We analysed the blood pressure, natriuretic and hormonal responses of 378 normal volunteers and 198 essential hypertensives after an intravenous infusion of 2 litres of isotonic saline and after a low-sodium diet and furosemide. Those in whom mean arterial blood pressure decreased by greater than 10 mmHg after sodium and volume depletion were considered sodium-sensitive, and those in whom a decrease of less than 5 mmHg was observed were considered sodium-resistant. The effect of sodium hyaluronate (Healon) on the post-operative course and IOP 1 week and 1 month after trabeculectomy is reported. Central pontine myelinolysis (CPM) was found at autopsy in 21 of 220 consecutive patients with chronic liver disease. It showed the same incidence in chronic nonalcoholic as in chronic alcoholic liver disease but did not occur in acute liver disease. The molecular weight of sodium hyaluronate in human amniotic fluid was determined by gel chromatography. Pooled samples from 16 weeks of gestation exhibited a broad molecular weight distribution (Mw = 3 X 10(5); Mn = 6 X 10(4)). Samples taken at 40 weeks showed a low molecular weight fraction (M less than 10(5) presumably of fetal origin, and a high molecular weight fraction (M greater than 10(6) which varied considerably, indicating a nonfetal origin. We examined the effectiveness of sodium fluoride in treatment of cochlear otospongiosis in ninety-four patients with cochlear otosclerosis and ninety-eight patients with stapedial otosclerosis and sensorineural hearing loss. In 21 patients with acute hypophosphatemia (AHPP), we measured red cell intracellular Na concentration (Nai) and either total (T), ouabain-sensitive (OS) or ouabain-resistant (OR) efflux rate constant of Na (0K Na) in a 'physiological' Na medium. High Nai, normal 0K Na T, reduced 0K Na OS and normal 0K Na OR were found. Total OS and OR unidirectional Na effluxes were increased, normal and increased, respectively. Diclofenac sodium was investigated in the treatment of juvenile rheumatoid arthritis (JRA). The pharmacokinetics of diclofenac in children aged 2-7 was assessed. Seven patients were included in a single-dose trial to determine plasma levels and renal elimination of diclofenac sodium. Venous blood samples were taken at 0, 0.5, 1, 2, 4 and 6 hours after administration of a 25 mg enteric-coated Voltaren tablet. It has previously been reported that sodium valproate (Epilim) lowers plasma ACTH levels in Nelson's syndrome. This report describes further experience with its use. Ten patients with Nelson's syndrome were treated with sodium valproate (600-1200 mg/day) for 5-32 weeks. Plasma ACTH was measured by cytochemical methods and RIA.
