Thoracoscopic mediastinal lymph node dissection using an endoscopic spacer.

Although fewer lymph nodes are dissected with video-assisted thoracic surgery than with open lobectomy, thoracoscopic lobectomy is increasingly becoming the preferred surgical approach for early-stage lung cancers. The endoscopic surgical spacer SECUREA™ is a medical device that has been effectively employed in laparoscopic surgery, but no study has evaluated its efficacy in thoracoscopic surgery. In this report, we demonstrate the utility of SECUREA for complete thoracoscopic mediastinal lymph node dissection in patients with non-small cell lung cancer.

[Successful Conservative Management of Bronchopleural Fistula after Intraoperative Bronchial Injury].

A 64-year-old woman diagnosed as primary lung cancer was admitted for surgery. Right lower lobectomy and ND2a-1 nodal dissection was performed under video-assisted thoracic surgery( VATS). The membranous portion of intermediate bronchus was injured about length of 5 mm while dissecting subcarinal lymph nodes. The fistula was closed by knotted suture using 4-0 polydioxanone (PDS) and covered with pericardial fat pad. Although the postoperative course was uneventful and discharged at postoperative day (POD) nine, bloody sputum appeared and right pneumothorax developed at POD 11. Bronchoscopy rev

Spontaneous chylous cardiac tamponade: a case report.

BACKGROUND: Chylous cardiac tamponade is a rare condition with little known cause. CASE PRESENTATION: A case of an otherwise healthy woman who admitted with dyspnea and palpitations is presented. She had a history of a painful flexion-hyperextension of the spine. Diagnostic evaluation proved a chylous pericardial effusion with a disruption of the anterior longitudinal spinal ligament. Video-assisted thoracic surgery with mass supradiaphragmatic ligation of the thoracic duct and pericardial window formation was carried out successfully and resulted in the complete cure of the patient's conditi

Quadrilateral space syndrome: a rare complication of thoracic surgery.

We report a case of quadrilateral space syndrome related to thoracic surgery. A 21-year-old man underwent video-assisted thoracic surgery for a left-sided pneumothorax. After the operation, he presented with difficulties in left arm abduction and paresthesia over the lateral aspect of the shoulder and upper arm. Deltoid muscle atrophy and tenderness over the quadrilateral space were also observed. On further examination, he was diagnosed with isolated paralysis of the axillary nerve, the so-called quadrilateral space syndrome. This is a rare complication, but it interferes with the activities

Minimally invasive Ivor Lewis oesophagogastrectomy in a patient with situs inversus totalis†.

Situs inversus totalis (SIT) is a rare congenital condition in which the internal organs of the thoracic and abdominal cavities experience a right-to-left reflection across the sagittal plane. We describe a case of locally advanced adenocarcinoma of the oesophagus treated with minimally invasive oesophagectomy using a laparoscopic and left video-assisted thoracoscopic surgery approach in a patient with situs inversus totalis.

A video-assisted thoracic surgical lobectomy for a lung tumour with a rare anomalous pulmonary artery.

A detailed knowledge of anomalies of the anatomical course of the pulmonary vessels is extremely important for video-assisted thoracic surgery (VATS). Inadvertent vascular ligation and dissection are catastrophic for patients undergoing lobectomy. We describe a case with a rare anomaly of the left pulmonary artery and highlight the importance of identifying the courses of pulmonary vessels for VATS lobectomy.

Robot-assisted resection of pulmonary sequestrations.

Pulmonary sequestration is a rare congenital malformation and may cause recurrent infections and hemoptysis. Although video-assisted thoracic surgery (VATS) is feasible, some drawbacks remain, mainly dealing the managing of anomalous vessels. We describe the use of a robotic system (da Vinci Robotic System, Surgical Intuitive, Mountain View, CA, USA) in the treatment of four consecutive cases of pulmonary sequestration.

Video-assisted mitral surgery through a micro-access: a safe and reliable reality in the current era.

BACKGROUND AND AIM OF THE STUDY: Minimally invasive mitral valve surgery was introduced into clinical practice during the mid 1990s. The clinical benefits of the technique, namely a reduction of surgical trauma, increased patient comfort and shorter hospital stay, are achieved by using a video-assisted, minithoracotomy approach rather than a standard median sternotomy. Herein is described the authors' experience with video-assisted mitral surgery through a micro-access. METHODS: Between September 2003 and September 2006, 100 patients (mean age 65.7 years; range: 16-84 years; 29 aged >75 years

Validity of the updated European Society of Thoracic Surgeons staging guideline in lung cancer patients.

OBJECTIVE: The European Society of Thoracic Surgeons (ESTS) has proposed a revised preoperative lymph node staging guideline for patients with potentially resectable non-small cell lung cancer (NSCLC). We aimed to assess the validity of this revised ESTS guideline and survival results in our patient cohort. METHODS: A total of 571 patients with potentially resectable NSCLC seen between January 2004 and November 2013 were included in the study. The preoperative mediastinal staging was performed by video-assisted cervical mediastinoscopy or video-assisted mediastinoscopic lymphadenectomy in all

Surgical techniques for thoracoscopic secondary carinal reconstruction with stepwise barbed sutures.

Parenchymal-preserving surgery is the preferred technique for selected patients with centrally located airway tumors limited to the secondary carina; however, reconstruction is challenging. We present a simplified video-assisted thoracoscopic surgery technique for secondary carinal reconstruction with three barbed sutures on each side. In each patient, the first thread was used to approximate the main bronchus and lower lobe bronchus on the left side or the bronchus intermedius and upper lobe bronchus on the right side. A horizontal mattress suture was critical to secure the backward joint sec

Uniportal Robotic-Assisted Sleeve Resection of the Left Main Bronchus and Secondary Carinal Reconstruction for Typical Carcinoid Tumour.

Bronchial sleeve resection is the preferred technique over pneumonectomy for centrally located tumours, preserving lung function and improving survival. We present a uniportal robotic-assisted thoracic surgery (RATS) technique for left main bronchus (LMB) sleeve resection with secondary carinal reconstruction in a patient with a typical carcinoid tumour. Through a single 4 cm incision, the LMB and its bifurcation were resected and reconstructed using a continuous absorbable barbed suture. Postoperative recovery was uneventful, with chest drain removal and discharge on postoperative day 2. Fina

Bronchoscopy and surgical staging procedures and their correlation with imaging.

Bronchoscopy, computed tomography (CT) and surgical staging procedures are complimentary methods of investigating patients with lung cancer. CT has been shown to be of value prior to bronchoscopy in the investigation of haemoptysis and malignancy, with excellent correlation between the detection of disease within the large airways on CT and direct visualisation at bronchoscopy. The utility of CT has been further increased by the development of multislice scanners with the generation of volumetric data enabling multiplanar image acquisition. Additionally the advent of CT co-registered with posi

Case Report: Occult splenic rupture during left-sided VATS decortication: diagnostic role of early perioperative FAST.

Video-Assisted Thoracic Surgery (VATS) is the preferred method for many thoracic diseases, featuring advantages of minimal trauma, reduced postoperative pain, and low complication rates. However, it carries potential risks, including splenic rupture-a rare but life-threatening complication. A 69-year-old male patient developed hemodynamic instability during VATS for left-sided empyema clearance; intraoperative exploration failed to identify an obvious bleeding source. Early post-procedural FAST bedside focused assessment with sonography for trauma (FAST) confirmed splenic rupture, which was ma

The safety and versatility of video-thoracoscopy: a prospective analysis of 895 consecutive cases.

BACKGROUND: The application of video-endoscopy to general thoracic surgery is radically changing the approach to many benign and malignant diseases of the chest. Since July 1991, we have performed 794 purely thoracoscopic and 101 video-assisted thoracic surgical (VATS) procedures on 860 patients. STUDY DESIGN: Comprehensive, prospectively acquired data examining the specific indications for and outcomes of this new technique were prospectively entered into a thoracic surgical database. Preoperative, intraoperative, postoperative, and outcome variables were studied for the entire group as well

Patient experiences in trials of minimally invasive thoracic surgery: A mixed-methods study.

BACKGROUND: Minimally invasive thoracic surgery, including robotic-assisted and video-assisted approaches, is increasingly used to treat lung cancer. While technical outcomes are well documented, patient experiences within clinical trials remain under-explored. OBJECTIVE: To evaluate patient experiences of clinical trial participation involving advanced surgical modalities for lung cancer. METHODS: This two-site, mixed-methods study evaluated patient perceptions of participation in the DURATION (ctDNA monitoring) and VGT-309 (fluorescent imaging) trials. Twenty-six participants completed quant

Pulmonary Artery Bleed from an Eloesser flap.

An open thoracic window involves the creation of a semi-permanent pleural-cutaneous connection to treat chronic organising empyemas, bronchopleural fistulas and postpneumonectomy empyemas in patients too ill for video-assisted thoracic surgery decortication. An Eloesser flap is a type of open thoracic window. Antibiotics have made the use of Eloesser flaps and other open thoracic windows for treatment of empyemas rare. Consequently, not much is known about the chronic complications of open thoracic windows. However, structural compromise of vasculature secondary to chronic inflammation has bee

Image-guided navigation and video-assisted thoracoscopic spine surgery: the second generation.

OBJECT: Video-assisted thoracoscopic surgery (VATS) has evolved for treatment of a variety of spinal disorders. Early incorporation with image-guided surgery (IGS) was challenged due to reproducibility and adaptability, limiting the procedure's acceptance. In the present study, the authors report their experience with second-generation IGS and VATS technologies for anterior thoracic minimally invasive spinal (MIS) procedures. METHODS: The surgical procedure is described in detail including operating room set-up, patient positioning (a lateral decubitus position), placement of the spinal refer

Hybrid surgery for intralobar pulmonary sequestration with aortic aneurysm.

A 62-year-old man with past history of chronic atrial fibrillation, congestive heart failure, and pneumonia was referred to our hospital for further workup of pulmonary sequestration. Enhanced computed tomography revealed intralobar pulmonary sequestration (IPS) of the lower left lobe, as well as aortic aneurysmal dilation at the origin of the aberrant feeding artery. We performed a hybrid operation consisting of thoracic endovascular aortic repair and excision of the IPS and left lower lobe by video-assisted thoracic surgery. The patient was discharged 5 days later without complications.
