PURPOSE: To assess the technical success rate, complications, and effect on intraoperative blood loss of preoperative transarterial embolization of cervical spine tumors.
METHODS: A retrospective analysis was performed on 38 patients with tumors of the cervical spine; 69 vertebrae were affected. Polyvinyl alcohol particles, coils, gelfoam particles, either alone or in combination, were used for preoperative tumor embolization. After embolization a total of 57 corporectomies with titanium basket implantation were performed.
RESULTS: In 36 of 38 patients, complete (n = 27) or partial (n = 9) e…

To a large extent, what is new in cervical spine trauma relates to evolving indications for the use of various imaging modalities and expanded understanding of conditions that may influence traumatic sequelae. The American College of Radiology Appropriateness Criteria now provide guidelines for decisions concerning imaging in various clinical situations. This article reviews many of the publications of the past 6 to 7 years that may contribute to a radiologist's understanding of cervical spine injuries.

Cervical spine movements in 38 severe hemiparetic patients were measured, at least 6 months after the breakout of the first and unique cerebral vascular accident. The purpose was to investigate if there was any difference in maximal head rotation and lateral bending movements between the plegic and the sound side. The measurements were taken by a spherical compass attached to a cubic protractor with a small mobile ball inside, both fixed to the patient's head. All the tests were done several months after the end of the rehabilitation process and compared to those obtained in 29 orthopaedic pat…

STUDY DESIGN: This was a retrospective review of 49 consecutive patient charts and a prospective study of 44 consecutive patients who underwent spinal fusion.
OBJECTIVE: To determine the incidence and clinical significance of hypomagnesemia after spinal fusion.
SUMMARY OF BACKGROUND DATA: Hypomagnesemia may be seen in 61% of patients in postoperative intensive care and may be associated with increased mortality. However, symptomatic hypomagnesemia is rare.
METHODS: A retrospective review of the charts of 49 consecutive patients who underwent spine fusion was completed to determine postopera…

STUDY DESIGN: This article reviews the criteria for evaluating the quality of clinical trials.
OBJECTIVES: To outline the current methodologic standards by which the validity of controlled trials need to be evaluated.
SUMMARY OF BACKGROUND DATA: Weber's study, published in 1983 in Spine, is the only randomized trial comparing surgery and conservative management of sciatica in herniated lumbar discs.
METHODS: Weber's article is revisited to illustrate basic principles in the design of clinical trials.
RESULTS: Weber's study is a classic in spine surgery and has changed thinking regarding th…

Neck pain may affect one third of the adult population from time to time and may persist for 6 months or longer in 10% to 15% of these patients. In addition, cervical spine disease or trauma may result in injury to the spinal cord or nerve roots, through a variety of mechanisms, leading to a group of syndromes that can be broadly classified as myelopathy or radiculopathy. This review addresses recent developments in the anatomy and physiology of the normal cervical spine, common ill-defined or miscellaneous neck pain problems, and cervical injuries, with emphasis on the flexion-extension injur…

This study analyzed a series of 35 patients with unstable fractures, with and without neurologic deficit including complete paraplegia. The patients underwent spinal stabilisation using plates and screws after sustaining fracture dislocations. Very good results were obtained in 19 patients with non-complicated fractures operated on by dorsal stabilization using Meurig-Williams plates. Complete recovery of the neurologic deficit was achieved in five out of 13 patients with neurologic deficit two of whom were paraplegic. Use of stabilization operation accompanied by dorsal vertebrodesis and post

Fifty-two cases of inveterate cervical spine injury have been presented. Nearly all the injury types are represented. Neurological complications were present in sixty-nine percent of the cases. Specific surgical treatment aimed at restoration of anatomical relations within the spine, decompressing neural elements and stabilization of the injured segment. Good results have been achieved with neurological improvement, especially in the group of patients with paresis or root symptoms. Several paraplegic patients also benefited from the treatment. The authors conclude, that management employed off…

We feel that the described system for staging cervical and lumbar spine injuries in the high-performance athlete affords a basic framework for the rehabilitation of those individuals. With spine injuries, one has to exercise care to rule out those injuries and additional pathologies presenting as neck or back pain that could result in catastrophic sequelae. We stress due diligence in the initial and continued follow-up of these patients with careful evaluation to rule out associated fractures, dislocations, and attendant disc pathology. The treatment of the "stinger" or "burner" is controversi…

In the years 1965-1990 in the hospital department for treatment of spinal injuries 1330 patients with spinal lesions at the C5-Th1 levels were treated after admission early after trauma. The most frequent causes of injuries of this part of the spine were falls from horsecarts, jumping into water head first and traffic accidents. The relationship was analysed between the degree of spinal cord injury and the mechanism of spine injury, and also the results were compared of conservative and surgical treatment. The most severe damage to the spinal cord was observed after traumas involving the flexi…

Cervical spine fractures are a serious complication of maxillofacial trauma because of the high potential for mortality and neurologic morbidity. This study reviewed 563 patients with facial fractures treated by the Division of Oral and Maxillofacial Surgery at a level I trauma center and identified 11 concomitant cervical spine fractures (2.0%). These patients were almost exclusively male (91%), white (100%), between 20 and 35 years of age (64%), involved in a motor vehicle accident (91%), with a concomitant mandibular fracture (91%). The examination, diagnosis, and management of patients wit…

After a head or a cervical injury dysphonia and its accompanying globus syndrome are often ignored, even if the existence of these traumatic … the functional dysphonia and last not least from the prove of a functional deficit of the cervical spine, especially in the height of C 2/3. The right diagnosis allows a causal treatment and enables a just expert opinion.

We report three cases of thoracic spine translocation without neurological deficit. In each case bilateral pedicular fractures, demonstrated by computed tomography, produced 'floating arches' which account for the sparing of the cord. If computed tomography demonstrates adequate canal dimensions, these patients may be treated conservatively, but the treatment of choice at specialist spinal centres is operative stabilisation.

We present a study of 30 fusion operations in 26 rheumatoid arthritics with cervical spine instability. Atlanto-axial instability was present in 15, of whom 12 were fused; three had cord involvement and all made a partial or complete recovery following fusion. Cranial settling necessitated cranio-cervical fusion in four patients; all fused, and one with myelopathy was relieved. Subaxial instability required fusion in seven cases; two postoperative deaths followed the only two anterior interbody fusions. Posterior fusion was successful in the other five, with remission of neurological compromis…

At the scene of an accident with possible injury to the cervical spine, management should focus on life support and prevention of further injury. On arrival of the patient in the emergency department, three standard x-ray films should be obtained in search of the abnormalities in soft tissues, alignment, and joints that indicate cervical spine injury and a thorough neurologic assessment should be performed. The potential of cord injuries to deteriorate makes careful monitoring of blood gases, urine output, and blood pressure imperative during this period. Early therapy with steroids and mannit…

Spinal fusion for deformity of the cervical spine was done in thirty-three patients with rheumatoid arthritis. The average follow-up was three years. The deformities present were atlano-axial subluxation, superior migration of the odontoid process into the foramen magnum, and subaxial subluxation of the vertebral bodies. We devised a classification of the pain and the neural involvement in these patients and a new method of measuring superior migration. The surgical procedures for treating instability, intractable pain, or neural involvement, or a combination of the three, were: (1) a Gallie f…
