[A case of non-convulsive status epilepticus worsened Wernicke's aphasia reversely].

A 62-year-old right-handed woman had presented progressive speech impediment over 4 months. She was alert without any convulsions or involun … fter an administration of anti-convulsive agents, thus establishing the diagnosis of non-convulsive status epilepticus (NSE). It should be emphasized that NSE manifesting as Wernicke's aphasia should be distinguished from dementia syndrome because it is a treatable disorder.

Treatment of refractory complex-partial status epilepticus with propofol: case report.

PURPOSE: We report a case of a 65-year-old woman who had a subarachnoid and intraventricular hemorrhage secondary to rupture of an anterior communicating artery aneurysm and developed nonconvulsive status epilepticus of the complex-partial type, refractory to phenytoin (PHT), phenobarbital (PB), valproate (VPA), and lorazepam (LZP). METHODS: Three weeks after diagnosis of nonconvulsive status epilepticus, general anesthesia was induced with propofol and titrated to burst suppression on the electroencephalogram (EEG). RESULTS: During propofol infusion, the serum VPA level declined markedly, a…

Late onset heterozygous ornithine transcarbamylase deficiency mimicking complex partial status epilepticus.

A 57 year old woman with post-traumatic complex partial seizures was admitted because of recurrent episodes of altered mental state over the preceding 4 years, each lasting up to 5 days. There was a history of dietary protein intolerance since childhood and two of her daughters had died in the neonatal period from unexplained encephalopathies. In hospital she developed fluctuating confusion, amnesia, and sudden episodes of unresponsiveness. An EEG was consistent with complex partial status epilepticus but there was no response to benzodiazepines. Nasogastric feeding and sodium valproate were g…

Non-convulsive status epilepticus: causes, treatment, and outcome in 65 patients.

The incidence of non-convulsive status epilepticus (NCSE) in The Netherlands is not known. Files of admissions in the years 1980-7 were studied from 40 adult patients (older than 15 years) with complex partial status epilepticus (CPSE) and 25 with absence status epilepticus (ASE). The clinical presentation sometimes made distinction between CPSE and ASE possible. Focal clinical signs were more frequent in CPSE; a fluctuating level of consciousness was more often present in ASE. All patients, but one, with ASE and most patients with CPSE (28) were known to have had previous epilepsy. Outcome in…

[Munchausen sydrome presenting repeated attacks of factitious status epilepticus (case report)].

A 27-year-old male patient with Munchausen syndrome presenting repeated pseudoseizures simulating grand mal status, is presented. Over a ten-year period there were more than hundred admissions to neurologic of psychiatric wards for suspected diagnosis of uncontrolled epilepsy. On many occasions the patient was admitted to the hospital under dramatic conditions of psychogenic seizures similar to grand mal status that initially demanted intensive antiepileptic treatment, in order to prove its pseudoepileptic nature. On the basis of his past history, our observation, and information obtained from

Complex partial status epilepticus presenting as gelastic seizures: a case report.

A middle-aged man, who presented to the emergency room because of bizarre outbursts of laughter, was found to be in partial complex status epilepticus. His seizure disorder had been misdiagnosed, at various times, as a variety of "functional" psychiatric disorders. Despite proper diagnosis and aggressive treatment, management was difficult, being complicated by postictal agitation and confusion, postictal psychosis, and interictal compulsive and paranoid personality features. This case is described, and issues of diagnosis and management in partial complex epilepsy are briefly discussed. The i…

Non-convulsive status epilepticus after abrupt withdrawal of hypnotic-sedative drugs.

Four patients with severe addiction to sedative-hypnotics and with acute withdrawal symptoms of these drugs are described. They developed latent confusional states with characteristic EEG patterns (bilateral slow and sharp waves of high amplitude). Following small doses of benzodiazepines the EEG became normal together with a reduction in the clinical symptoms. It is suggested that the confusional states were of an epileptic nature.

Protracted epileptiform encephalopathy: an unusual form of partial complex status epilepticus.

An 11-year-old previously healthy boy had an abrupt onset of partial complex, focal, multifocal, and generalized seizures, with interictal expressive aphasia, extreme emotional lability, agitation, and complex visual and auditory hallucinations. EEGs showed frequent runs of rhythmic high-voltage delta over the right and subsequently over the right and left temporal and frontal regions. All other studies were negative (repeated computed tomography, spinal fluids, viral titers, and cultures). Nadir during the second month showed virtual unresponsiveness, prolonged rhythmic motor and apneic seizu

Complex partial status epilepticus: case report and proposal for diagnostic criteria.

A child had complex partial status epilepticus after resection of a craniopharyngioma. The status epilepticus was manifested by confusion and documented electroencephalographically by continuous focal seizure patterns in the right temporal region. Complex partial status epilepticus is an electroclinical syndrome of prolonged or repetitive complex partial seizures (with continuous interictal confusion) accompanied by electroencephalographic seizure patterns, which are either focal (usually temporal lobe) or secondarily generalized from a focal pacemaker.

Claustrum Involvement in New Onset Refractory Status Epilepticus: A Systematic Review.

The claustrum sign is a distinctive neuroimaging finding characterized by bilateral T2/FLAIR hyperintensity of the claustrum, one of the most interconnected regions of the human brain. It was first described in new-onset refractory status epilepticus (NORSE) and febrile infection-related epilepsy syndrome (FIRES). A systematic search was conducted according to PRISMA guidelines from 2018 (formal definition of NORSE) to February 2026 (PROSPERO 2026 CRD420261307306). We included peer-reviewed original studies reporting MRI findings and patient-level data, and compared clinical characteristics, t…

Emergent EEG in Status Epilepticus: Off-Hours Delays Limit Clinical Utility.

BackgroundTo evaluate the use, diagnostic yield, and timing of EEG in emergency department (ED) patients receiving second-line intravenous antiseizure medication (IV ASM).MethodsWe retrospectively reviewed patients aged ≥16 years who received IV ASM at Tampere University Hospital over one year. Episodes were categorized by status epilepticus (SE) subtype and arrival time (office vs off-hours). Timing of ASM/EEG and EEG's diagnostic contribution were analyzed.ResultsWe identified 132 episodes in 116 patients (41% female; median age 66 years). Convulsive SE (CSE) was most common (n = 41), follow…

Time over drug in pediatric status epilepticus: A prospective observational study.

Convulsive status epilepticus (CSE) is a time-sensitive neurological emergency in which delayed treatment may contribute to pharmacoresistance and poor outcomes. Evidence comparing third-line antiseizure medications in children remains limited. Objective To compare intravenous lacosamide and phenytoin in pediatric refractory CSE and to determine whether treatment timing predicts seizure cessation. Methods This prospective observational cohort study included 100 children aged 1 month-16 years with refractory CSE treated with intravenous lacosamide (n = 46) or phenytoin (n = 54) after failure of…

Restimulation could stop status epilepticus after electroconvulsive therapy: 2 case reports.

BACKGROUND: Electroconvulsive therapy (ECT) is an effective treatment for severe depression, mania, psychosis and catatonia. While seizures are considered essential for the therapeutic effect of ECT, it concurrently has an anticonvulsant effect which plays a role in its mechanism of action. This property has also prompted the use of ECT in managing status epilepticus (SE). CASE PRESENTATION: We report two distinct cases of prolonged seizures during ECT that persisted for more than 5 min despite administration of propofol and lorazepam, ultimately meeting criteria for status epilepticus (SE). …

Pediatric new-onset super-refractory status epilepticus (NOSRSE): a case-series.

BACKGROUND: New-onset super-refractory status epilepticus (NOSRSE) leads to functional deficits and residual epilepsy. This study aimed to describe pediatric NOSRSE cohort to gain clinical insights of their features. METHODS: A retrospective review of children with NOSRSE in 2013-2024 was conducted at two tertiary hospitals. Patient clinical data, including MRI, CSF profile, EEG, and treatments, were collected and reviewed. The primary outcome measure was the modified Rankin scale score (mRS) at 3-month post-seizure. RESULTS: Twenty-three patients with NOSRSE, with a median age of 7.9 years,…

Staged treatment response in status epilepticus: Lessons from the SENSE registry.

OBJECTIVES: Although in epilepsy patients the likelihood of becoming seizure-free decreases substantially with each unsuccessful treatment, to our knowledge this has been poorly investigated in status epilepticus (SE). We aimed to evaluate the proportion of SE cessation and functional outcome after successive treatment steps. METHODS: We conducted a post hoc analysis of a prospective, observational, multicenter cohort (Sustained Effort Network for treatment of Status Epilepticus [SENSE]), in which 1049 incident adult SE episodes were prospectively recorded at nine European centers. We analyze…
