Haemodynamics at rest and during isometric exercise under long-term treatment with captopril in dilated and ischaemic cardiomyopathy.

In the present study 12 patients with severe chronic heart failure (7 with dilated cardiomyopathy and 5 with ischaemic heart disease) underwent right heart catheterization at rest and during isometric exercise by handgrip before and 6 months after treatment with captopril (mean dose 2 X 39.6 mg/day) while concomitant digitalis and diuretics were unchanged. After 6 months resting haemodynamics changed as follows: decrease in heart rate, mean arterial pressure, pulmonary capillary pressure and total vascular resistance; increase in stroke volume index and stroke work index. During handgrip the f…

The correlation between intracranial atherosclerosis and white matter hyperintensities in cerebral small vessel disease: a high-resolution magnetic resonance imaging study.

OBJECTIVE: Recent studies have indicated a close relationship between intracranial arterial stenosis and white matter hyperintensities (WMHs), but few have reported on the correlation between the characteristics of intracranial arterial wall plaques and WMHs. The aim of this study was to comprehensively assess the correlation between intracranial atherosclerosis plaques and WMHs using 3.0T high-resolution magnetic resonance imaging (HR-MRI). PATIENTS AND METHODS: Ninety-two ischemic stroke patients with middle cerebral artery (MCA) stenosis <50% on cranial magnetic resonance angiography (MRA)

Clinical Situations Associated with Inappropriately Large Regurgitant Volumes in the Assessment of Mitral Regurgitation Severity Using the Proximal Flow Convergence Method in Patients with Chordae Rupture.

BACKGROUND: Regurgitant volume (RVol) calculated using the proximal flow convergence method (proximal isovelocity surface area [PISA]) has b … ISA and two-dimensional Doppler volumetric methods. The volumetric method was used to measure total stroke volume using the two-dimensional Simpson biplane method, and forward stroke volume was measured using pulsed Doppler tracing at the left ventricular (LV) outflow tract. RVol by the volumetric method was calculated using total stroke volume - forward stroke …

Prediabetes as a critical stage for risk of dementia and stroke: evidence from the UK Biobank and Mendelian Randomization.

BACKGROUND AND AIMS: Type 2 diabetes (T2D) is a recognized risk factor for dementia and stroke, but whether risks begin during prediabetes remains unclear. We aimed to determine the risks of these neurological conditions among individuals with prediabetes and the glycemic thresholds at which risks emerge. METHODS: We studied 432,887 adults (mean age, 57 years; 55% women) from the UK Biobank who were free of dementia or stroke at baseline, categorized by glycemic status (normoglycemia, prediabetes, and T2D). Incident dementia, stroke, and Magnetic Resonance Imaging (MRI)-derived brain markers,

Correlating stroke risk with non-invasive cerebrovascular perfusion dynamics using a portable speckle contrast optical spectroscopy laser device.

Stroke poses a significant global health threat, with millions affected annually, leading to substantial morbidity and mortality. Current stroke risk assessment for the general population relies on markers such as demographics, blood tests, and comorbidities. A minimally invasive, clinically scalable, and cost-effective way to directly measure cerebral blood flow presents an opportunity. This opportunity has the potential to positively impact effective stroke risk assessment prevention and intervention. Physiological changes in the cerebrovascular system, particularly in response to hypercapni

Targeting Brain-spleen Crosstalk After Stroke: New Insights Into Stroke Pathology and Treatment.

The immune response following acute stroke has received significant attention. The spleen is an important immune organ, and more and more studies have shown that brain-spleen crosstalk after stroke plays an important role in its development and prognosis. There are many mechanisms of spleen activation after stroke, including activation of the sympathetic nervous system, the production of chemokines, and antigen presentation in the damaged brain. The changes in the spleen after stroke are mainly reflected in morphology, changes to immune cells, and cytokine production. Once activated, the splee

Immuno-inflammatory activation in acute cardio-embolic strokes in comparison with other subtypes of ischaemic stroke.

Few studies have examined the relationship between inflammatory biomarker blood levels, cardioembolic stroke subtype and neurological deficit. So the aim of our study is to evaluate plasma levels of immuno-inflammatory variables in patients with cardio-embolic acute ischaemic stroke compared to other diagnostic subtypes and to evaluate the relationship between immuno-inflammatory variables, acute neurological deficit and brain infarct volume. One hundred twenty patients with acute ischaemic stroke and 123 controls without a diagnosis of acute ischaemic stroke were evaluated. The type of acute

Association of 24-Hour Computed Tomography Infarct Density on Functional Outcomes in Stroke: Secondary Analysis From the AcT Trial.

BACKGROUND: The final infarct volume has been historically considered the most critical radiological outcome in ischemic stroke. Yet, discrepancies between final infarct volume and functional outcomes occur frequently. We aim to evaluate whether 24-hour computed tomography (CT) infarct density modifies the relationship between 24-hour CT infarct volume and functional outcomes and is associated with outcomes in acute ischemic stroke. METHODS: Data are from the AcT (Alteplase Versus Tenecteplase in AIS [Acute Ischemic Stroke] Within 4.5 Hours) trial. Patients with detected 24-hour CT infarct an

Two-dimensional echocardiographic evaluation of right ventricular size and function in newborns with severe right ventricular outflow tract obstruction.

Critical pulmonary stenosis or atresia with intact ventricular septum represents a congenital cardiac lesion for which the long-term prognosis appears to depend partly on the size of the right ventricle. Thus, the capability of noninvasive assessment of right ventricular size to predict operative outcome was examined in 15 infants (aged 1 to 30 days, mean 5.6) with severe right ventricular outflow tract obstruction (either critical pulmonary stenosis [7 patients] or pulmonary atresia with intact ventricular septum [8 patients]). Using echocardiography in two orthogonal subxiphoid views, right

Cardiac tamponade in dogs with normal coronary arteries. I. Effect of changing intravascular volume on hemodynamics and myocardial blood flow.

Intravascular volume expansion has been shown to improve cardiac output in experimental cardiac tamponade. To determine the limitations of intravascular volume manipulation, acute tamponade was created in 20 anesthetized, spontaneously breathing dogs. The intrapericardial volume causing tamponade was determined for each animal, and kept constant. Hemodynamics were recorded with and without tamponade at multiple levels of intravascular volume. During cardiac tamponade, intravascular volume expansion increased cardiac output only in animals which were initially volume-depleted. Volume expansion

[Study of left ventricular function in chronic volume overload (author's transl)].

Have been studied 51 patients with aortic incompetency (AI), 46 with mitral insufficiency (MI) and 31 with congestive cardiomyopathy (CM) as example of a primitive myocardial lesion. The mean parametres calculated were: the left ventricular end-diastolic pressure (LVEDP), the ventricular volumes, the ejection fraction (EF), the end-sistolic pressure-volume ratio (Emax) and the eccentricity. In the chronic volume overload, the relationship between the pump performance and the contractility (EF-Emax) is parabolic with an initial plateau (myocardial failure without circulatory failure); the EF-Em

Ischemic Post-Conditioning Induces Post-Stroke Neuroprotection via Hsp70-Mediated Proteasome Inhibition and Facilitates Neural Progenitor Cell Transplantation.

In view of the failure of pharmacological therapies, alternative strategies promoting post-stroke brain repair are needed. Post-conditioning is a potentially promising therapeutic strategy, which induces acute neuroprotection against ischemic injury. To elucidate longer lasting actions of ischemic post-conditioning, mice were exposed to a 60-min stroke and post-conditioning by an additional 10-min stroke that was induced 10 min after reperfusion onset. Animals were sacrificed 24 h or 28 days post-stroke. Post-conditioning reduced infarct volume and neurological deficits 24 h post-stroke, enhan

Lower Limb Task-Based Functional Connectivity Is Altered in Stroke.

The goal of this work was to examine task-dependent functional connectivity of the brain in people with stroke. The work was motivated by prior observations indicating that, during pedaling, cortical activation volume is lower in people with stroke than controls. During paretic foot tapping, activation volume tends to be higher in people with stroke than controls. This study asked whether these differences could be explained by altered network function of the brain. Functional magnetic resonance imaging was used to examine local and global network function of the brain during tapping and pedal
