Pediatric Lower Extremity Lawn Mower Injuries and Reconstruction: Retrospective 10-Year Review at a Level 1 Trauma Center.

INTRODUCTION: From 2004 to 2013, there were 9341 lawn mower injuries in children under 20 years old. The incidence of lawn mower injuries in children has not decreased since 1990 despite implementation of various different prevention strategies. In this report, the authors review the results of pediatric lawn mower-related lower-extremity injuries treated at a tertiary care referral center as well as review the overall literature. METHODS: A retrospective review was performed at a level 1 trauma center over a 10-year period (2005-2015). Patients younger than 18 years who presented to the emer

Substance exposure and helmet use in all-terrain vehicle accidents: Nine years of experience at a level 1 trauma center.

INTRODUCTION: The surging popularity of all-terrain vehicles (ATV) in the United States has caused an "epidemic of injuries and mortality." The U.S. Consumer Product Safety Commission reported 99,600 injuries and 426 fatalities from ATV accidents in 2013. The aim of this study was to examine the relationship between helmet use and positive toxicology screenings on outcomes in ATV accident victims. METHODS: This is a retrospective study of patients admitted to a Level 1 Trauma Center in southwestern West Virginia following an ATV accident between 2005 and 2013. Data were obtained from the inst

CT incidence of Morel-Lavallee lesions in patients with pelvic fractures: a 4-year experience at a level 1 trauma center.

The aim of this study is to determine the incidence and location of Morel-Lavallee lesions (MLLs) on pelvic CTs performed in evaluation of pelvic fractures and determine if correlation exists between MLLs and mechanism of injury or pelvic ring injury pattern. A retrospective review was performed of pelvic CTs on 1493 consecutive patients presenting with pelvic fractures at our level 1 trauma center. MLLs occurred in 182 of 1493 patients presenting with pelvic fractures. Statistical significance in MLL incidence was found across mechanism of injuries with MLLs being seen most frequently in MCC/

Popliteal artery injuries in an urban trauma center with a rural catchment area: do delays in definitive treatment affect amputation?

Extended length of time from injury to definitive vascular repair is considered to be a predictor of amputation in patients with popliteal artery injuries. In an urban trauma center with a rural catchment area, logistical issues frequently result in treatment delays, which may affect limb salvage after vascular trauma. We examined how known risk factors for amputation after popliteal trauma are affected in a more rural environment, where patients often experience delays in definitive surgical treatment. All adult patients admitted to the Level I trauma center, the University of Mississippi Med

Use of portable CT in the R Adams Cowley Shock Trauma Center. Experiences in the admitting area, ICU, and operating room.

The author's experience with portable CT has been positive. Nurses and clinical physicians have been pleased with this new imaging capability also, and have written testimonial letters endorsing its value. Recently, the STC extended availability of the mobile CT to the University of Maryland Hospital, an adjoining 600-bed center with numerous medical and surgical patients in ICU. They are particularly interested in assessing use of portable CT in the neonatal ICU, where patient transport outside the unit is particularly difficult and dangerous. Technical improvements for portable CT are in dev

Alcohol-positive polytrauma patients: injury patterns, ICU resource use and mortality at a German Level I trauma center.

BACKGROUND: In most countries, alcohol consumption is so culturally ingrained that even small amounts are permitted while driving. Even at a blood alcohol concentration of 0.5 g/L, ethanol impairs mental and cognitive performance. Studies from the US and Europe estimate that elevated blood alcohol levels may be detected in 10%-40% of all emergency department patients. Against this background, we investigated the impact of alcohol consumption on injury severity, injury patterns, as well as primary and secondary outcomes in polytrauma patients. MATERIALS AND METHODS: This retrospective, single-

Protocolized Rib Fracture Triage Reduces ICU Admissions Without Compromising Outcomes in a Rural Level 1 Trauma Center.

The objective of this study was to determine whether protocolized triage using the Rib Injury Guidelines (RIG) can safely reduce intensive care unit (ICU) admissions and improve institutional resource utilization in a rural, Appalachian, level 1 trauma center. This was a single-center retrospective cohort study that included a total of 1148 adult patients who presented to the institution with radiographically confirmed rib fracture(s) over the course of 2 years (557 pre-RIG, 591 post-RIG). Prior to RIG implementation, triage decisions were made at the discretion of the attending provider using
