[Bleeding during central venous catheterization : Cannot intubate, cannot ventilate due to massive cervical hematoma].

Central venous catheterization is an invasive procedure which can be associated with severe complications. These include in particular unsuc … ed and the subsequent hypoxia led to cardiac arrest that was only resolved after emergency surgical tracheotomy during cardiopulmonary resuscitation.

Laryngotracheal reconstruction.

Laryngotracheal reconstruction (LTR) along with cricotracheal resection and thyrotracheal anastomosis has become the standard of care for symptomatic subglottic stenosis in the pediatric age group. Success rates in achieving decannulation or avoiding tracheotomy approach 90%. Fearon and Cotton introduced pediatric LTR in 1972 using cartilage interposition grafting. The procedure has evolved to include a variety of techniques for expanding the laryngotracheal complex to obtain a stable airway of sufficient size for respiration. In this chapter, the authors will describe their single and double-…

Subglottic cysts in a patient with recurrent stridor and respiratory distress.

Subglottic cysts are rare causes of stridor in infancy and should be suspected in the case of intubation. A 15-month-old male presented with … by rupturing the cysts with bronchoscope, whereas development of a fibrotic stenosis may require a tracheotomy, with its attending morbidity.

Airway obstruction secondary to tracheobronchial involvement of asymptomatic undiagnosed Crohn's disease in a pediatric patient.

Tracheal involvement in Crohn's disease is extremely rare and can present with airway obstruction in any age group, including the pediatric patient. We describe the case of a 9-year-old female who presented with a 2-week history of intermittent stridor, resolving temporarily with the administration of systemic steroids. The patient denied any abdominal complaints and was otherwise healthy. Bronchoscopy revealed progressive tracheal inflammation necessitating an urgent tracheotomy. Subsequent laboratory investigation confirmed the diagnosis of Crohn's disease. This is the first reported case of…

[Means and organisation of the management of severe forms of COPD].

Chronic obstructive pulmonary disease is becoming the 3rd aetiology of death by disease. In its advanced presentation, it generates a chronic respiratory failure with hypoxia and frequent hypercapnia. Its complex management implies at that stage a medical treatment, physiotherapy and pulmonary rehabilitation, associated with long-term oxygentherapy. In the most severe stages, may be discussed long-term home mechanical ventilation with non invasive connection in order to avoid tracheotomy and to prevent acute respiratory episodes leading these patients in intensive care units and yet responsibl…

[ARDS complicated severe chest trauma: a review of 12 cases].

A total of 140 patients of thoracic trauma were admitted and treated in our hospital from 1970 to 1988. Thirty cases conformed the diagnosis of severe chest injury. Among them 12 patients (40%) were complicated by the adult respiratory distress syndrome (ARDS) with two deaths (16.7%). The diagnosis and treatment of ARDS following severe chest trauma was often-times delayed by the overlapping clinical manifestations of severe chest injury. Patient's condition usually deteriorates rapidly. Authors suggest early tracheotomy and appropriate mechanical ventilation as one of the major measures to de…

Management of subglottic stenosis in children.

We treated 21 children for subglottic stenosis at the Texas Children's Hospital from 1975 to 1983. Ages ranged from newborn to 14 years. Fifteen (71%) were younger than 13 months of age. Prolonged intubation was thought to be a primary contributing factor in 16 of 21 (76%). Nineteen (90%) required tracheotomy. Of these, 11 (58%) were decannulated, four (21%) have not been decannulated, and four (21%) were lost to follow-up. Two of the 11 children who were successfully decannulated required laryngotracheoplasty or thyrotomy. There were three instances of complications and no deaths.

Acute epiglottitis in adults. A management protocol based on a 17-year material.

During a 17-year period 75 adult patients were treated for acute epiglottis. Twenty of the patients had respiratory symptoms and in 13 patients an artificial airway was established by means of nasotracheal intubation or tracheotomy. One patient died of suffocation and one other patient had severe incapacitating brain damage because of cardiac arrest in combination with respiratory obstruction. All the other patients were discharged from the hospital without any sequelae. On the basis of our 17-year material and the literature, a protocol for the handling of patients with suspected acute epiglo…

[Dysphagia in Tracheostomized Patients after Long-Term Mechanical Ventilation - Become Sensitive to Reduced Pharyngo-Laryngeal Sensitivity].

Independent of the type of critical illness, tracheostomized patients have a high risk of developing a dysphagia. This is potentially life-threatening as it can lead to aspiration and pneumonia. It is therefore essential to perform swallowing diagnostics by means of a bolus dyeing test and/or FEES before oral feeding. Since a physiological airflow through the larynx and adequate subglottic pressure are key components of an effective swallowing act, oralisation should be avoided as far as possible with a blocked tracheal cannula.

Surgical treatment of dysphagia.

The role of surgery in the management of dysphagia is clear in some areas and controversial in others. Evaluation for the causes of dysphagia can elucidate conditions in which surgery can improve safety, quality of life, or both. Surgical therapy, when indicated, is safe and effective for many causes of dysphagia. This article includes a general overview of the causes of dysphagia that can be addressed successfully with surgery as well as a discussion of why surgery may be less appropriate for other conditions associated with dysphagia.

[Congenital laryngeal stridor].

We reviewed the diagnosis, complications and treatment of congenital laryngeal stridor (CLS), in 97 patients who consulted our clinic between 1991 and 2001. The 97 patients were diagnosed with laryngeal malacia (32%), vocal cord paralysis and laryngeal stenosis (22%), a neoplastic disease like hemagioma and papilloma (11%), or cystic disease (7%). The cases with vocal cord paralysis, laryngeal stenosis or laryngeal cysts were usually diagnosed within 2 months of birth based on severe dyspnea. Two of the 31 cases of laryngeal malacia and 2 of the 22 cases of vocal cord paralysis were associated

[Congenital laryngo-tracheo-esophageal cleft].

A typical case of congenital laryngo-trachea-esophageal cleft (LTEC) is presented with a Review of the literature. LTEC is a rare congenital anomaly caused by defective fusion of the septum between larynx/trachea and hypopharynx/esophagus. The septum is formed by fusion of two lateral folds growing medially in very early foetal life. Fusion progresses in a cranial direction. Disturbances in septum formation result in LTEC. The disease gives respiratory problems with aspiration and excessive salivary production. The diagnosis is best made by intubating the larynx and examining the postcricoid r

An alternate method for fiberoptic bronchoscopic examination of the intubated patient.

Fiberoptic bronchoscopy can be performed easily on patients requiring mechanical ventilation through endotracheal or tracheostomy tubes greater than 8 mm inside diameter. In patients with smaller tubes the procedure can still be performed by passing the bronchoscope transnasally and then through the glottis alongside the endotracheal tube.
