INTRAMUSCULAR TERLIPRESSIN AS MONOTHERAPY FOR EARLY MANAGEMENT OF UNCONTROLLED TRAUMATIC HEMORRHAGIC SHOCK: A PRELIMINARY STUDY.

Monotherapy using a single-dose vasopressor to maintain essential organ perfusion could be a viable alternative to restrictive fluid resuscitation in uncontrolled traumatic hemorrhagic shock (UTHS). This study evaluated the impact of intramuscular terlipressin on hemodynamics and organ injuries in UTHS. Adult male Sprague-Dawley rats, with eight rats in each group, were assigned to six distinct groups. Immediately after randomization, the rats in each group were exposed or not exposed to specific interventions over a 60-min period, as follows: sham, shock, intramuscular terlipressin (5 μg/100

Effect of sodium bicarbonate Ringer's solution on lung injury in rats with traumatic hemorrhagic shock.

This study aimed to explore the impact of different pH values of resuscitation fluid on traumatic hemorrhagic shock (THS), focusing on their effects on glycocalyx and inflammation. A rat model of THS was induced by hemorrhage from a left femur fracture, while an oxygen-glucose deprivation/reoxygenation (OGD/R)-induced HULEC-5a cell model was considered as an in vitro THS model. The lung tissue pathology and glycocalyx structure were assessed through hematoxylin-eosin (H&E) staining and transmission electron microscope examination. The levels of glycocalyx-related factors and inflammation-relat

Revisiting the "Golden Hour": An Evaluation of Out-of-Hospital Time in Shock and Traumatic Brain Injury.

STUDY OBJECTIVE: We evaluate patients with shock and traumatic brain injury who were previously enrolled in an out-of-hospital clinical trial to test the association between out-of-hospital time and outcome. METHODS: This was a secondary analysis of patients with shock and traumatic brain injury who were aged 15 years or older and enrolled in a Resuscitation Outcomes Consortium out-of-hospital clinical trial by 81 emergency medical services agencies transporting to 46 Level I and II trauma centers in 11 sites (May 2006 through May 2009). Inclusion criteria were systolic blood pressure less th

[Strategy of the metabolic acidosis correction and indices of coagulation profile in patients with severe traumatic shock].

The strategy for correction of metabolic acidosis and coagulation profile disorders in severe traumatic shock was adduced. There was established, that severe traumatic shock is characterized by prominent metabolic acidosis, the bases deficiency reduction, the lactate content in arterial blood enhancement, as well as ionized calcium in the blood serum, significant enhancement of the partially activated thromboplastin time and protrombin time, what witnesses the prominent coagulation disorders presence on the metabolic acidosis background. The "Soda-buffer" (manufactured by "Yuriya Pharm", Ukrai

Treatment of post-traumatic myositis ossificans of the anterior thigh with extracorporeal shock wave therapy.

OBJECTIVE: This case study demonstrates the effectiveness of a novel approach to the treatment of post-traumatic myositis ossificans with extracorporeal shockwave therapy in an elite athlete. CLINICAL FEATURES: A 20 year-old male semi-professional rugby player presented with progressive pain and loss of range of motion after sustaining a severe, right quadriceps contusion nine weeks earlier. The differential diagnosis of myositis ossificans was suspected and confirmed on radiographic examination. INTERVENTION AND OUTCOME: A two week treatment protocol was undertaken consisting of three sessi

[The role of desensitization of glucocorticoid receptors in the development of vascular resistance to endogenous vasoconstrictors in traumatic shock].

The fact that the activity of cytosol glucocorticoid receptors decreases in shock have been shown before [Golikov P. P. et al., 2001]. The connection between the development of vascular hyporeactivity to endogenous vasoconstrictors and desensitization of glucocorticoid receptors was studied in this investigation. On Kenton traumatic model in a rat experiment, it was shown that the strength of the isometric constriction of the isolated aorta in response to angiotensin II, endothelin-1, phenylephrine, noradrenaline, and vasopressin falls on the second day after a severe mechanical injury (3.3, 2

Hypertonic saline: A review of the literature and evaluation of its role in resuscitation of traumatic hypovolaemic shock.

Hypertonic saline solutions have properties that may be advantageous for resuscitation. In both animal and human studies rapid improvements in cardiovascular status occur when hypertonic saline is administered to hypovolaemic subjects and the possible side effects of hypertonic solutions have not been shown to be of clinical importance. Hypertonic saline certainly has a potential role in resuscitation of hypovolaemic casulaties in the modern military setting.

Invited opinion: role of nitric oxide in hemorrhagic, traumatic, and anaphylactic shock and thermal injury.

The free radical nitric oxide (NO) is synthesized from the guanidino group of L-arginine by a family of enzymes termed NO synthase (NOS). Hemorrhagic shock leads to an inhibition of NO production by the calcium-dependent, endothelial NOS (ecNOS), which may lead to maldistribution of blood flow leading to, e.g., coronary, renal, and cerebral ischemia and may enhance the adhesion of neutrophil granulocytes and platelets to the endothelial surface. Prolonged periods of hemorrhagic shock are associated with the induction of a calcium-independent isoform of NOS in a variety of organs and in the vas

[Dynamics of changes in pulmonary catecholamine content and lipid fatty acid composition in traumatic shock].

The content of noradrenaline in the lungs, pulmonary arteries and veins was found to be reduced at definite stages of traumatic shock. At the same time the level of oleic acids was decreased in pulmonary lipid fractions. The content of adrenaline rose in the pulmonary arteries and veins, and it was high in the blood collected from the right atrium as compared to the blood from the carotid artery. It is assumed that the changes in the content of fatty acids are linked with those occurring in lung catecholamine metabolism.
