In this study, we evaluate the role of the thalamus in the neural circuitry of arousal. Level of consciousness within the first 12 hours of a thalamic stroke is assessed with lesion symptom mapping. Impaired arousal correlates with lesions in the paramedian posterior thalamus near the centromedian and parafascicular nuclei, posterior hypothalamus, and midbrain tegmentum. All patients with severely impaired arousal (coma, stupor) had lesion extension into the midbrain and/or pontine tegmentum, whereas purely thalamic lesions did not severely impair arousal. These results are consistent with gro…

A 12 yr old female neutered Carthusian crossbreed cat was presented due to progressive neurological signs. Clinical signs included dehydration, stupor, and anisocoria. Laboratory examination revealed severe hypernatremia, azotemia, hyperglobulinemia, and an erythrocytosis. Clinical signs and hypernatremia suggested an intracranial process. Imaging studies revealed a loss of structure in the cerebrum, hypothalamus, and pituitary gland. Due to a poor prognosis, the cat was euthanatized. Histopathological examination revealed a subacute granulomatous and necrotizing panencephalitis with Toxoplasm…

A clinical picture named neuroleptic malignant-like syndrome has been described in patients with Parkinson's disease (PD) who suddenly stop their L-dopa treatment. The sudden withdrawal of the drug is deemed to lead to an acute deficiency stage in a patient who has an iatrogenic increase of dopaminergic transmission. We present a series of 11 patients with PD with an average age of 72.09 years, a mean disease duration of 9.45 years who developed this problem after a 92.72 h latency period. If patients with PD develop severe rigidity, stupor and hyperthermia, L-dopa withdrawal should be suspect…

1. The neuroleptic malignant syndrome (NMS) may occur, occasionally, in Parkinson's disease (PD) after withdrawal of antiparkinsonian drugs. However, the circumstances in which the NMS occurs and the pathophysiologic mechanisms remain uncertain. 2. The authors studied a woman with PD, who developed hyperthermia, increased muscular tone, tremor, signs of autonomic dysfunction and stupor as symptoms of acute hyponatremia due to gastrointestinal loss of sodium in excess of water. 3. The correction of hyponatremia led to a complete recovery after about 6 hours. During this period the antiparkinson…

This report describes the clinico-pathological features of acute lymphocytic meningoencephalitis (ALME) of a patient with a long clinical course of 24 months. The patient rapidly developed a confusional state, occasionally with stupor, various involuntary movements and generalized convulsions followed by fever and headache at the age of 29. Clinical symptoms, except fever and convulsions, had not distinctly improved throughout the clinical course, and cortical atrophy on CT and MR gradually progressed. SPECT revealed a low blood perfusion in the cerebral cortices. Neuropathologically, inflamma…

Ultrastructural findings in the brains and livers of four calves affected with citrullinaemia due to a hereditary deficiency of the urea cycle enzyme arginosuccinate synthetase are described. The calves were affected in the 1st week of life with severe neurological disease characterised by depression, head pressing, stupor, convulsions and coma, accompanied by marked elevation of plasma citrulline and increasing plasma ammonia levels. Lesions included mild to moderate diffuse astroglial oedema in the cerebrocortical grey matter, and mild to severe hepatocellular hydropic change. The onset of t…

Part II of the affected sibling study raises questions of symptom correlation (incidence of delusions, states of confusion, suicide tendencies and attempts, hallucinatory episodes, hypochondriac attitudes, angry manias, and depressive stupor) in mood disorders. The investigated sample (307 sibling groups, 701 hospitalized patients; selection principle: at least 1 sibling showing a bipolar or manic axial syndrome) directs our attention to the possibility that the multiplicity of specific manic and depressive syndromes derives not only from the genetic complexity of the mood-producing system but…

Electroencephalographic and clinical effects of baclofen were investigated in 46 neurological patients and 7 healthy volunteers. 27 cases sh … ed EEG-slowing and bilateral synchronous sharp and slow waves. This was interpreted as an epileptic stupor induced by Baclofen. The patient had renal insufficiency and accumulated Baclofen. Both conditions may have contributed to the epileptic manifestation.

Several obstacles exist that impede the scientific study of pathological underarousal (stupor and coma). For instance, there is a lack of a clear, rational, standardized taxonomy with which to describe the phenomenon. Moreover, there is considerable confusion of the construct of arousal with other neurobehavioral constructs. The field also suffers from a general lack of acceptably reliable and valid instruments, especially measures of long-term outcome. Additionally, current treatments for chronic pathological underarousal are frequently presumptive and applied haphazardly, and thus do little …

Drowning is one of the most common causes of death in children. Near-drowning is even more frequent: near-drowning/drowning ratio is approximately 9-10/1. The age-groups between 1 and 3 years and 15 and 25 years are most likely to be at risk for submersion accidents. The majority of drowning accidents in children occur in swimming pools and bathtubs. The awake or stuporous victim has an excellent prognosis. In comatose patients, the prognosis is directly proportional to the severity of coma. Treatment is mostly supportive. Despite optimal care, it is not infrequent for a near-drowning accident…

Five cases of poisoning by indigenous mushroom Hikageshibiretake (Psilocybe argentipes) are reported. As this mushroom contains psilocybin, in general, clinical features were similar to those seen by pure psilocybin. Acute toxic stuporous state with complete amnesia in the culminating period occurred in one case, psychedelic state with dreamy consciousness in one case and psychotic adverse reactions with vivid visual hallucinations with consciousness in three cases. There were accompanied with anxiety and panic reactions to subjective experiences. Though these toxic effects were usually short-…

A 14-year-old boy with a tetralogy of Fallot showed a subacute, progressive encephalopathy characterized by headache, stupor and coma. Death occurred three months after the beginning of the illness. Neuropathologic examination revealed diffuse foci of necrosis of the cerebral hemispheres and cerebellum, and the presence of the nematode Lagochilascaris minor in the parenchyma and in the cisterns at the base of the brain. In almost all 28 cases of human lagochilascaris previously reported, the worms were located in the soft tissues of the neck and throat, tonsils, mastoids and paranasal sinuses.…

Seventy-eight patients with aseptic cerebral venous thrombosis between 1966 and 1990 were reviewed in order to determine prognostic factors for this condition. Sixty-eight patients were women. The average age of the patients was 30.5 years. Fifty-two percent of patients developed the thrombosis during the postpartum or puerperal stage. The clinical manifestations that were associated with a poor prognosis were: stupor or coma, bilateral pyramidal tract signs, generalized seizures, meningeal signs, bilateral lesions on computed tomography, and hemorrhagic cerebrospinal fluid. Using these clinic…

Delusional disorders are one of the psychiatric disorders with unknown etiology. Various factors have been considered as risk factors (predisposing factors) for delusional disorders. Positive family history of psychiatric disorders particularly delusional disorders, is the most important risk factor. In this report, we present a 40-year old man with somatic delusions following intoxication with Phenobarbital and left brachial plexus injury (C6, C7, C8) in stupor state. Two weeks after discharge from the hospital, he developed a significant somatic delusion about his left upper extremity. To ou…

A 3-year-old, spayed female rat terrier was evaluated for acute onset of stupor, disorientation, and tetraparesis. Clinical signs progressed over 3 weeks to eventual right-sided hemiparesis and circling to the left. A Cuterebra spp. larva was discovered in the vomitus of the dog 2 weeks after the onset of clinical signs. Cerebrospinal fluid analysis showed chronic inflammation, and magnetic resonance imaging supported a diagnosis of a parasitic tract through the left cerebral hemisphere. Medical management included a tapering anti-inflammatory dose of prednisone. Clinical signs improved slowly…

A 3.5-year-old healthy boy with 4 days of fever was referred to the emergency department for respiratory distress. The physical examination was remarkable for stupor, tachycardia, tachypnea, and dyspnea. Initial blood tests showed pancytopenia. He rapidly developed torticollis. Computerized tomography of the neck revealed a thrombus in the internal jugular vein. A presumptive diagnosis of Lemierre's syndrome was made and he was started on antibiotics and anticoagulation. He subsequently developed adult respiratory distress syndrome and required high frequency oscillatory ventilation for 9 days…

Hashimoto's encephalopathy may present with a wide variety of different neurological symptoms and signs. These include recurrent severe migrainous headache, psychoses, seizures, ataxia, dementia, stupor and coma. We present a personal series of 18 adult patients with Hashimoto's encephalopathy and a review of the literature in this paper. The natural history, laboratory abnormalities and neuroimaging data in these cases favour an immunopathological basis for this syndrome similar to relapsing acute disseminated encephalomyelitis. We suggest that Hashimoto's encephalomyelitis should be consider…

High-altitude cerebral edema (HACE) is a potentially fatal metabolic encephalopathy associated with a time-dependent exposure to the hypobaric hypoxia of altitude. Symptoms commonly are headache, ataxia, and confusion progressing to stupor and coma. HACE is often preceded by symptoms of acute mountain sickness and coupled, in its severe form, with high-altitude pulmonary edema. Although HACE is mostly seen at altitudes above that of the Denver/Front Range visitor-skier locations, we report our observations over a 13-year period of skier-visitor HACE patients. It is believed that this is a form…
