Postoperative weakness and anesthetic-associated rhabdomyolysis in a pediatric patient: a case report and review of the literature.

BACKGROUND: Anesthesia-associated rhabdomyolysis is a rare complication of surgery that causes postoperative myalgia, weakness, and potential renal failure if not managed promptly. Predisposing conditions that may lead to this complication include muscular dystrophies and myopathies. CASE PRESENTATION: This rare case describes a pediatric non-Indigenous Australian patient developing this complication, with no known predisposing risk factors, and no clear etiology. A 9-year-old child with a background of asthma underwent an elective removal of keloid scar on her chest wall. The procedure was b

[Prolonged apnea after suxamethonium administration during staphylococcal toxic shock].

A toxic shock syndrome occurred after a femoral nail removal requiring revision surgery. After administration of suxamethonium (1 mg.kg-1), an apnoea prolonged over 45 minutes was observed. The trachea was extubated 105 minutes after suxamethonium administration. For the nail removal, two days before, the anaesthetic had been given by the same anaesthesiologist, with a similar protocol. Apnoea extended over 20 minutes. The day of the revision surgery, plasma cholinesterase activity was 410 UI.L-1 and reached 910 UI.L-1, 9 months later. Dibucaine number was 20 and fluorure number 17. The apnoea

Anaphylactoid or carcinoid?

A patient with a carcinoid tumour and a history suggestive of carcinoid syndrome, but with no biochemical evidence in support, had a cardiovascular collapse during an anaesthetic with propofol and suxamethonium. Subsequent investigations suggested an anaphylactoid reaction to suxamethonium, but there were features in common with a carcinoid crisis. The necessity for a second anaesthetic soon afterwards posed a dilemma. In the event of a similar reaction during another anaesthetic, a management plan beforehand should include ready availability of appropriate drugs and the use of sympathomimetic

"Crash induction" in patients with full stomach.

The rationale of "crash induction" in patients with full stomach is reviewed. The technique does not precipitate regurgitation in normal patients having competent cardia, provided respiratory obstruction and IPPV are avoided during induction. On the other hand, in patients with incompetent cardia such as intestinal obstruction or hiatus hernia, excessive material may accumulate in the lower oesophagus. The accumulation will be suddenly released with the cricopharyngeal relaxation subsequent to "crash induction". The stomach and oesophagus should be adequately decompressed pre-operatively, and

Porcine malignant hyperthermia. II: Heat production.

The contributions of aerobic and anaerobic muscle metabolism to the heat produced in porcine malignant hyperthermia were studied in seven Pietrain pigs. Oxygen consumption measurements were used to calculate the increase in muscle temperature as a result of aerobic metabolism and this was compared with the observed muscle temperature. The results show that in the initial stage of porcine malignant hyperthermia heat production is largely aerobic in origin. Terminally, aerobic metabolism can account for about half the observed temperature increase.

[Mild form of malignant hyperthermia? OR, ABNORMAL REACTION To succinyldicholine?].

In addition to two published cases of malignant hyperthermia (M.H.) 13 more cases have been observed in this institution since 1971, which are probably early reactions of the M.H. type. All 13 patients were children, 3-12 years of age, who developed a rigor of the jaw muscles after succinyldicholine and/or halothane within minutes after the induction of anesthesia. Further symptoms that could be observed regularly were a tachycardia of several hours duration and a marked elevation of the serum CPK-activity with a peak after 24 hours. Since the anesthesia was terminted immediately and the opera

[Prolonged curarization with suxamethonium in a four-week old infant].

A case of a 28-day-old infant who developed suxamethonium apnoea is described. He was found to be homozygous for atypical cholinesterase. Main characteristics of this disorder are reviewed. Other causes of prolonged apnoea in infants recovering from anaesthesia for surgery of pyloric stenosis are discussed.

[Localized rhabdomyolysis after anaphylactic shock caused by suxamethonium].

A young woman without remarkable medical history, experienced a life threatening anaphylactoid reaction after induction of general anaesthesia for an emergency curettage. Hypersensitivity reaction involving IgE antibodies against suxamethonium was proven by positive skin test and detection of specific IgE by radio-immuno assay. After intensive therapy for 2 to 3 hours, the patient exhibited rhabdomyolysis localized on both calves. This complication, which required fasciotomies, did not result in renal failure. Local and general outcome was good. Rhabdomyolysis was due to compartmental ischaemi

Rhabdomyolysis and anesthesia: a report of two cases and review of the literature.

Rhabdomyolysis occurred in two apparently healthy boys aged 9.5 and 5.5 years after general anesthesia with suxamethonium. Mild hyperkalemia and renal failure were observed in the first patient, who was subsequently diagnosed with Becker dystrophy. In the second patient, the clinical presentation was not classic for malignant hyperthermia and a muscle biopsy failed to disclose any pathological finding. A review of the literature revealed 66 pediatric cases (56 boys and 10 girls) of anesthesia-associated rhabdomyolysis. Forty-nine (74%) cases were caused by an underlying, mostly unrecognized co

Suxamethonium-induced histamine release from the heart of naïve and suxamethonium-sensitized guinea-pigs: evidence suggesting spontaneous sensitization in naïve animals, and relevance to anaphylactoid reactions in man.

A guinea-pig model for anaphylactic and anaphylactoid reactions to suxamethonium was evaluated. 'Sensitization' to that drug was demonstrated both by cardiac anaphylaxis in the Langendorff preparation, and by serum antibody studies. Spontaneous sensitization to cross-reacting chemicals in a proportion of control animals is strongly suggested, somewhat akin to spontaneous sensitization in patients with anaphylactoid reactions to neuromuscular blockers on first exposure, and in whom IgE antibodies are detected.

Atopy and anaphylactic reactions to suxamethonium.

Muscle relaxants are widely used for general anesthesia and may be responsible for IgE-dependent anaphylactic reactions (AR). A controversial issue in the field of drug allergy is whether ARs are more frequent or severe in atopic subjects. Thus, we performed a case-control study comparing the distribution of various clinical and biologic signs of atopy. The case group included 32 patients with a history of AR to suxamethonium, the most commonly used muscle relaxant. The control group included 128 subjects, matched to the case group according to age, gender, and socioeconomic status. The case g
