Since the cranial nerves and the base of the skull are spatially close to each other, certain complexes of symptoms and signs give a very accurate indication of the possible location of the lesion. Basing on such neurologic-topical structuralisation, lesions of the cerebrellopontine angle, the foramen jugulare, the clivus and of the foramen magnum are discussed systematically. The various techniques of radiological imaging or examination are assessed in accordance with their diagnostic ranking, and characteristic constellations of findings are described.

One thousand patients had both CT of the head and a conventional skull series of radiographs. Radiographic findings were abnormal in 250 patients (25%), but only 64 patients (6.4%) had diagnostically significant abnormalities at radiography that were not detected by CT. A case of traumatic thrombosis of the internal carotid artery at the base of the skull is reported, and 23 similar cases collected from the literature. This review shows that 85% of the patients either died or were left with major neurological deficit, and only 15% recovered completely. There were two primary sites for the onset of thrombosis. One in the petrous pyramid and usually associated with basal skull fracture, and the other immediately distal to the posterior communicating artery in patients with minor head trauma.

Jugular venography done to evaluate abnormalities at the base of the skull demonstrated three distinctly different patterns depending on whether there is occlusion, invasion, or growth with in the internal jugular vein.
