The influence of generalized joint hypermobility on the sagittal profile of the spine in children aged 10-13 years.

BACKGROUND: Children commonly attend exercise programs to correct abnormal sagittal curvatures of the spine. The presence of generalized joint hypermobility (JH) is often disregarded during exercise planning. The aim of the study was to assess the influence of JH on the sagittal curvatures of the spine. MATERIAL AND METHODS: The JH group included 38 girls and 37 boys aged 10-13 years with known JH (Beighton test cut-off ≥ 5 points for girls, and ≥ 4 for boys). A control group included 197 girls and 150 boys. The children were matched for age, height, weight and BMI. The sacral slope (SS), lum

Treatment of multilevel cervical fusion with cages.

BACKGROUND: Multilevel cervical discectomy usually requires plate and screw fixation for maintaining the spinal curvature, and increasing the graft fusion rate. However, the use of plate and screw fixation may cause a few complications, such as screw breakage, screw pullout, esophagus perforation, and cord or nerve root injury. In this study, we try to use cages to replace plate function in multilevel cervical fusion. METHODS: From January 1997 to June 2001, there were 180 consecutive cases of multilevel cervical degenerative disease. We randomized them into three groups: Group A (60 patients

Spinal balance failure: a potential cause of spinal ligament ossification.

Ossification of the posterior longitudinal ligament (OPLL) and of the ligamentum flavum (OLF), as well as calcification of ligamentum nuchae in the cervical spine and ossification of ligamentum flavum in the thoracic spine, are common types of spinal ligament ossification. As is known, the pathogenesis of OPLL and OLF has been unclear until recently, and mechanical stress assumably plays an important role. In this paper, we try to make a hypothesis that spinal balance failure (characterized by the gravity line, spinal curvature and flexion/extension in the sagittal plane), may play more import

A New Normative Zone for Acetabular Anteversion Positioning in ASD Patients.

Adult spinal deformity patients undergoing total hip arthroplasty experience higher hip dislocation rates than those with normal spinal alignment. The traditional Lewinnek safe zone does not account for spinopelvic variation such as pelvic retroversion. To address this, three patient-specific normative zones for acetabular anteversion were defined. A multicenter retrospective analysis of 146 adult spinal deformity patients and 47 asymptomatic controls was performed using three-dimensional biplanar radiograph reconstructions to measure spinopelvic alignment and acetabular orientation. Normative

The learning curve for performing three-column osteotomies in adult spinal deformity patients: one surgeon's experience with 197 cases.

BACKGROUND CONTEXT: Three-column osteotomy (3CO) is used to correct rigid adult spinal deformity. It presents risk of complications because it involves extensive osseous resection and spinal destabilization. PURPOSE: Our purpose was to characterize the learning curve for performing 3CO in adult spinal deformity patients. DESIGN: Retrospective review. PATIENT SAMPLE: A surgical registry at a tertiary care center was used to identify 238 cases of 3CO for correction of adult spinal deformity by 1 surgeon between 2005 and 2014. Patients with at least 1 year of clinical and radiographic follow-u

Can knee flexion contracture affect cervical alignment and neck tension? A prospective self-controlled pilot study.

BACKGROUND CONTEXT: The coordination of the alignment between the lower extremities and cervical spine helps to achieve balance and horizontal gaze during standing and walking. Malalignment in any segment can disturb the global balance, causing compensation in another segment. Knee flexion contracture (KFC) can cause spine inclination with increased C7 tilt or C7 SVA (sagittal vertical axis). Cervical alignment and the posterior muscles are essential for maintaining the horizontal gaze which is closely related to neck tension (NT). PURPOSE: This study aimed to determine whether KFC can affect

Factors Associated with Cervical Spine Alignment in an Asymptomatic Population: A Preliminary Analysis.

BACKGROUND: Cervical sagittal imbalance compromises health-related quality of life and can lead to myriad incapacitating symptoms through compression of the spinal cord. Questions regarding which parameters play primary roles in the progression of cervical sagittal imbalance and which might be compensatory factors remain unanswered. METHODS: This study enrolled 246 asymptomatic volunteers from July 2016 to June 2018. After demographic and radiologic parameters were measured, the data were analyzed using correlation coefficient test and multiple regression analysis. A predictive equation was a

Reliability assessment of a novel cervical spine deformity classification system.

OBJECT: Despite the complexity of cervical spine deformity (CSD) and its significant impact on patient quality of life, there exists no comprehensive classification system. The objective of this study was to develop a novel classification system based on a modified Delphi approach and to characterize the intra- and interobserver reliability of this classification. METHODS: Based on an extensive literature review and a modified Delphi approach with an expert panel, a CSD classification system was generated. The classification system included a deformity descriptor and 5 modifiers that incorpor

Camptocormia and dropped head syndrome as a clinic picture of myotonic myopathy.

Dropped head syndrome is primarily based on weakness localized at neck extensors. It may result from motor neuron disease, myasthenia gravis, and chronic inflammatory demyelinating polyneuropathy and also from various neuromuscular diseases including inflammatory, dystrophic and metabolic myopathies. Camptocormia (CC) on the other hand is an unusual condition characterized by progressive weakness of the extensor vertebral muscles and results in involuntary trunk flexion. CC may emerge as a clinical feature of many different conditions such as several myopathies and Parkinson's disease. The ass

The effects of pineal gland transplantation on the production of spinal deformity and serum melatonin level following pinealectomy in the chicken.

Pinealectomy frequently produces spinal deformity in some animal models, but the precise biological mechanism of this phenomenon remains obscure. The current study investigated the effects of an autograft pineal body on the development of spinal deformity and serum melatonin (MLT) concentration after pinealectomy in the chicken. Thirty-six chickens (2 days of age) were divided into three equal groups. While the removal of the pineal gland was performed in groups B and C, a pineal body autograft was surgically implanted into the body wall musculature only in the pineal transplantation group (gr

Conditional inactivation of the L-type amino acid transporter LAT1 in chondrocytes models idiopathic scoliosis in mice.

Scoliosis, usually diagnosed in childhood and early adolescence, is an abnormal lateral curvature of the spine. L-type amino acid transporter 1 (LAT1), encoded by solute carrier transporter 7a5 (Slc7a5), plays a crucial role in amino acid sensing and signaling in specific cell types. We previously demonstrated the pivotal role of LAT1 on bone homeostasis in mice, and the expression of LAT1/SLC7A5 in vertebral cartilage of pediatric scoliosis patients; however, its role in chondrocytes on spinal homeostasis and implications regarding the underlying mechanisms during the onset and progression of

Predicting postoperative imbalance in adult spinal deformity staged surgery using predictive thresholds.

Two-stage surgeries are increasingly used to minimize complications in adult spinal deformity (ASD) correction, yet the specific contributions of lateral lumbar interbody fusion (LLIF) and posterior column osteotomy/posterior spinal fusion (PCO/PSF) remain underexplored. This study evaluates their roles in deformity correction and establishes predictive thresholds for optimizing surgical planning. A total of 151 ASD patients (mean age 69.5 years) underwent staged LLIF and PCO/PSF surgeries one week apart. Radiographic parameters were analyzed preoperatively (Pre-), post- 1st LLIF (M-), post- 2

The Lower Lordotic Arch Is a Driver of Pelvic Positioning and Inflection Point Migration in Adult Spinal Deformity: A Retrospective Cohort Analysis.

STUDY DESIGN: Retrospective cohort. OBJECTIVE: To evaluate the relationship between the lower lordotic arch (LLA) and other parameters of spinopelvic alignment in adult spinal deformity (ASD). SUMMARY OF BACKGROUND DATA: Lordotic and kyphotic curves maintain a geometrically balanced relationship defined by inflection points and apices along the spine. One proposed relationship suggests that pelvic positioning should be intimately related to the LLA, defined as the Cobb angle between the lordotic apex and L5. METHODS: We reviewed data of 304 adult thoracolumbar fusion patients between 2014 a
