Anterior and posterior fusion for children with tuberculosis of the spine.

The authors reviewed the use of chemotherapy and anterior/posterior spinal fusion without instrumentation to treat children with extensive spinal tuberculosis and kyphosis. Six children underwent anterior and posterior spinal fusion. All of the patients were followed until after maturity, except for 1 child who died of pulmonary tuberculosis 4 months after surgery and thus was excluded from the study. Preoperative kyphotic deformity averaged 100 degrees (range, 75 degrees-130 degrees). The average age at the time of surgery was 7.5 years (range, 4.7-10 years). Spinal involvement extended from

Pott's Disease Resulting in Complete Cervical Vertebral Destruction.

Skeletal tuberculosis, otherwise known as Pott's disease, has been recognized for centuries. Although typically diagnosed in citizens from countries with endemic tuberculosis, long-term workers in these regions, such as military deployees, can also acquire the disease. We present a case report of a military veteran presenting with neck pain and initially diagnosed with cervical disc disease. The patient's pain progressed to the point of developing paresthesias in his bilateral upper extremities. Eventually, cervical spine radiographs were obtained that revealed complete cervical vertebral body

Tuberculous spondylodiscitis with ureteral involvement: a rare case report.

BACKGROUND: Tuberculosis spondylitis, also known as Pott's disease, is a form of osteomyelitis that primarily affects the vertebral bodies and can lead to severe complications such as paravertebral abscesses, kyphosis, and degenerative spinal changes. Although it typically involves the skeletal system, contiguous spread to adjacent organs, such as the genitourinary tract, is rare. METHODS: We report the case of a 64-year-old male with chronic back pain who underwent a renal protocol abdominopelvic CT scan following ultrasound findings of right kidney stasis. RESULTS: The CT revealed obstruct

Case Report: Diagnosis of vertebral alveolar echinococcosis upon next-generation sequencing in a suspected tuberculosis.

INTRODUCTION: Alveolar echinococcosis (AE), caused by larval stages of Echinococcus multilocularis, is a rare zoonotic disease that mainly involves the liver. The diagnosis of extrahepatic AE is usually difficult. Here, we describe a rare case of vertebral alveolar echinococcosis with a suspected history of spinal tuberculosis, diagnosed by metagenomic next-generation sequencing (mNGS). CASE PRESENTATION: A 44-year-old woman presented with repetitive neck and back pain, with a surgical history of suspected spinal tuberculosis. Magnetic resonance imaging (MRI) showed cystic masses in the crani

[Sacral bone tuberculosis in a 6 year-old child].

BACKGROUND: Spinal tuberculosis is now rare in developed countries; its appearance in sacral vertebrae is exceptional. CASE REPORT: A 6 year-old boy was admitted because he had suffered from lumbar pain and rigidity for 24 hours, limiting anterior flexion. X-rays of the spine was normal, but a CT-scan showed lytic lesions of the sacrum. Bone scintigraphy showed no inflammatory lesions. Ewing sarcoma was diagnosed: surgical biopsy showed a cold abscess containing many bacilli that where resistant to decoloration with acid-alcohol. They were cultured for 12 days and identified as Mycobacterium

Clinical outcomes and surgical strategy for spine tuberculosis: a systematic review and meta-analysis.

PURPOSE: Spinal tuberculosis (TB) is a slow-developing disease that often causes cord compression, spinal instability, and deformity. Surgery is generally required in cases of refractory disease, severe kyphosis, neurological deficits, or lack of improvement. However, there is a lack of comprehensive evidence in comparing the efficacy of various surgical approaches. The study aims to provide a clearer understanding of the relative effectiveness of the available surgical modalities in the management of spinal TB. METHODS: This review adhered to the PRISMA statement with searching conducted unt

Primary spinal epidural diffuse large B-cell lymphoma with paraplegia as the first manifestation: a case report.

Primary spinal epidural diffuse large B-cell lymphoma (DLBCL) is rare and often easily misdiagnose. Here, we presented a case of primary spinal epidural DLBCL with paraplegia as the first manifestation. He was misdiagnosed as thoracic disc herniation and thoracic tuberculosis successively. DLBCL was confirmed after surgery ultimately. After 6 cycles of chemotherapy with R-CHOPE, the patient went back to normal life without relapse during 2-year follow-up. Primary spinal epidural lymphoma should be suspected in patients with spinal cord compression with imaging showing isointense, homogeneous e

[Experimental study of GeneXpert(®) system in the diagnosis of extra-pulmonary tuberculosis].

OBJECTIVE: To explore the application value of GeneXpert MTB/RIF for detection of extra-pulmonary tuberculosis and resistance to rifampin. METHODS: A total of 150 samples were collected, including 33 needle aspirates from lymphoid tuberculosis, 23 needle aspirates from spinal tuberculosis, 49 from tuberculous pleural effusions, 24 from cerebrospinal fluid of tuberculous cephalomeningitis, and 21 urinary sediment samples from renal tuberculosis. Smear microscopy, mycobacterium tuberculosis culture and the MTB/RIF method were used to examine these samples and their positive rates were compared.

Ventral Spinal Cord Displacement After Single-Level Anterior Cervical Corpectomy: A Case Report.

Spinal cord herniation is a rare condition characterized by protrusion of the spinal cord through a dural defect, typically associated with progressive neurological deficits. However, postoperative alterations at the ventral dural-cord interface may present as cord displacement without a demonstrable dural breach. We report the case of a 44-year-old woman who presented with progressive gait imbalance, neck pain, and right upper limb radiculopathy of four weeks' duration. Imaging suggested compressive cervical pathology, and the patient underwent anterior cervical corpectomy and stabilization.

Intradural Staphylococcus aureus Abscess of the Cauda Equina in an Otherwise Healthy Patient.

Abscesses involving the spine are usually located in the epidural space. In rare circumstances, intradural spinal abscesses can occur, typically in the setting of tuberculosis or other predisposing systemic conditions. In this illustrated case report, we discuss the imaging and intraoperative findings of an otherwise healthy patient with an intradural abscess of the cauda equina caused by Staphylococcus aureus. Although rare, intradural spinal abscesses can occur in the absence of typical "red flags" for infection, and a bacterial abscess should be considered in the differential diagnosis of i

Atypical Cerebral Manifestations of Disseminated Mycobacterium tuberculosis.

BACKGROUND: We investigated the patterns of cerebral manifestations in patients with underlying pulmonary or extrapulmonary tuberculosis or disseminated tuberculosis. MATERIALS AND METHODS: From January 2010 to September 2016, brain magnetic resonance imaging (MRI) scans were obtained to evaluate cerebral manifestations in patients with underlying pulmonary or extrapulmonary tuberculosis. We also included patients with drug-resistant tuberculosis or disseminated tuberculosis. MRI findings of tuberculous meningitis and tuberculoma were classified as typical; other MRI findings were classified

FUNCTIONAL AND RADIOLOGICAL OUTCOME FOLLOWING EXTENDED POSTERIOR CIRCUMFERENTIAL DECOMPRESSION IN THE TUBERCULOSIS OF DORSAL SPINE.

Spinal Tuberculosis ranks as one of the most common extrapulmonary varieties of tuberculosis. The study outlines the Extended Posterior Circumferential Decompression (EPCD) procedure for managing tuberculous spondylitis, a prevalent extrapulmonary form of tuberculosis. EPCD involves 360-degree dural decompression, anterior column debridement, and reconstruction following posterior instrumentation. This technique addresses both the infection and associated complications, particularly beneficial in cases with or without paraplegia. EPCD aims to improve outcomes by effectively tackling the pathol

Multiple tuberculomas invading the central nervous system as a paradoxical reaction in a kidney transplantation recipient.

A paradoxical reaction during anti-tuberculosis (anti-TB) therapy is commonly reported in patients with human immunodeficiency virus (HIV). However, a similar reaction to anti-TB therapy can also occur in patients without HIV, especially in patients who have undergone solid organ transplantation. A 65-year-old woman who underwent kidney transplantation six months prior presented to our emergency room with progressive paraparesis. She had been diagnosed with drug-susceptible miliary TB and had undergone two weeks of treatment with anti-TB medication. Magnetic resonance imaging showed a spinal i

Multiloculated thoracoabdominal tuberculosis: A radiological presentation of disseminated tuberculosis.

Tuberculosis is more frequently found among high-risk populations in the United States. It has a challenging diagnosis since it can present with diverse organ involvement that may delay the diagnosis. This is especially true regarding hepatic tuberculosis, with prevalence varying in each study but highly suggestive of underdiagnosis. An 18-year-old male with high-risk exposure to multidrug-resistant tuberculosis presented with fever, night sweats, weight loss, and cough. Imaging revealed a right lung cavitary mass with bilateral pulmonary nodules, right pleural nodular thickening traversing di

Intramedullary Spinal Tuberculoma: An Uncommon Presentation of a Common Disorder.

Intramedullary tuberculoma (IMT) is rare and usually indistinguishable from spinal cord tumors. Thus, the diagnosis of an IMT is challenging. Our case deals with an unusual presentation of a 55-year-old Asian man who had presented with lower limb weakness which was found to be caused by the dissemination of tuberculosis (TB) resulting in an IMT, a rare complication of tuberculosis. The patient also had a concurrent incidental hepatitis B infection. The treatment of IMT is anti-tuberculous medication. This case highlights the significance of the prompt diagnosis of an IMT, urgent intervention p

Atypical tuberculosis of the spine.

Spinal tuberculosis characteristically involves the paradiscal area of vertebral bodies with a narrowing of the disc space. In this study, we reported four atypical forms of Pott's disease, including one hundred and eighty-four patients treated between 1985 and 1998. Two cases presented with noncontiguous multilevel involvement, where one case had transverse process involvement alone and the other had involvement of the neural arch. Atypical tuberculosis of the spine was found in 2.1% of the patients.

Mycobacterium tuberculosis chaperonin 10 stimulates bone resorption: a potential contributory factor in Pott's disease.

Pott's disease (spinal tuberculosis), a condition characterized by massive resorption of the spinal vertebrae, is one of the most striking pathologies resulting from local infection with Mycobacterium tuberculosis (Mt; Boachie-Adjei, O., and R.G. Squillante. 1996. Orthop. Clin. North Am. 27:95-103). The pathogenesis of Pott's disease is not established. Here we report for the first time that a protein, identified by a monoclonal antibody to be the Mt heat shock protein (Baird, P.N., L.M. Hall, and A.R.M. Coates. 1989. J. Gen. Microbiol. 135:931-939) chaperonin (cpn) 10, is responsible for the
