A modified method of traction for young children with congenital dislocation of the hip as a preliminary to reduction.

Many authors agree that preliminary traction prior to closed or open reduction for congenital dislocation of the hip is helpful. Different ways of traction have been used and each of them has its own advantages and disadvantages. One of the problems in the very young child is the difficulty in maintaining a suitable traction that is biomechanically effective. We found that using a rocker bed made the traction more "user friendly" for the child, the parent and the doctors.

Effects of fibrovascular traction and pooling of tears on corneal topographic changes induced by pterygium.

AIM: To investigate the effects of fibrovascular traction and the pooling of tears at the pterygium apex on the corneal topographic changes induced by pterygium. METHODS: A total of 16 eyes of 14 cases with primary pterygium were included in the study. A computerized corneal topography system was used for corneal topography examinations. Baseline keratographs were taken two times at straight gaze. A repeat corneoscope photograph was immediately obtained in temporal gaze. Then the tears at the pterygium apex were dried with a cellulose sponge, and a new corneoscope photograph was immediately o…

Traction splinting of femoral shaft fractures in a paediatric emergency department: time is of the essence?

OBJECTIVE: To describe the use of traction splinting in children with femoral shaft fracture and to determine if timing of traction splinting application effects outcome. METHODS: A retrospective descriptive study conducted over a five and a half year period (1 January 1996 to 1 July 2001) on children presenting with femoral shaft fracture to a Paediatric trauma centre. Data were collected on all children with a radiological diagnosis of fracture to the femoral shaft. Evidence for hypovolaemic shock and neurovascular compromise was sought. The administration of parenteral analgesia and whethe…

Optical coherence tomography of tractional macular elevations in eyes with proliferative diabetic retinopathy.

PURPOSE: To document the findings of optical coherence tomography in eyes with tractional macular elevation associated with proliferative diabetic retinopathy, paying special attention to differentiating between tractional retinal detachment and retinoschisis. DESIGN: Observational case series. METHODS: We retrospectively reviewed images of optical coherence tomography obtained from 17 consecutive eyes (17 patients) with proliferative diabetic retinopathy without vitreous opacity and showing tractional macular elevation with or without foveal involvement. RESULTS: Tractional retinal detachm…

[The influence of the traction on the wrist in the course of healing of the radius distal part comminuted fractures].

Traction and external fixation are the methods of choice by treating the comminuted fractures of the distal part of the radius. Bringing in two screws into the second metacarpal bone and the radius proximity from the fracture makes possible the traction and fixate the fractured bone splinters. The reposition of the comminuted fractures needs a very strong traction and provides to expand the ligaments and the wrist articular capsula. The aim of our study is to explain the mechanical influence of the external stabilization on the wrist and the wrist function after the immobilisation with the dis…

The role of magnetic resonance imaging in the management of traction injuries to the adult brachial plexus.

Magnetic resonance imaging (MRI) of the cervical spine and brachial plexus was performed on 26 consecutive patients presenting with traction injuries of the brachial plexus during 1996 and 1997. These included T1 and T2 weighted coronal, sagittal and axial images of the cervical spine and coronal images of the brachial plexus. The results were compared with surgical findings, intraoperative neurophysiology, and subsequent clinical progress. Operations for exploration and repair have been performed in 23 and 26 patients scanned. Evidence of root avulsion was seen in 11 patients in the form of d…

Successful treatment of tractional corectopia using 2 mJ of energy with an Nd:YAG laser.

Although the Nd:YAG laser is most commonly used to perform posterior capsulotomies after cataract surgery, it has also been used to treat a variety of other anterior segment abnormalities including tractional corectopia, iridocorneal adhesions, persistent pupillary membranes, and posterior synechiae. Numerous reports on the use of the Nd:YAG laser to treat structures in the anterior segment have emphasized the need to use higher pulse energy for pupillary membranes, compared with the lower settings required for posterior capsulotomy. Steinert and Puliafito noted that single pulses of 4 to 12 m…

Is preoperative tibial traction responsible for peroneal nerve palsy in patients with a fractured hip?

A study of 456 cervical hip fractures treated with internal fixation was performed to evaluate the incidence of paralysis of the peroneal nerve following hip-fracture treatment. There were no cases of peroneal nerve paralysis in a group of 206 patient treated with a dynamic hip screw without preoperative traction. There were 2 cases of peroneal nerve paralysis in a group of 250 patients treated with 3 parallel screws with preoperative skeletal traction via a tibia pin. This difference was not significant according to Fischer's test (two-tailed P value = 0.5036). We conclude that the incidence …

[Vitreoretinal proliferation as a cause of tractional retinal ablation after severe perforating injury].

The main idea of this paper was to investigate the influence of proliferative reaction in vitreous body, in case of serious injuries of the eyeball on the development of complications. It was shown that the presence of hemophthalmia which intensified PVR process and anteroposterior traction influenced the intensity of PVR and also the development of retinal ablation. The complete perforation of the posterior wall of the eye ball (duplex perforation) with the small foreign body would produce bigger scar on the posterior wall than in case of singular perforations, which would usually be enough t…

Nerve grafting for traction injuries of the common peroneal nerve. A report of 17 cases.

We report 17 patients who had grafting of the common peroneal nerve after traction injuries. Sixteen were reviewed at a median follow-up of 36 months. The nerve gap ranged from 7 to 20 cm. A functionally satisfactory result was achieved in six patients (37.5%), a fair result in six and a poor result in four. Seven patients had, in addition, a posterior tibialis tendon transfer; this improved the result in five. Grafting produced some satisfactory results even when the nerve gap was as long as 20 cm.

[Effects of caudal traction of the spinal cord on evoked spinal cord potentials in the cat].

This study attempts clarify the mechanism of neurological deficits in tethered cord syndrome using evoked spinal cord potentials (ESCPs). ESCPs in response to both sciatic nerve (SN-ESCP) and spinal cord stimulation (SC-DESCP) were recorded from the dorsal epidural space. With a fixed degree of caudal traction on the spinal cord in ten cats for 2-4 hours, ESCPs were increased in amplitude in the N1 and N2 deflections of the SC-DESCPs to 158% and 154% at L5 and decreased to 91% and 76% after transient augmentation at L3. On the other hand, the amplitude in the N1 deflection of the SN-ESCPs at L…

Management of unreduced traumatic posterior dislocation of the hip: heavy traction and abduction method.

Eight cases of unreduced traumatic posterior dislocation of the hip were treated by heavy traction and abduction. In four out of eight cases a concentric reduction was obtained. In two patients where a concentric reduction was not obtained, open reduction was performed. In these patients, there was an excellent or good result. In the remaining two patients a non-concentric reduction was accepted, and they had poor clinical results.

[Commentary on the article "Is the mechanical traction of the cervical spine still acceptable?" by R. Dorian ].

Referring to literature Dorian reported on severe complications after apparative traction of the cervical vertebral column up to cross-sectional paralysis and sudden death during treatment. Based on these reports he presents a list of contraindications which comprises nearly all diseases of the cervical vertebral column. The author summarizes his statements claiming that the apparative traction at no means can be a routine method because of it's inherent extreme risks. The comment in hand states that none of the complications claimed have been exemplified by the author in the original paper. N…

Hinged cast and roller traction for fractured femur. A system of treatment for the Third World.

Hinged casts and roller traction were used in two developing countries to treat fractured femora, most of which were due to road traffic accidents or civil violence. This method of treatment, developed by Neufeld, is particularly useful in the Third World because it uses local materials, adapted in a hospital workshop, and circumvents the difficulties and complications of standard traction and of operative treatment. The results are reported from 11 patients treated in Uganda in 1979 and from 110 treated in the Dominican Republic in 1981 and 1982. All but one fracture united without complicati…

Trauma or treatment? The role of intermittent traction in the treatment of cervical soft tissue injuries.

Neurophysiology does not provide a satisfactory theory which explains the phenomenon of muscular "spasm" which is said to be present in the neck following soft-tissue injury. Lacking knowledge as to whether long continued intermittent traction--ranging from 10 pounds to total body weight pull--is therapeutic or traumatic, such treatments nevertheless are prescribed in physiotherapy departments and at home for months. They are believed to be non-physiological and irrational and, in the author's opinon, represent the persistence of several medical myths associated with the "rear-end" collision. …
