Tendon transfers in traumatic foot.

A clinical review was performed of 11 cases of traumatic foot treated by tendon transfer. Among 11 patients, there were eight males and three females, ranging from four to 36 years of age. The anterior tibial tendon was used in six cases for tendon transfer and among these, split anterior tibial tendon was used in three cases in children. The posterior tibial tenodn was used in two cases and the extensor digitorum longus tendon of the foot was in three cases. In six cases of soft tissue injury where tendon transfer was impossible, microvascular free cutaneous flap transplantation was combined

[Patellar and quadriceps tendon ruptures].

Ten cases of rupture of quadriceps tendon and seven cases of rupture of the patellar ligament were studied. All patients were reexamined. One must differentiate between rupture of the quadriceps tendon (tendon ruptures in elderly patients with degeneratively changed tendon in trivial trauma) and rupture of the patellar tendon (tendon rupture in younger patients with considerable trauma). All the patients were discussed with regard to accident mechanism, histology, underlying systemic and local diseases, therapy and results. There were good results in all patients with rupture of the patella te

[Closed tendon injuries of the hand].

Cases of subcutaneous tendon ruptures of the hand were compiled over a period of 10 years; the lesions were all traumatic rather than due to degenerative illness. The 914 injuries fall into two categories: 867 extensor and 47 flexor tendon ruptures. The localisation of the lesions is most often distal. The extensor tendon lesion is accompanied more often by a ruptured bony fragment; the distal flexor tendon is mostly torn-off bony fragment. The extensor tendon ruptures can be subdivided as follows: distal injuries of the extensor aponeurosis in the DIP joint (751 cases), IP joint (31), tractus

Bridge flexor tendon grafts.

In adults, when flexor tendons cannot be repaired until weeks or months after injury or when tendons have ruptured and part of the tendon is nonviable, it is often inadvisable to suture the tendon ends together. When a primary repair has failed, when there has been a delay in tendon reconstruction to allow skin coverage or joint mobilization, or when repair has been delayed because of infection, continuity of a profundus tendon or flexor pollicis longus tendon can often be restored by using a free graft to bridge the defect. Many patients treated by this procedure will have a satisfactory resu…

Rheumatoid extensor tendon ruptures.

Rheumatoid tendon ruptures occur primarily by the effect of the tenosynovium on the tendon or secondarily by the effect of the tenosynovium on the bone. An understanding of these complications combined with a knowledge of the anatomy helps to explain the pathology and suggest possible treatments.

Transposition of the posterior tibial tendon.

The posterior tibial tendon was rerouted by the technique described by Baker and Hill in 35 feet of children with a dynamic varus deformity due to spastic cerebral palsy. The average follow-up period was 11.4 years. In ten of the feet, rerouting of the posterior tibial tendon was the only procedure performed. Eight of the ten feet obtained a satisfactory correction. There were no overcorrection problems in these ten feet. In the remaining 25 feet, the Baker-Hill procedure was done concurrently with other procedures, such as lengthening of the triceps surae (22 feet), calcaneal osteotomy (two f

[Pathological study of the supraspinatus tendon].

To clarify the aging process and pathogenesis of rotator cuff tears, left supraspinatus tendons from 268 cadaveric specimens (171 men and 97 women, ages 0 to 87 years) were studied. The incidence of complete thickness tear was 6.7%, and that of incomplete thickness tear was 13.8% (bursal side tears: 2.6%, intratendinous tears: 7.5% and joint side tears: 3.7%). Microscopic examinations were conducted topographically in five sites near the supraspinatus insertion. In the articular side of the tendon, there were fewer tenocytes, fewer arterioles and more chondrocyte like cells than in any other p…

Acute calcific tendonitis in the hand.

Calcific tendonitis in the hand is an uncommon entity with a clinical presentation that is very similar to that of infection. Because of the similarity, the potential for misdiagnosis and mismanagement is high. An awareness of calcific tendonitis in the hand and the associated clinical and radiographic findings will aid in its early diagnosis and treatment.

Peroneus longus tendon lengthening.

This article presents lengthening of the peroneus longus tendon as an adjunctive procedure in the surgical correction of a plantarflexed first ray. The technique has enhanced the effectiveness of primary surgical procedures utilized in the correction of a plantarflexed first ray. Technical difficulty associated with lengthening of the peroneus longus tendon is minimal, and the incision placement introduced in this manuscript provides excellent exposure.

Tendon lesions in broiler fowls.

Tendon lesions have been studied in 35 broiler or broiler breeder fowls. Tendon rupture most commonly affected the gastrocnemius and flexor muscles but was also seen in cranial tibial muscles (four cases) and the patellar tendon (one case). In nine birds tenosynovitis, characterised by a marked inflammatory response, was identified. In others, however, rupture occurred without appreciable inflammation and these cases were concluded to be comparable to spontaneous tendon rupture in other species. A traumatic aetiology is suggested in these cases, as other pelvic limb tendons or ligament rupture…

Rupture of the tibialis posterior tendon.

The clinician should be aware of the possibility of a rupture of the tibialis posterior tendon in adults who have developed a unilateral flatfoot deformity. By careful physical examination, the diagnosis can readily be made. In more advanced cases roentgenographic changes of medial subluxation of the talar head and evidence of sagging of the talonavicular joint are noted on the lateral view. If conservative treatment fails to result in a satisfactory clinical response, I believe the treatment of choice is a flexor digitorum longus transfer through the navicular. If this procedure fails, bony s

Tibialis posterior tendon rupture.

Rupture of the tibialis posterior (TP) tendon occurs most commonly in adult women. It evolves as an insidious, painful, and progressive flatfoot deformity. Watching for such deformities as too many toes and including the single heel rise test in the examination will facilitate an accurate diagnosis. The treatment rationale should consider the site and extent of tendon disruption, as well as the presence of secondary deformity. Early treatment will help to prevent long-term functional impairment.

Subluxing biceps tendon in the athlete.

Subluxation or dislocation of the long head of the biceps tendon from the bicipital groove is one of the common causes of shoulder disability in the throwing arm of the athlete. This condition has characteristic symptoms; pain on throwing and relieved by rest; palpable snap at a certain point in the arc of motion. The signs are tenderness over the bicipital groove and a reproducible pop in rotation of the humeral head. The diagnosis can be quite secure and is often confirmed by X-rays profiling the bicipital tunnel. The remedy is a relatively nontraumatic operation which removes the biceps ten

Treatment of partial tendon lacerations.

We treated twenty six patients with thirty four partial tendon lacerations by not suturing the tendons and by early mobilization of the digit. These partial tendon lacerations varied from 25 to 95 per cent of the cross sectional area. The mean and median cross-sectional area lacerated was 60 per cent. Twenty three of these patients obtained excellent function, one patient obtained good to excellent function and one patient obtained fair function. One patient was lost to follow-up. No tendon ruptured but one patient did develop trigger finger which spontaneously resolved. Partial flexor tendon …

Habitual extensor tendon dislocation.

Dislocated tendons are frequently seen in the rheumatoid hand. Habitual dislocation is rare. Five patients, all sero-negative with this problem, are described. The causation of the deformity and its treatment is discussed in relation to other cases reported in the literature. An extensor loop operation is recommended.

Tendon transfers in the paralytic hip.

We analyzed the results of transfers of the iliopsoas or external oblique muscles performed to augment the abductor power of the hips in 149 patients with flaccid paralysis of the hips. More than 800 supplemental procedures were performed on the lower extremities of these patients. Twenty-three of the thirty-eight hips that had been subluxated or dislocated became stable in the reduced position after the transfer operation. Every hip that had been stable before the transfer remained stable, and thirty-seven patients who had been brace-dependent became brace-free. We concluded that transfers of

Recurrent dislocation of the peroneal tendon.

Recurrent dislocation of the peroneal tendons is an important, although infrequent, cause of disability of the ankle. We used a simple procedure that corrects the basic deformity of a shallow peroneal groove, without the use of metallic fixation or transfer of a tendon or ligament. The groove for the tendons is deepened by removal of some inner fibular substance, while the smooth tenosynovial channel is maintained as an intact periosteal flap on the fibula. The technique was used successfully in ten cases.
