Open plate fixation in displaced pediatric proximal humerus fractures is safe and leads to very good functional outcomes.

INTRODUCTION: In displaced pediatric proximal humerus fractures (PHF), surgical treatment ranges from closed to open procedures. Soft tissue interposition can impede closed reduction, making open techniques necessary. While K-wire fixation and elastic stable intramedullary nailing (ESIN) lead to good results, plate fixation could be an alternative in patients with limited growth potential and highly unstable or insufficiently retained fractures. Only few studies with low sample sizes have assessed plate fixation, yet. In this study, the outcome of pediatric PHFs treated with plate fixation was

Suture cerclage for stabilizing the humeral shaft during shoulder arthroplasty.

BACKGROUND: In shoulder arthroplasty, cerclage fixation techniques are used to stabilize osteotomies, fractures, and allografts. Fixation techniques including cerclage with metal and polymer cables have been described. The purpose of this study was to evaluate suture cerclage fixation of the humeral shaft during shoulder arthroplasty. MATERIALS AND METHODS: Shoulder arthroplasty cases performed from 2012-2017 by 3 fellowship-trained shoulder arthroplasty surgeons were reviewed. Cases in which suture cerclage was used for osteotomy, fracture, or allograft fixation were identified. Demographic

Osteoporosis and Implant-Related Complications After Anatomic and Reverse Total Shoulder Arthroplasty.

INTRODUCTION: Osteoporosis is a widespread and growing medical condition, with significant orthopaedic implications. However, the effect of osteoporosis on outcomes after total shoulder arthroplasty (TSA) is not well understood. The goal of the present study was to characterize the incidence of osteoporosis in patients undergoing shoulder arthroplasty and to examine whether patients with osteoporosis undergoing anatomic and reverse TSA are at an increased risk of prosthetic-related complications. METHODS: Complication rates were calculated for patients with osteoporosis who underwent anatomic

Nerve lesions in proximal humeral fractures.

In the literature nerve injury is not frequently considered a problem in proximal humeral fractures. Only a few studies exist concerning traction injury of nerves in fractures of the proximal humerus after low-velocity trauma. Almost all of them are retrospective and did not use electromyography. Patients with identical fractures can show quite different outcomes, which vary between complete recovery and severely limited shoulder function. On the assumption that nerve lesions can play a role in the recovery of conservatively and operatively treated proximal humeral fractures, we started a pros

Self-reducing proximal humerus fractures.

BACKGROUND: Proximal humeral fractures are common in the elderly and can present with multiple fracture configurations. Our case series describes a three-part valgus-impacted fracture pattern of the proximal humerus that underwent spontaneous reduction by nonsurgical management with good functional outcomes. METHODS: Seven patients who sustained low-energy three-part valgus-impacted fractures of the proximal humerus were studied. We retrospectively reviewed serial anteroposterior radiographs of each patient taken as part of routine follow-up. For clinical outcomes, patients were assessed by a

Shoulder dislocations in patients older than 40 years of age.

Shoulder dislocations in patients over 40 years of age are common and are frequently associated with serious injuries to adjacent structures. Associated injuries such as brachial plexus injury, rotator cuff tear, axillary artery injury, fractures about the shoulder, and recurrent dislocation can make shoulder dislocations challenging problems for the clinician. Early diagnosis and treatment of associated injuries and treatment complications are the mainstays in prevention of morbidity and, even, mortality.

Pediatric shoulder trauma.

Pediatric shoulder trauma is relatively uncommon. Injuries requiring surgical intervention are even rarer. However, it is important for the practicing orthopaedic surgeon to differentiate nonoperative injuries from the urgent and potentially operative injuries. Missing such an injury in the pediatric population could be potentially life threatening or lead to long-term disability. Posterior sternoclavicular dislocations should be differentiated from medial clavicular physeal injuries and promptly reduced. Open fractures or neurovascular threatening fractures should be attended to immediately.

Functional Outcomes of Intramedullary Interlocking Nailing for Humeral Shaft Fractures: A Prospective Study.

Background Humeral shaft fractures were frequent orthopedic injuries, typically caused by trauma, and the optimal treatment approach remained a subject of discussion. Intramedullary interlocking (IMIL) nailing was a minimally invasive surgical method that offered superior biomechanical stability, especially for diaphyseal fractures, while promoting early mobilization. This prospective study assessed the functional outcomes of IMIL nailing in managing humeral shaft fractures. Objectives This study aims to evaluate the functional outcomes of IMIL nails in treating closed humeral shaft fractures

Long-term patient-reported outcome after fractures of the clavicle in patients aged 10 to 18 years.

BACKGROUND: Fractures of the clavicle are common among adolescents and have traditionally been treated nonoperatively. Recent literature has demonstrated less satisfactory results than expected after conservative management of displaced fractures in adults. The purpose of this study was to evaluate the long-term patient-reported outcome after clavicle fractures in older children and adolescents. METHODS: Children aged 10 to 18 years who sustained a fracture of the clavicle between 2006 and 2008 were identified in our institution's computerized files. The radiographs were examined and the frac

Surgical management of 2- or 3-part proximal humeral fractures: comparison of plate, nail and K-wires.

BACKGROUND: Proximal humeral fractures (PHFs) are fairly common injuries, and their treatment is a challenge. The aim of this study is to compare clinical and functional outcomes of different osteosynthesis techniques. MATERIALS AND METHODS: We retrospectively reviewed patients' files and the hospital's digital database between March 2002 and April 2018. We treated surgically 148 patients with 2- and 3-part PHFs: 64 with plate and screws, 53 with intramedullary nailing and 31 with retrograde K-wires. We constituted three groups according to the type of treatment and two subgroups for each acc

Acute shoulder injuries.

The shoulder is the most mobile joint in the human body. The cost of such versatility is an increased risk of injury. It is important that family physicians understand the anatomy of the shoulder, mechanisms of injury, typical physical and radiologic findings, approach to management of injuries, and indications for referral. Clavicle fractures are among the most common acute shoulder injuries, and more than 80 percent of them can be managed conservatively. Humeral head fractures are less common and usually occur in elderly persons; 85 percent of them can be managed nonoperatively. Common acute

What is the postoperative prognosis of proximal humeral fractures in China? Protocol for a multicentre prospective cohort study (CAST).

INTRODUCTION: Proximal humeral fractures are increasingly common, particularly among older people, whereas the prognosis of surgical treatments for these fractures remains substantially uncertain. In China, where the ageing population is rapidly growing, high-quality prospective data on surgical outcomes, complications and cost-effectiveness are lacking. To address this gap, we propose to initiate a prospective, multicentre cohort study on surgical treatment for proximal humeral fractures in China-Cohort of Acute Shoulder Trauma (CAST) study. METHODS AND ANALYSIS: The CAST is a multicentre, p

[Anatomical fracture endoprosthesis-who and how?].

The aim of this review article is to present the indications for an anatomical fracture prosthesis, mostly as a shoulder hemiprosthesis (SHEP) and its value in comparison to other procedures. The healing of the tuberosities in the anatomical position and an intact rotator cuff are particularly important for the successful implantation of a SHEP after proximal humeral fractures. For older patients (> 70 years), the use of reverse shoulder arthroplasty achieves more reliable results and is associated with a lower revision rate. The indications for implantation of a SHEP in non-reconstructible pr

Acute locked superior shoulder dislocation in a patient with cuff tear arthropathy.

In the field of traumatic shoulder dislocations, this report of a 61-year-old female patient discusses the case of an acute locked superior shoulder dislocation in conjunction with a chronic rotator cuff arthropathy resulting from a low-energy fall on the outstretched and abducted arm. Radiological assessment revealed a complex combination of associated bony injuries including a fracture of the upper part of the glenoid and an impaction fracture of the inferior articular surface of the humeral head. Closed reduction and immobilisation were not successful in obtaining joint stability. This uniq
