"The right thing for our patients": perspectives of physical therapists transitioning to a 7 day a week service in an urban acute care center in Canada.

BACKGROUND: True seven day a week acute care physical therapy (PT) coverage is rare. Our facility is one of a few in Canada that has increased weekend PT coverage on medicine units to seven days of full PT staffing levels. OBJECTIVE: This article investigates the perspectives of physical therapists involved in the change with the focus on the emotional experiences of those therapists. METHODS: Thematic analysis of interview and focus group transcripts with 18 physical therapist and 2 manager participants were aligned with four themes of: 1) optimism, hope and excitement; 2) frustration, guil

Falling Through the Cracks: The Current Gap in the Health Care Transition of Patients With Kawasaki Disease: A Scientific Statement From the American Heart Association.

Background Health care transition (HCT) is a period of high vulnerability for patients with chronic childhood diseases, particularly when patients shift from a pediatric to an adult care setting. An increasing number of patients with Kawasaki disease (KD) who develop medium and large coronary artery aneurysms (classified by the American Heart Association according to maximal internal coronary artery diameter Z-scores ≥5 and ≥10, respectively) are becoming adults and thus undergoing an HCT. However, a poor transition to an adult provider represents a risk of loss to follow-up, which can result

Formerly Incarcerated Community Health Workers Engaging Individuals Returning From Incarceration Into Primary Care: Results From the Transition Clinic Network.

Over half a million individuals return from United States prisons and millions more from jails every year, many of whom with complex health and social needs. Community health workers (CHWs) perform diverse roles to improve health outcomes in disadvantaged communities, but no studies have assessed their role as integrated members of a primary care team serving individuals returning from incarceration. Using data from participants who received primary care through the Transitions Clinic Network, a model of care that integrates CHWs with a lived experienced of incarceration into primary care team

Handoffs and transitions in critical care-understanding scalability: study protocol for a multicenter stepped wedge type 2 hybrid effectiveness-implementation trial.

BACKGROUND: The implementation of evidence-based practices in critical care faces specific challenges, including intense time pressure and patient acuity. These challenges result in evidence-to-practice gaps that diminish the impact of proven-effective interventions for patients requiring intensive care unit support. Research is needed to understand and address implementation determinants in critical care settings. METHODS: The Handoffs and Transitions in Critical Care-Understanding Scalability (HATRICC-US) study is a Type 2 hybrid effectiveness-implementation trial of standardized operating

Early Start of Anti-Dementia Medication Delays Transition to 24-Hour Care in Alzheimer's Disease Patients: A Finnish Nationwide Cohort Study.

BACKGROUND: Dementia is one of the strongest predictors of admission to a 24-hour care facility among older people, and 24-hour care is the major cost of Alzheimer's disease (AD). OBJECTIVE: The aim of this study was to evaluate the association of early start of anti-dementia medication and other predisposing factors with 2-year risk of transition to 24-hour care in the nationwide cohort of Finnish AD patients. METHODS: This was a retrospective, non-interventional study based on individual-level data from Finnish national health and social care registers. The incident cohort included 7,454 A

Transition Care in Anorexia Nervosa Through Guidance Online from Peer and Carer Expertise (TRIANGLE): Study Protocol for a Randomised Controlled Trial.

UNLABELLED: TRIANGLE is a multicentre trial investigating whether the addition of a novel intervention for patients and carers (ECHOMANTRA) to treatment as usual (TAU) improves outcomes for people with anorexia nervosa (AN). ECHOMANTRA is based on the cognitive interpersonal model of AN and includes assessments, workbooks, videos, online groups and joint Skype sessions for patients and carers. People receiving intensive hospital treatment (N = 380) will be randomised to TAU or TAU plus ECHOMANTRA. Participants will be assessed over an 18-month period following randomisation. Transition to parenthood in the neonatal care unit: a qualitative study and conceptual model designed to illuminate parent and professional views of the impact of webcam technology.

BACKGROUND: Complications during pregnancy, childbirth and/or the postnatal period may result in the admission of a baby to a neonatal unit (NNU). While the survival and long-term prospects of high-risk infants are enhanced by admission, the enforced separation of the parent and child may have psychological consequences for both. There is a need to develop and evaluate interventions to help parents 'feel closer' to their infants in circumstances where they are physically separated from them. In this paper we present findings from an in-depth, theoretically-driven, evaluation of a technological

Factors Influencing Risk of Homelessness among Youth in Transition from Foster Care in Oklahoma: Implications for Reforming Independent Living Services and Opportunities.

Research suggests that youth aging out of foster care may be at higher risk of experiencing homelessness than other youth. Among this already at-risk population there may be certain characteristics that further exacerbate the risk. This paper uses data collected from various local and state agencies to further examine significant predictors of homelessness among youth who have aged out of foster care.

Psychosocial pathways to sexually transmitted infection risk among youth transitioning out of foster care: evidence from a longitudinal cohort study.

PURPOSE: To test the fit of a theoretically driven conceptual model of pathways to sexually transmitted infection (STI) risk among foster youth transitioning to adulthood. The model included (1) historical abuse and foster care experiences; (2) mental health and attachment style in late adolescence; and (3) STI risk in young adulthood. METHODS: We used path analysis to analyze data from a longitudinal study of 732 youth transitioning out of foster care. Covariates included gender, race, and an inverse probability weight. We also performed moderation analyses comparing models constrained and u

Transition in high-acuity clinical learning environments: A qualitative meta-synthesis of newly graduated nurses in intensive care units and emergency departments.

AIM: To synthesise qualitative evidence on newly graduated nurses' transition experiences in intensive care units and emergency departments and identify implications for clinical education and learning-environment design. BACKGROUND: Transition to practice among newly graduated nurses is well documented, but evidence focused on high-acuity clinical learning environments remains fragmented. Intensive care units and emergency departments combine rapid decision-making, technological complexity and emotionally demanding care. DESIGN: A qualitative systematic review and meta-synthesis using Joann

Post-transition Care Retention and Viral Suppression Outcomes Among Youth Living with HIV in South Carolina: A Real-World Data Research.

Youth living with HIV (YLWH) face substantial challenges following transition from pediatric to adult HIV care, yet evidence on post-transition outcomes in the Southern United States remains limited. Using statewide HIV surveillance and administrative data, we conducted a retrospective cohort study of 493 YLWH in South Carolina (SC) who transitioned to adult care between 2005 and 2021. Retention in adult care was defined as ≥ 2 viral load or CD4 tests at least 6 months apart within one year of the first adult visit, and viral suppression as a last recorded viral load < 200 copies/mL within one

Healthcare Transition From Pediatric to Adult Care Among Youth Living With HIV in South Carolina: Retrospective Study From 2005 to 2021.

PURPOSE: As more youth with HIV (YWH) survive into adulthood, successful healthcare transition (HCT) from pediatric to adult care is crucial for optimal health outcomes. This study aimed to characterize YWH transition status and identify individual- and county-level predictors. METHODS: Statewide HIV surveillance data in South Carolina were used for analysis. The first outcome, "ever transition," indicated whether a patient had at least one adult HIV care visit after the last pediatric care before age 26. Among those who transitioned, a successful transition was defined as timely linkage and

French consensus statement on transition of adolescent and young adults with rare pulmonary disease from paediatric to adult care: a Delphi method study.

BACKGROUND: Transition of adolescents with chronic diseases to adult care is at risk of health complications and loss of medical follow-up. There is currently no official general consensus specific to rare pulmonary diseases. We aimed at setting up a consensus of experts to establish a consensus statement for the transition of patients with rare pulmonary diseases in France. METHODS: We sought consensus using a three-round Delphi method, involving the French rare lung disease network. Statements were submitted to a panel of 38 experts (including nurses, patients, physiotherapists, specialised

The Role of Associated Treatment Factors and Mental Health on Transition From Pediatric to Adult Care for Adolescent/Young Adults With Sickle Cell Disease.

BACKGROUND: Adolescents and young adults (AYA) living with sickle cell disease (SCD) transitioning to adult hematology care are at risk of dropping out of care and relying on emergency departments (ED) for acute crisis management. This can result in decreased continuity of care, an increase in serious health complications, and a greater mortality risk. This study aimed to investigate how transitioning to adult care is associated with SCD treatment outcomes, and test whether this association is moderated by comorbid mental health concerns (i.e., symptoms of anxiety and depression). PROCEDURE:

Usefulness of the Palliative Prognostic Index in Predicting Prognosis when Considering the Transition from Hospital to Home Care in Patients with Terminal Stage Cancer.

BACKGROUND: No accurate prognostic tool is available for patients with cancer who spend their final days at home. In this study, we examined whether performance status (PS) and the palliative prognostic index (PPI), a well-known prognostic tool in palliative care units, could be used to predict prognosis in the home care setting at the time of intervention by home physicians. SUBJECTS AND METHODS: Using medical records, we conducted a retrospective analysis of 132 patients who were referred to the Home Clinic Naginoki for home care for terminal stages of carcinoma in situ.
