Unmasking an Intracardiac Shunt in a Case of Persistent Unexplained Hypoxia: A Case Report.

Background and Clinical Significance: Pulmonary hypertension (PH) is characterized by an increase in mean pulmonary arterial pressure and pulmonary vascular resistance. It is frequently encountered in patients with significant intracardiac shunts, often necessitating the implementation of a closure device or surgical correction. Nevertheless, the occurrence of a concomitant atrial septal defect (ASD) with a right-to-left shunt inducing left ventricular dysfunction is a rare phenomenon. Case Presentation: A 69-year-old female patient with a history of heart failure (with preserved ejection frac

Practical valvular issues in patients requiring ventricular assist devices.

PURPOSE OF REVIEW: As ventricular assist device (VAD) therapy in patients with advanced heart failure continues to grow, experience with concomitant valvular diseases present either before or after VAD implantation continues to accrue. In this review, we discuss recent data and current practice as it pertains to the subject of concomitant valvular disease in patients requiring VADs. RECENT FINDINGS: Persistent aortic valve closure has been identified as a potential contributor to aortic valve 'disuse atrophy' resulting in valve degeneration. Dilation of the aortic root may be predictive of fu

Transcatheter atrial septal defect closure in an infant (body weight 6.4 kg) using the GORE CARDIOFORM septal occluder (GCSO).

INTRODUCTION: Transcatheter closure has become the treatment of choice for secundum atrial septal defects (ASD II), but particularly in small children, there is concern regarding procedure-related complications. CASE DESCRIPTION: We report on a 10-month-old infant, body weight of 6.4 kg, with a large ASD who was referred for failure to thrive and dyspnea on exertion. Echocardiography showed two neighboring ASDs centrally located within an atrial septum with a length of 27 mm resulting in significant left-to-right shunting. During cardiac catheterization, hemodynamic significance of the defect

Association Of Bleeding Avoidance Strategies with age-related bleeding and In-hospital mortality in patients undergoing percutaneous coronary Interventions.

BACKGROUND: The association of bleeding avoidance strategy (BAS) (consisting of a combination of radial access, bivalirudin [rather than heparin +/- glycoprotein GPIIb/IIIa antagonists], and/or vascular closure devices after femoral access) with bleeding and in-hospital outcomes has not been evaluated among elderly patients undergoing percutaneous coronary interventions (PCI). METHODS: We studied BAS use, bleeding and in-hospital mortality among 121,635 patients categorized by age (<50, 50-59, 60-69, 70-79, and ≥80years) undergoing PCI from the BMC2 registry (1/2010-12/2013). RESULTS: The us…

Remote periosteal distraction promotes ischemic diabetic foot wound repair associated with VEGF/VEGFR2-related angiogenic responses.

Bone transport-induced remote microinjury may promote diabetic foot ulcer healing but requires osteotomy and may cause fracture or osteomyelitis. This study evaluated whether osteotomy-free remote periosteal distraction (RPD) could promote repair in a rabbit ischemic diabetic foot wound model. Seventy-two diabetic male rabbits were randomized into RPD, Sham, and Control groups, and ischemic foot wounds were created. In the RPD group, a custom-made plate was implanted beneath the tibial periosteum and connected to an external distractor. The periosteum was distracted outward for 7 days and retu

Reduction of vascular complication rates from femoral artery access in contemporary women undergoing cardiac catheterization.

BACKGROUND: Femoral arterial access (FAA) during diagnostic coronary angiography (CAG) and percutaneous coronary interventions (PCI) are associated with several vascular complications (VC). VC rate in our experience a decade ago was 3.02% and higher in women (4.7% in women, and 1.67% in men, p < 0.0006), with an OR of 2.81 (95% CI: 1.51-5.22). METHODS: Patients who underwent CAG and PCI utilizing FAA (n = 2617) were separated into Period 1 (2005 to 2008; 1970 patients; Male 1045; Female 925) and Period 2 (2016-2017; 647 patients; Male 357; Female 290). Efficacy and safety of intravascular lithotripsy for the treatment of peripheral arterial disease: An individual patient-level pooled data analysis.

BACKGROUND: Peripheral arterial disease (PAD) is one of the most common manifestations of atherosclerotic disease worldwide. Peripheral arterial calcification reduces acute success and long-term patency of endovascular therapy for PAD. Several calcium modification devices are available for use in peripheral interventions. Outcomes after peripheral intravascular lithotripsy (IVL), a novel approach using pulsatile sonic waves to treat luminal and medial calcium in patients with PAD, have not been extensively characterized. Therefore, we sought to perform an individual patient-level data (IPD) po

Endovascular treatment of complications of femoral arterial access.

Endovascular repair of femoral arterial access complications is nowadays the treatment of choice in a group of patients who cannot tolerate vascular reconstruction and bleeding due to advanced cardiovascular disease. Endovascular procedures can be performed under local anesthesia, are well tolerated by the patient, and are associated with a short hospitalization time. Ninitinol stent technology allows for safe stent and stent-graft extension at the common femoral artery (CFA) level, due to increased resistance to external compression and bending stress. Active pelvic bleeding can be insidious,

Transcatheter aortic valve implantation-related vascular access complications: Are balloon-expandable stent grafts a good option?

BACKGROUND: Among the most challenging complications of transfemoral transcatheter aortic valve implantation (TF-TAVI) are vascular access complications (VACs), with access-site bleeding frequently impacting outcomes despite preclosing techniques. Endovascular bailout strategies have become increasingly common despite historical concerns about stent placement in flexing zones. AIMS: To evaluate the efficacy and safety of balloon-expandable stent graft (BESG) implantation for VACs in patients undergoing TF-TAVI with a preclosure device. METHODS: This retrospective single-centre study included

Predictors of percutaneous access-related complications in aortic endovascular procedures: 'real-world' insights and a comparison to open access.

BACKGROUND: Percutaneous endovascular aneurysm repair (PEVAR) is becoming increasingly popular due to fewer access-related complications, shorter procedural times and length of stay (LOS). Our aim was to explore factors associated with access-related complications and their impact on procedural time and LOS. METHODS: We retrospectively analyzed consecutive aorto-iliac endovascular procedures in a tertiary hub comprising 2 institutions and 18 consultant vascular surgeons and interventional radiologists between 2016-2017. Access-related complications were defined as: bleeding requiring cutdown

Percutaneous decannulation of extracorporeal membrane oxygenation using MANTA device: A real-world single-center experience.

BACKGROUND: The MANTA vascular closure device (VCD) represents a novel approach to achieving hemostasis after large-bore femoral access procedures. Numerous clinical studies have evaluated the efficacy of the MANTA device across a range of patient populations undergoing different procedures. However, there is still a paucity of data available concerning the use of MANTA devices in aiding the decannulation of venoarterial extracorporeal membrane oxygenation (VA-ECMO). AIM: To present our single-center experience of utilizing the MANTA VCD in patients undergoing this procedure. METHODS: This s

Mechanical interference between a closure device and the rigid stent of mitral bioprosthesis: a mechanism for closure device late dislodgement.

: We report here the mechanical interference between mitral bioprostheses and devices currently adopted for percutaneous paravalvular leak closure as a possible cause for late dislodgement and embolization of successfully implanted and apparently stable closure devices. The images included in this study demonstrate that the topographical relationship of the target leak with the bioprosthetic valve posts may be a major obstacle for device stability after percutaneous paravalvular leak closure procedures.
