Transmandibular approach for excision of maxillary sinus tumors extending to pterygopalatine and infratemporal fossae.

En bloc resection of maxillary sinus tumors that extend through the posterior wall necessitates resection of maxilla along with pterygopalat … ameters. All the patients had negative margins at the infratemporal fossa region. Varying degree of trismus was present in all patients. The cosmetic outcome of the procedure was excellent. Other anterior and transcranial approaches could be combined with this procedure when indicated. We conclude that transmandibular approach is an effective technique fo …

Severe temporomandibular dysfunction and joint destruction after intra-articular injection of triamcinolone.

BACKGROUND: Steroid injections into joints are frequently used to control symptomatic pain. Risks associated with intra-articular steroid injections are not well documented. METHODS: We report the case of a 29-year-old woman who was referred to a dental surgeon because of a suspected relationship between persisting chronic back pain and an arthrosis of the temporomandibular joint (TMJ). RESULTS: The dental surgeon diagnosed capsulitis of the right TMJ and injected 40 mg triamcinolone into the joint. Within 4 months the patient developed progressive pain and trismus of the right TMJ and the i…

DESIGN: Retro … LUSIONS: Children younger than 5 years who present with poor oral intake, high fever, drooling, and trismus should be suspected of having a peritonsillar abscess. A computed tomographic scan of the neck is usually required to confirm a suspected diagnosis. Prompt diagnosis and treatment will lead to a considerable decrease in morbidity. Immediate tonsille …

An audit of CO2 laser surgery in the mouth.

The use of the carbon dioxide laser to perform surgical procedures in the oral cavity has been described the last few years. This study invo … in treating oral soft-tissue pathology presented advantages over conventional techniques. Although trismus, numbness and early signs of infection were observed after surgery, local discomfort and pain were the most common complaints after laser surgery. The carbon dioxide laser does not offer any enhanced cure-rate for oral pathology, but rather it is a …

Effect of lingual-based flap design on postoperative pain of impacted mandibular third molar surgery: Split-mouth randomized clinical trial.

BACKGROUND: The extraction of third molars is one of the most common procedures in oral and maxillofacial surgery clinic. Surgical extraction involves the manipulation of both soft and hard tissues, so the patient usually experiences pain, swelling, and trismus in the immediate post-operative period. Several studies have been conducted using different types of surgical flaps to provide access with the least possible damage of soft tissues. Designing and implementing an optimum flap, which provides easier technique, better visibility, minimal post-operative complications, and best healing, is a…

Bi-paddled radial forearm free flap in the reconstruction of bilateral buccal mucosal defects-a case series.

Oral submucous fibrosis is a commonly encountered clinical problem in the Indian subcontinent. Treatment with surgery and reconstruction with appropriate flap, followed by vigorous physiotherapy is the ideal management. We report three advanced cases that were successfully managed and rehabilitated with bi-paddled radial forearm free flaps. Mean preoperative mouth opening was 3 mm, and mean postoperative mouth opening was found to be 27 mm after 18 months of follow-up. Thus, instead of two flaps, use of a single-donor-site, bi-paddled radial forearm free flap, is recommended for the reconstruc

A Case of a Large Peritonsillar Abscess.

A patient with a peritonsillar abscess (PTA) may initially appear clinically stable, but certain risk factors can predispose to a sudden airway obstruction. Those risk factors include being a young-to-middle-aged male, obesity (defined as a BMI ≥30.0 kg/m²), or diabetes. An obese 52-year-old diabetic male presented to the emergency department with throat pain, trismus, and a muffled voice. CT imaging of the neck without contrast showed a large abscess extending into the parapharyngeal space and compressing the airway anteriorly. Although he initially had normal oxygenation, he suddenly lost co…

The use of intraoral devices in reducing oral and dental side effects in head and neck cancer patients undergoing radiotherapy - a systematic review.

PURPOSE: To evaluate whether intraoral devices reduce the adverse oral and dental effects of radiotherapy in head and neck cancer patients. … e remaining four studies reported that intraoral devices reduced the risk of xerostomia, mucositis, trismus, dysphagia, and dental caries over 2 to 6 months. CONCLUSION: There are limited data to support the use of intraoral devices in head and neck cancer patients undergoing radiotherapy. Photobiomodulation in the treatment of xerostomia associated with hyposalivation in a pediatric patient with systemic scleroderma.

Scleroderma is a rare autoimmune disease characterized by excessive collagen production. The oral manifestations of the patient with scleroderma can include microstomia, xerostomia, and changes in the resorption teeth. We report the case of a 7-year-old female patient diagnosed with systemic scleroderma where photobiomodulation therapy was used to treat xerostomia associated with hyposalivation. She attended a pediatric clinic and presented with dry and rigid facial skin, trismus, xerostomia, malocclusion, and difficulty swallowing. Stimulated salivary flow was assessed before, during, and aft…

Areca nut in pathogenesis of oral submucous fibrosis: revisited.

BACKGROUND: Oral submucous fibrosis is a disease due to a chronic, insidious change in fibro-elasticity, characterized by burning sensation in the oral cavity, blanching, and stiffening of the oral mucosa and oropharynx leading to trismus and inability to open the mouth. The symptoms and signs depend on the progression of the lesions and number of affected sites. It is predominantly seen in Indians and other Asians. Once the disease has developed, there is neither regression nor any effective treatment. It is considered as a pre-malignant stage of oral cancer, and the reported risk of malignan…

Recurrent malignant otitis externa: management and outcome.

Necrotizing (malignant) external otitis (NEO) is an infection involving the temporal and the adjacent bones. It is a rare type of external otitis that occurs primarily in immunocompromised persons. The present study aimed to evaluate its management and outcome in recurrent cases. Ten patients attending the ENT Department, Alexandria University were included. They had recurrence of otitis externa over a period of 6 months after-cure, as well as severe night otalgia and high ESR level. Peri-auricular soft tissue swelling, cranial nerve paralysis and trismus were the main persistent or developing…

[Generalized tetanus complicating a traditional medicine applied for snakebite].

In many of Africa's rural areas, snakebite victims often resort to traditional healers for first line treatment. This may be source of infectious complications. We report a case of generalized tetanus which occured after 15 days in a 13-years old boy who had applied a traditional, plant-based concoction on a snake bite. He presented with trismus, generalized contractures and fever extended musculo-aponevrotic necrosis of the right upper limb, without loss of consciousness. The only accompanying biological sign was an increased leukocyte count (11,200/mm3) with a predominance of neutrophils (84…

The value of routine antibiotic prophylaxis in mandibular third molar surgery: acute-phase protein levels as indicators of infection.

Postoperative infections in the oral region are usually caused by anaerobic bacteria. While some authors claim that routine antibiotic prophylaxis is necessary after third molar surgery, others do not recommend this practice. The major subject of controversy is what constitutes postoperative infection. Previous studies that have examined the benefit of routine antibiotic prophylaxis have used several clinical symptoms (pain, swelling, and trismus) as indicators of infection; however, these clinical symptoms may be vague and unreliable, and cannot be evaluated scientifically. As a result, their…
