Endovascular palliation of AVM-associated intractable trigeminal neuralgia via embolization of the artery of the foramen rotundum.

OBJECTIVE: Trigeminal neuralgia is rarely caused by arteriovenous malformations of the posterior fossa. Embolization of aberrant vessels can provide symptomatic relief; however, embolization is not always technically possible, and its effects can be temporary. Embolization of the nerve's blood supply could reduce its excitability and provide pain relief. SETTING: The study was set in an academic tertiary care center. STUDY DESIGN: The study was designed as a report of a clinical case. SUMMARY: The authors report the case of a 13-year-old girl with a large, unruptured posterior fossa arterio

The minipterional craniotomy: technical description and anatomic assessment.

OBJECTIVE: To describe a modification of the pterional approach (PT), the minipterional craniotomy (MPT), and compare the anatomic exposure provided by these two approaches. METHODS: The anatomic exposure offered by the MPT and PT were compared in eight sides of cadaver heads using a computerized tracking system, a robotic microscope, and an image-guidance system. The area of surgical exposure, angular exposure, and anatomic limits of each craniotomy were evaluated. Three recently operated clinical cases (EGF) are also reported. RESULTS: There were no statistical differences in the total are

Internal maxillary artery variability in the pterygopalatine fossa.

The third segment or pterygopalatine branch of the internal maxillary artery has variable branching in the pterygopalatine fossa. This variability in branching can lead to failure in controlling persistent nasal epistaxis by transantral ligation. Although the surgical approach has been previously studied, a systematic classification of this branching has not been previously reported. Through the performance of serial cadaver dissections in the PPF, we have demonstrated the anatomic course and variations of this third segment and offer a relatively simple classification system.

Transphenoidal meningohydroencephalocoele.

A case of transsphenoidal meningohydroencephalocoele is presented. The clinical features and pathogenic factors are discussed briefly. An awareness of this condition should prevent attempted biopsy--which might otherwise prove disasterous. Air or contrast cisternography, with plain or computerised tomographic radiology, would seem to be the investigation of choice.

Is Pterygoid hamulus fracture a pre requisite for cleft palate surgery? - A Systematic review of surgical, otologic, and speech outcomes.

BACKGROUND: Palatoplasty for cleft palate repair often incorporates adjunctive manoeuvres aimed at achieving tension-free closure. Fracture of the pterygoid hamulus has traditionally been performed for this purpose; however, its necessity and influence on postoperative outcomes remain controversial. OBJECTIVE: This systematic review aimed to evaluate whether fracture of the pterygoid hamulus during palatoplasty is required and to assess its impact on surgical, otolaryngological, and speech outcomes. MATERIALS AND METHODS: A systematic search of PubMed/MEDLINE, Embase, Cochrane Library, Scopu

Selective Microsurgical Disconnection of Greater Sphenoid Wing Dural Arteriovenous Fistula Through the Subtemporal Approach Under Exoscope Magnification.

OBJECTIVE: Sphenoid dural arteriovenous fistulas located on the greater sphenoid wing carry a notable risk of intracranial hemorrhage due to the frequent presence of cortical venous reflux. Because of the challenging embolization procedure, microsurgery remains a valuable option in these cases. METHODS: In most cases, the fistula is located in the anterolateral endocranial surface of the middle cranial fossa just below the sphenoid ridge; however, in a small subset of cases, the fistula is positioned posteriorly and medially in the region between the foramen ovale and rotundum. The former reg

Postural Change Assists Surgical Maneuverability During Endoscopic Transsphenoidal Surgery for Pituitary Macroadenoma.

BACKGROUND: In endoscopic transsphenoidal surgery for pituitary adenoma with suprasellar extension, the prolapse of the stretched floppy diaphragma sellae can obstruct the surgical corridor, posing challenges during pituitary surgery. We introduce a simple postural change technique to mitigate this issue and share our clinical experience. METHODS: Pituitary surgery is performed on an operating table where the patient's position can be adjusted. During transsphenoidal pituitary surgery, raising the patient's head to a Fowler position causes the diaphragma sellae to move upward, whereas lowerin

A Newly Discovered Dural Venous Sinus of the Skull Base: The Anterior Petroclinoid Sinus.

INTRODUCTION: Neurosurgeons must master microanatomy and its variants that occur at the skull base. To the best of our knowledge, we describe a previously unreported dural venous sinus (DVS) within the anterior petroclinoid fold and discuss its potential surgical relevance. METHODS: Fifteen latex-injected human cadaveric heads (30 sides) underwent skull base dissection using a surgical microscope. The anterior petroclinoid fold was opened in search of evidence of an embedded DVS. When identified, this sinus was documented, photographed, and measured with microcalipers. RESULTS: An anterior p

Foramen venosum: a clinicoanatomic insight into its occurrence and morphometry.

PURPOSE: The present study aimed to evaluate the foramen venosum (FV) frequency, incidence, morphometry and relation with foramen ovale in an Indian population. The emissary vein passing through it may spread extracranial facial infections to the intracranial cavernous sinus. Due to its close proximity with the foramen ovale and its variable occurrence, awareness about its presence and anatomy is essential to neurosurgeons operating in this region. METHODS: 62 dry adult human skulls were studied for the occurrence and morphometry of foramen venosum, both at the middle cranial fossa and extrac

Review of olfactory cleft roof anatomy.

Progress in skull-base endoscopic surgery has improved our knowledge of olfactory cleft (OC) anatomy. This article presents a review of current knowledge concerning the OC roof: descriptive anatomy, radiological exploration, and endoscopic observation. The OC is a narrow area in the most superior part of the nasal cavity. Its roof is the thinnest structure separating the nasal cavities from the brain; it comprises three superimposed tissues: nasal mucosa, ethmoid cribriform plate (ECP), and dura mater. The ECP comprises the anterior ECP containing the ethmoidal slit (ES) medially and the cribr

Transcranial Surgery for Pituitary Tumors: A "Community Neurosurgery Experience".

BACKGROUND: Despite the evolution of endoscopic techniques, large pituitary adenomas with unfavorable characteristics and irregular anatomical configurations continue to pose a challenge for the dexterity, skills, and patience of endoscopic surgeons. Transcranial surgery retains a significant role in these situations where the tumor access, hemostasis, and dissection around adjoining neurovascular tissues can be controlled efficaciously. OBJECTIVE: In this report, we describe our experience with transcranial surgery for pituitary adenomas highlighting its safety and versatility in peripheral

Morphologic and Morphometric Measurements of the Foramen Ovale: Comparing Digitized Measurements Performed on Dried Human Crania With Computed Tomographic Imaging. An Observational Anatomic Study.

The foramen ovale (FO) of the sphenoid bone is clinically important for the interventional treatment of trigeminal neuralgia. Percutaneous procedures applied to treat the chronic pain condition typically involve the cannulation of this oval-like foramen located at the base of the skull. Anatomic variations of the FO have been reported to contribute to difficulties in the cannulation of this structure. Computed tomography (CT) can help the surgeon improve the accuracy and safety of the intervention. However, even with navigation technology, unsuccessful cannulation of the FO has been reported. …

How I do it: management of M2 tear injury caused by drilling during intradural anterior clinoidectomy for microsurgical clipping of intracranial aneurysms.

BACKGROUND: High-speed drilling is associated with potential injury to neurovascular structures, particularly during intradural drilling of the anterior clinoid process. METHOD: During an anterior clinoidectomy, a cotton patty and middle cerebral artery branches became inadvertently wrapped around the bit, causing a tear on the inferior M2 trunk. Following temporary clipping of the internal carotid artery, the tear was identified. Temporary clips were placed proximally and distally. The tear was then repaired with interrupted microsutures. CONCLUSION: Extreme care should be exercised during

Pediatric paranasal sinuses-Development, growth, pathology, & functional endoscopic sinus surgery.

The paranasal sinuses (maxillary, frontal, ethmoid, and sphenoid sinuses) are complex anatomical structures. The development and growth of these have been investigated utilizing a number of different methods ranging from cadaveric analysis to modern cross sectional imaging with 3D modeling. An understanding of normal pediatric paranasal sinus embryology and development enables us to better determine when pathology may be affecting the normal developmental process. Cystic fibrosis, chronic sinusitis, deviated nasal septum and cleft lip and palate are some of the conditions which have been shown
