BACKGROUND: Fulminant disseminated tuberculosis (TB) presenting as Landouzy sepsis is rare but carries high mortality. When complicated by acute respiratory distress syndrome (ARDS) and multiorgan failure, evidence for optimal management, including extracorporeal membrane oxygenation (ECMO) and modified antituberculous regimens, remains limited.
CASE PRESENTATION: We describe a 23-year-old man with miliary tuberculosis who developed Landouzy sepsis, ARDS, hepatic dysfunction, and refractory hypoxemia requiring veno-venous (VV) and subsequent veno-veno-venous (VVV) ECMO for six weeks. Standard

Liver injury is a globally serious problem that may be observed post-chemotherapeutic administration in chronic crises such as tuberculosis (TB). Rifamycin (RIF), particularly, is an indispensable treatment regimen for TB with a significant negative hepatic impact. Therefore, this research aims to assess the restorative role of melatonin (MEL) against rifampicin (RIF)-associated hepatic damage in rats. Moreover, to investigate the ultimate mechanism of the antioxidant potential of MEL with multimodal assessment and in silico molecular conformation. Adult male Wistar albino rats were weighed an

The German guideline for the treatment of psoriasis vulgaris was updated using GRADE methodology. The guideline is based on a systematic literature review completed on December 1, 2016, and on a formal consensus and approval process. The second part of this short version of the guideline covers the following special patient populations and treatment situations: tuberculosis screening before and during psoriasis treatment, choice of psoriasis treatment for individuals wishing to have children, as well as during pregnancy and breast-feeding, and patients with joint involvement and vaccinations.

Patients with rheumatoid arthritis (RA) are at increased risk of infection compared to healthy individuals. The increased risk may be associated with the underlying disease, comorbidities and immunosuppressive therapy required to control RA activity. In several recent studies, influenza, pneumococcal and hepatitis B vaccines administered to RA patients were reported to be safe and serologically effective. However, several lines of evidence suggest a possible aberrant immunologic response following vaccination due to the compromised immunity of these patients. Therefore, vaccination of RA patie

Hepatotoxicity associated with any antibiotic is rare. With the wide-spread use of antimicrobial agents, however, hepatic injury is not an infrequent occurrence. Penicillins remain a widely used class of antimicrobials with a well defined record of low hepatotoxicity. The combination of clavulanate with amoxicillin may be associated with the greatest risk for liver injury from any antimicrobial agent. Significant hepatotoxicity also may occur with sulfamethoxazole/ trimethoprim and combination regimens used to treat tuberculosis. An autoimmune-like hepatitis may result from minocycline or nitr

INTRODUCTION: The endoscopic ultrasonography (EUS) is an endoscopic technique of proven clinical validity today, having a significant impact on the diagnosis and evaluation of several diseases with a low complication rate. The EUS-fine-needle aspiration (FNA) allows the evaluation of subepithelial lesions, extra-luminal lymph nodes or the gastrointestinal tract that are difficult to access by other methods with safe and high diagnostic accuracy.
AIMS AND METHODS: In the case reported, the EUS-FNA was useful for the differential diagnosis with residual biliary microlithiasis and the diagnosis

Isoniazid (INH), one of the most important drugs used in antituberculosis (anti-TB) treatment, is also the major drug involved in hepatotoxicity. Differences in INH-induced toxicity have been attributed to genetic variability at several loci, such as NAT2, CYP2E1, GSTM1 and GSTT1, that code for drug-metabolising enzymes. Our goal was to examine the polymorphisms in these enzymes as susceptibility factors to anti-TB drug-induced hepatitis in Brazilian individuals. In a case-control design, 167 unrelated active tuberculosis patients from the University Hospital of the Federal University of Rio d

Malaria and tuberculosis (TB) are two major global diseases mostly affecting the developing countries. Their treatment is often complex because of the drugs used, multidrug resistance, drug interactions and logistic problems such as drug availability and access. Patients are treated for TB for a minimum of 6 months and may concomitantly develop and be treated for malaria, especially during the rainy season. Rifampicin, a standard component of combination regimens for treating TB, is a potent inducer of hepatic cytochrome and other metabolic enzymes and is able to influence the pharmacokinetics

Prophylactic isoniazid therapy is currently recommended for adolescents with positive tuberculin reactions because of the risk of activation of tuberculosis in this age group. Since this drug has been associated with hepatic dysfunction in adults, the present study was undertaken to evaluate its effect on liver function in teen-agers. Accordingly, liver function tests were performed on 178 adolescents prior to and following administration of INH. Abnormalities in SGPT values were encountered in 10% of these patients and occurred in the main within the first 10 weeks of testing, suggesting INH

Two cases of prolonged fever are reported. In the first case a tuberculous abscess of the liver was considered after an ultrasonic examination of the periphery of an hepatic calcification discovered many years ago; a right hepatectomy confirmed the diagnosis. The second patient was treated twenty years ago for a tuberculous exsudative peritonitis. The pelvic examination showed a myoma. A computed body tomogram disclosed a picture of pyometritis. This was confirmed by laparotomy showing tuberculous salpingitis and pyometritis by praevia myoma.

A 5-year-old child with tuberculous meningoencephalitis developed severe drug-induced hepatitis after conventional antitubercular therapy. Consequently, standard treatment was discontinued, and second-line drugs were administered in combination with high-dose corticosteroids. However, the disease continued to progress significantly. The introduction of infliximab resulted in substantial clinical and radiologic improvement, highlighting its potential role in managing refractory cases of central nervous system tuberculosis.

Minnesota accepts more refugees per capita than any other state. For that reason, primary care physicians throughout the state are likely to encounter refugees in their practices. This article describes 5 infectious diseases that are common among refugees and what physicians need to know about them in order to effectively screen and treat patients who are newcomers to this country. The diseases discussed are strongyloides, schistosomiasis, malaria, hepatitis B, and tuberculosis.

All nations that are part of the European Union share the same aim for the control and eradication of vaccine-preventable diseases. However, there are differences in child immunization strategies and schedules between nations, depending upon health care systems, immunization habits and epidemiology of infectious diseases. All nations immunize children against diphtheria, tetanus, poliomyelitis, measles, rubella and mumps. Immunization against pertussis, Haemophilus influenzae, hepatitis B and tuberculosis are not systematically applied.

Antibodies (Abs) hydrolyzing proteins, DNA, and RNA are detected in the blood of patients with various autoimmune diseases. In the present work, homogeneous preparations of IgG Abs from the blood of the healthy donors as well as patients with A, B, C, and delta types of viral hepatitis, influenza, pneumonia, tuberculosis, tonsillitis, duodenal ulcer, and some types of cancer were purified. For the first time, the fraction of IgG and its Fab fragments of patients with viral hepatitis were shown to have high DNA- and RNA-hydrolyzing activity. In case of Abs from the healthy donors and patients w

Between April 1976 and March 1987, in an Internal Medicine department some 300 unguided percutaneous liver biopsies were performed, using the Tru-Cut excision needle. The procedure contributed to the diagnosis in 76.2% of the cases. In alcoholism-related pathology with its specific lesions, liver biopsy is particularly useful in diagnosing incipient fatty degeneration and hepatitis and helps in the prognosis of cirrhosis. In chronic hepatitis, it asserts the diagnosis and provides aetiological and prognostic data. The finding of granulomas at histology sometimes clinches a hitherto undecided d

In 2002 nearly 36 000 cases of notifiable infectious diseases were reported in Bavaria representing a 10 % increase compared to 2001 (33 000 cases). As in 2001, around 75 % of reported cases were gastrointestinal infections. Every third infection was due to salmonella. As compared to last year, the incidence of Norwalk-like virus infections increased fivefold. These infections occurred mostly as outbreaks in nursing homes, hospitals or other institutions, affecting as many as 200 persons. Other frequently reported infections in Bavaria are tuberculosis and hepatitis. The relatively high incide
