This article is the final response in a scientific forum on the optimal intensity of intervention in speech-language pathology. It is a reflection on the state of knowledge offered by the 13 commentaries in this issue, addressing the areas of early communication and language impairment, speech sound disorders in children, emergent literacy, reading, aphasia, dysphagia, stuttering, motor speech disorders, voice disorders, and traumatic brain injury. Although more intense intervention can lead to better outcomes, the relationship between intensity and outcome is not always linear. More is not al…

A popular hypothesis holds that specific reading disability (SRD) and specific language impairment (SLI) result from an impaired ability to process rapid and brief sounds. However, the results of experiments that have tested this hypothesis are incongruous. A number of factors could explain these contradictory findings, including the questionable reliability and validity of rapid auditory processing tasks, individual differences in the auditory processing abilities of SRD and SLI populations, the age of listeners, the quality of control groups, and the relationship between verbal and non-verba

BACKGROUND AND PURPOSE: Acquired apraxia of speech (AOS) is a motor speech disorder caused by brain damage. AOS often co-occurs with aphasia, a language disorder in which patients may also demonstrate speech production errors. The overlap of speech production deficits in both disorders has raised questions on whether AOS emerges from a unique pattern of brain damage or as a subelement of the aphasic syndrome. The purpose of this study was to determine whether speech production errors in AOS and aphasia are associated with distinctive patterns of brain injury.
METHODS: Forty-three patients wit

PURPOSE: Reporting of outcome and experience measures is critical to our understanding of the effect of intervention for speech sound disorders (SSD) in children. There is currently no agreed-upon set of measures for reporting intervention outcomes and experiences. In this article, we introduce the Speech Outcome Reporting Taxonomy (SORT), a tool designed to assist with the classification of outcome and experience measures. In a systematic search and review using the SORT, we explore the type and frequency of these measures reported in intervention research addressing phonological impairment i

Like other areas of speech-language pathology, the behavioural management of voice disorders lacks precision regarding optimal dose-response relationships. In voice therapy, dosing can presumably vary from no measurable effect (i.e., no observable benefit or adverse effect), to ideal dose (maximum benefit with no adverse effects), to doses that produce toxic or harmful effects on voice production. Practicing specific vocal exercises will inevitably increase vocal load. At ideal doses, these exercises may be non-toxic and beneficial, while at intermediate or high doses, the same exercises may a

Determining the optimal amount of intervention is possibly the biggest challenge facing speech-language pathologists (SLPs) today. Baker (2012) has provided an erudite and pithy summary of the relevant literature in the field of optimizing intervention outcomes, and proposed a conceptual framework to measure all the inputs and acts that may contribute to the algorithm of intervention intensity. In the following article, two issues are discussed: first, that the use of technological advances to increase intensity should focus on everyday communication outcomes and, secondly, that measuring the

In response to Baker (2012) unpacking critical components of optimal intervention intensity, this article explores what is currently known regarding intervention intensity for emergent literacy for children at risk for delays in this area of development. Studies specifically addressing intervention intensity related to phonological awareness and print knowledge suggest that more intensity does not always yield better results, and other factors suggested by Baker may influence the impact of intensity on children's outcomes. However, none of the research to date includes speech-language patholog

There is sufficient evidence in the literature related to speech-language pathology, physiotherapy, occupational therapy, and psychology to indicate that intensive therapy is required in order to influence the neurophysiological basis of various impairments. Conversely, the influence of therapy on reducing communication restriction, psychosocial impact, and well-being is less well documented in speech-language pathology, but research in related areas indicates that the amount of therapy required to have a positive influence on these areas is associated with a broad range of individual and soci

This paper addresses optimal intervention intensity in stuttering with reference to the Lidcombe Program of early stuttering intervention. This is an operant program in which the parent provides the actual treatment, for proscribed periods each day, in the child's everyday environment. The parent learns how to do this during weekly visits with the child to the speech-language pathologist. This program was chosen because it is supported by considerable research evidence. This evidence includes randomized controlled trials and file audits. Individual children vary in the time taken to reach the

Empirical evidence exists for many of the different interventions in speech-language pathology. However, relatively little is known about the optimal intensity of those interventions. In order for speech-language pathology services to be both effective and efficient speech-language pathologists need to know how to faithfully administer ideal doses of the active ingredients of interventions, in what forms, how often and for how long. This is the lead paper to a scientific forum on this fundamental yet under-studied issue of clinical practice. Borrowing from the work of Warren, Fey, and Yoder, t

Although Warren, Fey and Yoder (2007) have described the key components of "dosage", one needs to go beyond description if one is to understand "optimal" dosage, specifically one needs to relate the characteristics of the intervention to the size of the intervention effect. This study examines the association between dose, intensity, and effect size in 20 randomized controlled studies taken from a few systematic reviews focusing on interventions aiming to ameliorate vocabulary, phonology, and syntax. Reporting of dosage characteristics is an important issue. Our analysis shows that "teaching e

PURPOSE: While there has been mounting research centered on the diagnosis of childhood apraxia of speech (CAS), little has focused on differentiating CAS from pediatric dysarthria. Because CAS and dysarthria share overlapping speech symptoms and some children have both motor speech disorders, differential diagnosis can be challenging. There is a need for clinical tools that facilitate assessment of both CAS and dysarthria symptoms in children. The goals of this tutorial are to (a) determine confidence levels of clinicians in differentially diagnosing dysarthria and CAS and (b) provide a system…

BACKGROUND: There are conflicting reports in the research literature of the literacy outcome of children with speech disorder. The link between phonological awareness and literacy in typically developing and literacy delayed children is well established, but there is less research specifically into children with an isolated speech disorder (i.e. with age-appropriate language skills). There is a clinical need for clarification on their likely outcome as a group and for a means of predicting those individuals who might need support in literacy development.
AIMS: To assess literacy and phonologi

A new approach is described for the design of speech materials used in subjective speech quality evaluation. Speech sounds are classified by their acoustic properties, and sentences are composed so as to concentrate all sounds with similar properties within one sentence. As a test of the method, subjective quality data were collected, using both a rank ordering and a rating task, from a set of 12 linear predictive vocoders, whose parameters were chosen so as to equate their bit rates at 2600 bps. The results show that the method can reliably reveal small differences in quality, and also yields

UNLABELLED: Most mobile cleft lip and palate (CLP) surgical units and international missions developing countries generally address surgical issues that reduce the physical abnormalities of the people. Stigmatization persists as speech therapy for most patients is delayed (or they get none at all) due to a shortage of speech and language pathologists.
OBJECTIVE: To estimate cost effectiveness of speech camps.
MATERIAL AND METHOD: The 4-day speech camp and 1-day follow-up session were established based on the Community-Based Model for Speech Disorders for Children with Cleft Lip/Palate. Twelv…

BACKGROUND: The nature of speech disorders in individuals with Down Syndrome (DS) remains controversial despite various explanations put forth in the literature to account for the observed speech profiles. A high level of word production inconsistency in children with DS has led researchers to query whether the inconsistency continues into adolescence, and if the inconsistency stems from inconsistent phonological disorder (IPD) or childhood apraxia of speech (CAS). Of the studies that have been published, most suggest that the speech profile of individuals with DS is delayed, while a few recen…

PURPOSE: The study aimed to investigate how cluttering specialists rated degree of prominence or saliency of various communication dimensions as contributing to the overall cluttering severity.
METHOD: Using a 9-point Likert type scoring system 31 cluttering specialists (with an average of 19 years of experience with cluttering) rated the relative importance of eight speech and language dimensions often associated with cluttering from '1' ('not important') at the low end to a '9' ('very important') at the high saliency end.
RESULTS: Though the salience ratings differed the values in most cas

BACKGROUND: Apraxia of speech is a disorder of speech-motor planning in which articulation is effortful and error-prone despite normal strength of the articulators. Phonological alexia and agraphia are disorders of reading and writing disproportionately affecting unfamiliar words. These disorders are almost always accompanied by aphasia.
OBSERVATIONS: A 36-year-old woman underwent resection of a grade IV astrocytoma based in the left middle precentral gyrus, including a cortical site associated with speech arrest during electrocortical stimulation mapping. Following surgery, she exhibited mod

Background and objectives: Speech disorders are observed in 30% of newly diagnosed sporadic amyotrophic lateral sclerosis (ALS) patients. Characterized by a dynamic course, dysfunction of articulation has not so far been well understood. The aim of this study was to analyze the influence of demographic factors (sex, age, duration of the disease) and concomitant diseases (degenerative spine disease, depression, hypertension, hypothyroidism, hyperthyroidism, and allergy) on the functioning of speech organs in ALS patients. Materials and Methods: The study group consisted of 65 patients with spor…

The paper presents perceptions and feedback from speech therapists and parents embracing the idea of using digital tools in improving the language of the children with speech disorders. The authors investigated the perception of speech therapists and parents and their readiness to use digital tools in speech therapy starting from several digital applications from the domain. The feedback was positive, 88.3% of the parents agree to use digital apps at home, between face-to-face speech therapy sessions coordinated by speech therapists and 75% of parents agree to use them several days a week.
