IH excisions, focusing on surgical outcomes and complications. A comprehensive search of the US National Library of Medicine National Institutes of Health (PubMed) database identified 19 relevant case studies. Most patients were female, with a wide age range at the t…

Scalp Seborrheic Dermatitis: What We Know So Far.

Scalp seborrheic dermatitis (SSD) is a prevalent chronic, relapsing inflammatory skin disease. The etiology is related to sebum production, bacterial proliferation - Staphylococcus sp., Streptococcus, and M. restricta - and host immunity factors - NK1+, CD16+ cells, IL-1, and IL-8. Trichoscopy features include mostly arborizing vessels and yellowish scales. New trichoscopic findings were described to guide the diagnosis as dandelion vascular conglomerate, "cherry blossom" vascular pattern, and intrafollicular oily material. Antifungals and corticosteroids constitute the essential therapy, but …

Traumatic intradiploic epidermoid cyst manifest as scalp papule.

A posterior fossa intradiploic epidermoid cyst was removed from the calvarium of an otherwise healthy 34-year-old woman after work up for dizziness and blurry vision. The patient admitted to a history of a firm papule that occurred occasionally after a trauma sustained to the region years prior. The authors propose that trauma and cutaneous stigmata are potential associations with intradiploic epidermoid cysts.

Experience of fetal scalp blood sampling during labor.

Fetal scalp blood sampling (FBS) is often claimed to be painful for women in labor and difficult for obstetricians to perform. Our aim was to assess women's experience of pain during FBS and obstetricians' experience of difficulty in performing the test. At a tertiary center in Sweden, a questionnaire with answers on a 10-point scale was completed by 51 women and the obstetricians performing the test. Women's experience of pain had a median of 3.5. FBS was well tolerated in women who had epidural analgesia but might be associated with pain in women without. Higher maternal body mass index and …

Mutations in KCTD1 cause scalp-ear-nipple syndrome.

Scalp-ear-nipple (SEN) syndrome is a rare, autosomal-dominant disorder characterized by cutis aplasia of the scalp; minor anomalies of the external ears, digits, and nails; and malformations of the breast. We used linkage analysis and exome sequencing of a multiplex family affected by SEN syndrome to identify potassium-channel tetramerization-domain-containing 1 (KCTD1) mutations that cause SEN syndrome. Evaluation of a total of ten families affected by SEN syndrome revealed KCTD1 missense mutations in each family tested. All of the mutations occurred in a KCTD1 region encoding a highly conser…

Scalp metastasis from malignant fibrous histiocytoma of bone.

Malignant fibrous histiocytoma (MFH) of bone is a relatively rare primary malignant neoplasm and an aggressive tumor, characterized by a hig … med a striform-pleomorphic type of MFH of bone. Six months postoperatively, the patient developed a scalp metastasis. Wide local excision was performed. Six months after the scalp operation, the patient was alive with no further relapse. We should keep the skin in mind as the possible initial site of metastatic MFH of bone.

Fine needle aspiration cytology of metastatic scalp nodules.

OBJECTIVE: To analyze cases of palpable cutaneous/ subcutaneous scalp nodules and evaluate the clinical features and role of fine needle aspiration cytology (FNAC) in their diagnosis. STUDY DESIGN: A total of 1,022 cases of metastatic malignancies at various sites were analyzed over a period of two years. FNAC was performed as a routine outpatient procedure for palpable lesions and under ultrasound or computed tomographic guidance for nonpalpable, internal sites. Histologic correlation was done when possible. RESULTS: Eighteen cases were found to be metastatic scalp nodules. The scalp as a m…

Scalp flap rotation with primary donor site closure.

A 57-year-old man with a 7-cm defect of the left temple underwent reconstruction with a large scalp rotation flap that shifted the donor defect to the back of the neck so that it could be closed primarily. This approach, also used successfully in 7 other patients with similar defects, avoided the need for an unsightly skin graft in the donor site while providing tissue with a color and texture that blended well with the surrounding forehead and facial skin. Primary closure of the donor site reduces scalp flap morbidity and improves the overall aesthetic outcome.
