Liver transplantation for patients with alcoholism and end-stage liver disease.

Liver transplantation for alcoholic cirrhosis remains controversial. In particular, criteria for the selection of patients who will remain recovered from alcoholism post-transplant require better definition. We analyzed the long-term predictive value of categorizing transplant referral patients with alcoholism and end-stage liver disease into risk groups for recidivism and noncompliance. Forty-seven patients with the diagnosis of alcoholism and advanced liver disease were evaluated and placed into predefined risk groups (low-, moderate-, and high-risk) for recidivism and noncompliance. No abso

Acute scleritis in porphyria cutanea tarda.

We examined three patients who had dermatologic and biochemical manifestations of porphyria cutanea tarda and localized thinning or excavation in the sun-exposed interpalpebral sclera, adjacent to the cornea. All three patients had signs of acute scleritis. The acute scleritis responded to oral indomethacin in one patient, but systemic corticosteroids were required to control the inflammation in the other two patients. Phlebotomy, protection from sunlight, and refraining from alcohol played an important part in the treatment of the patients.

Early alcoholism treatment: the Emmanuel Movement and Richard Peabody.

The history of alcoholism treatment in the early twentieth century is outlined. The methods of the Emmanuel Movement and of Richard Peabody … group support and self-help were important. Worcester tried to reduce patients' guilt and rejected temperance preaching. He felt that recovery must come from surrender to external forces and to the healing capacities of the unconscious. One patient of his, Courtenay Baylor, began to work with him at the E. Church. Like Worcester, Baylor believed that alcoho …

Crystal and Cryo-EM structure of PPL, a novel hexameric R-type lectin from the poisonous mushroom Pleurocybella porrigens.

Pleurocybella porrigens is a mushroom that grows widely around the temperate northern hemisphere, and was once considered edible, especially in Japan. Following a number of deaths in 2004, investigations revealed the presence of various toxins, including a lectin (PPL) that apparently survives cooking and enters the bloodstream via the stomach. We have cloned PPL and solved its structure by X-ray crystallography and cryo-EM. We report the sugar binding properties of this β-trefoil lectin, which has a novel hexameric structure.

Factors impacting vaccine hesitancy toward Coronavirus disease-19 (COVID-19) vaccination in Brooklyn, New York.

The Coronavirus disease-2019 (COVID-19) pandemic led to the development of several candidate vaccines. However, current research suggests that the potential of successful vaccines is tempered by vaccine skepticism or hesitancy. If vaccine efficacy is 80%, then the herd immunity required from vaccination is about 75-90%. The aim of the current study was to study factors impacting COVID-19 vaccine hesitancy in a representative sample of adults (age≥18 years) in a COVID-19 hotspotAbbreviations: COVID-19: coronavirus disease-19.

Cardiac Magnetic Resonance of Myocardial Involvement in Leptospirosis.

Leptospirosis is a zoonotic infection caused by the Leptospira interrogans. Although it is endemic in tropical countries, global incidence has increased in several temperate and developed regions. Here, we present a cardiac magnetic resonance (CMR) and multidetector computer tomography (MDCT) chest features of active systemic leptospiral infection in a 19-year-old male. The MDCT appearances of lungs and CMR appearances of myocardium in icteric leptospirosis are described. Early diagnosis and prompt treatment is important to manage the cardiothoracic complications.

Leptospirosis in Greece.

Leptospirosis is a zoonotic disease with increased public health concern worldwide. The disease occurs more often in temperate and tropical regions. The present study analyzes the demographic, epidemiological and clinical data of 168 leptospirosis cases laboratory diagnosed in northern Greece during 1998-2014. Most patients were males, aged 50-69 years, working in animal husbandry or farming. Cases were observed more frequently in summer and autumn. Severity of the disease was correlated with presence of pulmonary involvement and hemorrhagic manifestations.

Detection of Dirofilaria immitis in mid-western arid Argentina.

Dirofilariosis, caused by Dirofilaria immitis and D. repens, is (re-) emerging worldwide. Dogs are the main reservoirs, while human infection has recently become an important focus of interest and attention. In Argentina, canine D. immitis infection has been described in eastern and northern subtropical and temperate humid regions, but never reported in mid-western arid regions so far. In this research note we report for the first time the occurrence of autochthonous human and canine D. immitis infection in the region.

Should we be giving enhanced vitamin D intakes to all?

It is widely established that vitamin D is critical for bone health. There is also an increasing body of evidence from observational studies that low levels of vitamin D are associated with a range of other disorders, including cancer and cardiovascular disease. People in temperate climates are often deficient in vitamin D, particularly in wintertime. The key question is whether there is sufficient evidence to justify supplementing vitamin D intakes for all. In this 'Controversy in Medicine', two international experts argue the case 'for' and 'against' universal vitamin D supplementation.

Improving fixation of the osteoporotic fracture: the role of locked plating.

The use of locking technology has expanded significantly over the last decade. This technology has led to improvements in implant design for fixation in osteoporotic bone and allowed more secure and stable constructs. Locking plates and screws have been helpful in surgical repair of metaphyseal fractures and those with significant comminution and in the elderly. Biomechanically, creating a fixed-angle design leads to stronger constructs and potentially decreases failure rates. The use of this technology must be tempered by awareness of the complications associated with both the technique and i

First case of cutanous leishmaniasis in Nepalese patient.

Visceral Leishmaniasis (VL) is a serious disease caused by Leishmania donovani (LD) complex and prevalent in the temperate and tropical zones of the earth. VL, endemic in the southern plains of the 14 districts in the Terai region of Nepal, is considered a major public health problem. Cutanous leishmaniasis (CL) is prevalent mainly in the tropics and subtropics, affects nearly 1.5 million people worldwide. No reported cases of CL have been identified in Nepal until now. We report the first case of CL in a Nepalese patient.

Future vaccines: recent advances and future prospects.

The 20th century has witnessed the introduction of over 20 vaccines that prevent or even conquer diseases such as smallpox, polio, and measles. The continued threat of infectious diseases demands the creation of many more vaccines, especially against common respiratory and gastrointestinal pathogens. Thanks to recent advances in molecular biology, immunology, and adjuvant technology, the next decade likely will bring a vaccine for HIV/AIDS also. We enter the 21st century with a tempered optimism, proud of past achievements, but mindful of the challenges that lie ahead.

[Primary nephrotic syndrome in the black African child].

Idiopathic nephrotic syndrome (INS) in black African children differs from that of children in temperate areas. The main differences are the high rate of corticosteroid non-responders and the low rate of minimal change glomerulopathy in black African children, possibly related to a racial factor. The identification of a high corticosensibility in certain African regions (Togo and Ghana) can lead to the identification of an ethnic factor. Further genetic studies should be carried out in order to provide a better approach to INS in Africa.

[Autochthonous and imported amebiasis: description of 2 clinical cases].

Two cases of intestinal amoebiasis, one of which autochthonous and the other acquired in Africa, are described. The first of the patients, who never moved to the Tropics nor to Southern Italy, had profuse bloody diarrhea leading to hypovolemic shock; the second case was characterized by massive hemorrhage. Both responded well to appropriate treatment. Infection with E. histolytica should be borne in mind in patients with abdominal symptoms not only if they have travelled in the Tropics, but also in temperate regions, including Northern Italy.

Prophage-Derived Molecules as Anti-Vibrio Agents in Aquaculture: Mechanisms of Biofilm Control and Virulence Suppression.

Vibriosis causes substantial losses in marine and brackish-water aquaculture, while increasing antimicrobial resistance limits the effectiveness of conventional antibiotic treatment. Biofilm formation and quorum-sensing-controlled virulence further contribute to the persistence of pathogenic Vibrio species. Prophages integrated into Vibrio genomes encode proteins and regulatory elements that may provide alternative approaches for controlling these pathogens. This review evaluates endolysins, polysaccharide depolymerases, nucleases, holins, spanins, tailocins, regulatory proteins, and small RNA

A LysR-Type Transcriptional Regulator Links Phosphonate Metabolism to Cool Virulence of Ralstonia solanacearum in Tomato.

Ralstonia solanacearum, the causal agent of bacterial wilt, is a major threat to solanaceous crops in tropical and subtropical regions worldwide. While most strains are pathogenic at high temperatures (>28°C), a subset can cause disease at low temperatures (16-18°C), posing a risk to temperate solanaceous crops. Previous comparative genomics of cool virulent with regular tropical strains identified candidate transcriptional regulators associated with cool virulence. In this work, we tested function in virulence to tomato at high and low temperatures of four of the candidate transcription regul

Recurrent heatwaves reduce production but reveal species mixture‑mediated resistance and recovery in grasslands.

Current and future heatwaves threaten temperate grasslands as they impair plant temperature regulation, photosynthesis, and eventually lead to production losses. Species mixture could mitigate the negative effects of heatwaves as documented for drought. Our main objective was to test the effect of species mixture on grassland responses to heatwaves by comparing mixtures with monocultures. We conducted a greenhouse experiment to simulate four heatwave treatments: i) control ii) two mild heatwaves, iii) a severe heatwave, and iv) recurrent scenario combining the mild and severe heatwaves. We ana

Modeling the Effects of Temperature and Limiting Nutrients on the Competition of an Invasive Floating Plant, Pontederia crassipes, with Submersed Vegetation in a Shallow Lake.

The potential for a non-native plant species to invade a new habitat depends on broadscale factors such as climate, local factors such as nutrient availability, and the biotic community of the habitat into which the plant species is introduced. We developed a spatially explicit model to assess the risk of expansion of a floating invasive aquatic plant species (FAV), the water hyacinth (Pontederia crassipes), an invader in the United States, beyond its present range. Our model used known data on growth rates and competition with a native submersed aquatic macrophyte (SAV). In particular, the mo

The roles of root-nodulating bacterial associations and cyanogenesis in the freezing sensitivities of herbaceous legumes.

PREMISE: Reduced snow cover and increasing temperature variability can increase freezing stress for herbaceous plants in northern temperate regions. Legumes have emerged as a plant functional group that is highly sensitive to these changes relative to other herbaceous species in these regions. We explored root-nodulating bacterial associations and cyanogenesis as potential mechanisms explaining this relatively low freezing tolerance of legumes. METHODS: To examine the influence of bacterial associations, we grew four legume species with or without crushed-nodule inoculum at three severities o
