Vocational options for those with sickle cell trait: questions about hypoxemia and the industrial environment.

Many patients have parents and siblings who possess the sickle cell trait and who often require not only genetic counseling, but also information about their own health. Some have been informed that they cannot pursue careers in aviation. Some have been told that they are at special risk for the hypoxemic effects of heavy exertion, especially the exertion associated with sports. A few believe that certain elements of the industrial environment are unusually hazardous to them. An examination of the literature reveals little evidence in support of any of these beliefs. Most, if not all, unfavora…

Growth and development in children with sickle-cell trait. A prospective study of matched pairs.

To ascertain if sickle-cell trait (Hb AS) impairs physical growth and cognitive development, we prospectively investigated 50 matched pairs of black children. For each child with Hb AS, an Hb AA child was matched at birth for sex, birth date, birth weight, gestational age, five-minute Apgar score and socioeconomic status. Between the ages of three and five years, the members of each matched pair were evaluated, with one month of one another, by persons "blind" to the hemoglobin genotype. Twelve outcome measurements were obtained at evaluation: height, weight, head circumference, skin-fold thic

An up-date on the prevalence of sickle cell trait in Eastern and Western Uganda.

BACKGROUND: The first survey on sickle cell disease (SCD) done in Uganda in 1949, reported the district of Bundibugyo in Western Uganda to have the highest sickle cell trait (SCT) prevalence (45%). This is believed to be the highest in the whole world. According to the same survey, the prevalence of SCT in the districts of Mbale and Sironko in the East was 20-28%, whilst the districts of Mbarara and Ntungamo in the West had 1-5%. No follow-up surveys have been conducted over the past 60 years. SCA accounts for approximately 16.2% of all pediatric deaths in Uganda. The pattern of SCT inheritanc…

Pregnant women with the sickle cell trait are not at increased risk for developing preeclampsia.

The primary objective of this study was to determine whether having the sickle cell trait is independently associated with preeclampsia. We performed a retrospective cohort study of 1998 pregnant patients who either did or did not have the sickle cell trait. All patients were screened for the sickle trait using the "Sickledex" test. Data on neonatal and maternal outcome, including preeclampsia, and potential confounding variables were abstracted from medical records. Unadjusted, stratified, and multiple logistic regression analyses were used to identify interactions, and confounding between mu…

Plasma, erythrocyte and urinary selenium levels in sickle cell homozygotes and traits.

Plasma, erythrocyte and urinary selenium levels were determined in 21 sickle cell homozygote patients, 20 siblings with sickle cell traits, 29 parents and in 21 healthy controls with an HbAA pattern. The mean levels of plasma and erythrocyte selenium and the daily urinary selenium depletion were found to be lower in the HbSS patients than in the controls (p < 0.02; p < 0.001; p < 0.001, respectively). Urinary selenium depletion was found to be lower in the sickle cell trait siblings than in the controls (p < 0.001), but the plasma and erythrocyte selenium levels were close to normal values (p …

Feasibility of peripheral blood stem cell collection from sickle cell trait donors with an intensified G-CSF regimen.

OBJECTIVES: Allogeneic hematopoietic stem cell transplantation (HSCT) is the only curative treatment for SCD and bone marrow from an HLA-matched sibling is currently the standard of care. Haploidentical HSCT from a family donor with a TCR αβ/CD19 depleted graft (T-haplo) is an increasingly successful alternative, which requires the generation of G-CSF stimulated peripheral stem cell (PBSC) from haploidentical relatives. These sickle cell trait (SCT) donors reported to develop SCD-related complications in conditions of severe stress. METHODS: In this retrospective analysis, we compared the saf…

Phenome- and laboratory-wide meta-analyses of sickle cell trait reveal multi-system disease associations.

Sickle cell trait (SCT) is increasingly recognized as a risk factor for adverse health outcomes. We utilized three large US electronic health record-based biobanks (Vanderbilt University Medical Center's BioVU, Penn Medicine Biobank, and All of Us) to conduct phenome-wide association (PheWAS) and clinical laboratory-wide association (LabWAS) meta-analyses of 4,813 individuals with SCT among 58,830 African genetic ancestry participants (∼60% female). Significant associations were replicated using a published PheWAS of SCT from the Million Veteran Program. Our PheWAS meta-analysis confirmed the …

A Rare Case of Mononucleosis-Induced Splenic Infarction in a Patient With Sickle Cell Trait.

Splenic infarction is a rare complication of Epstein-Barr virus (EBV) mononucleosis, particularly in patients with underlying hematologic conditions. We present a rare case of EBV-associated splenic infarction in a 20-year-old male with sickle cell trait (SCT), highlighting the potential overlap between SCT and infection-related ischemic complications. The patient presented to the emergency department with one week of left upper quadrant pain, fever, night sweats, dyspnea, and concentrated urine following suspected dehydration after outdoor activity. Initial Monospot testing was negative. CT i…

Risk and Protective Factors for COVID-19 Infection among Pregnant Women with Sickle Cell Trait.

Pregnant women and individuals with sickle cell trait (SCT) and underlying comorbidities are both independently more vulnerable to severe illness from coronavirus disease 2019 (COVID-19) compared to nonpregnant women and those without SCT. However, our understanding of the specific factors influencing susceptibility to COVID-19 infection among pregnant women with SCT is currently constrained by limited available data. This study aims to determine the risk and protective factors that influence the likelihood of COVID-19 infection in this population. A retrospective analysis was done among 151 w…

A Rare Case of Progressive Encephalopathy in a Sickle Cell Trait Patient: A Case Report.

Fat embolism syndrome (FES) is one of the underdiagnosed and underrecognized complications that can happen in multiple medical and surgical conditions. FES can manifest in a broad spectrum of signs and symptoms and affect multiple organ systems in the human body. One of the most commonly involved is the central nervous system (CNS), mainly the brain, which can be involved in different ways, and the presenting symptoms can vary in type and severity. One of the most common causes of FES is trauma, mainly a long bone fracture or any orthopedic injury. However, one of the rare causes of FES is sic

Single-center experience of LVAD implantation in patients with sickle-cell trait: A retrospective analysis.

BACKGROUND: The most worrisome complications in patients supported by left ventricular assist device (LVAD) are pump thrombosis, embolism, and bleeding. The actual rate of these events in patients with sickle-cell disease (SCD) has not well investigated. The aim of our study is to evaluate the outcomes of LVAD implantation in patients with sickle-cell hemoglobinopathy at our institution. METHODS: This retrospective, observational, single-center study was conducted on patients with sickle-cell trait (SCT), who underwent LVAD implantation using the HeartMate3 LVAD. RESULTS: LVAD devices were i
