An incident of positional vertigo associated with osteotome technique for installation of multiple maxillary dental implants is reported. The symptoms resolved after 2 weeks with restricted physical activity and prohibition of lifting. There is a discussion of labyrinthine concussion and treatments. Suggestions for prevention are to use small sizes of osteotomes first and then progress to larger sizes and to avoid neck extension head position during osteotome use.

Certain patients with balance disorders report a 'visual vertigo' in which their symptoms are provoked or aggravated by specific visual contexts (e.g. supermarkets, driving or movement of objects). In order to determine the causes of visual vertigo (VV), we assessed symptoms, anxiety and the influence of disorienting visual stimuli in 21 such patients. In 17 out of 21 patients, a peripheral vestibular disorder was diagnosed. Sixteen bilateral labyrinthine-defective subjects (LDS) and 25 normal subjects served as controls. Questionnaire assessment showed that the levels of trait anxiety and chi…

Benign paroxysmal positional vertigo (BPPV) originating from the posterior semicircular canal (pSCC) is a common vestibular disorder that is easy to diagnose and usually easy to treat. The majority of patients with BPPV have no known inner ear disease; they have "primary" or "idiopathic" BPPV. However, a minority does have objective evidence of an inner ear disease on the same side as the BPPV and this group has "secondary" or "symptomatic" BPPV. Previous publications differ on the prevalence of secondary BPPV and about the types of inner ear diseases capable of causing it. In order to determi…

OBJECTIVE: To correlate patient response and recurrence rate after canalith repositioning to clinical presentation and cause of BPPV.
STUDY DESIGN: This was a retrospective chart review.
SETTING: This study was performed at a tertiary referral center.
PATIENTS: The study included patients with a diagnosis of benign paroxysmal positional vertigo based on history and the presence of a positive response to the Dix-Hallpike maneuver (affected ear down) who were treated at the University of Arkansas for Medical Sciences between January 1993 and February 1997 using canalith repositioning. There w…

Benign paroxysmal positional vertigo (BPPV) is one of the most common and treatable causes of vertigo. We examined BPPV types and the effectiveness of physical therapy in each type. BPPV is caused by a utricular statoconium that blocks the semicircular canal. Statoconia can block any of the semicircular canals, but they generally affects the posterior canal. Diagnosis is based on a typical history and characteristic eye movements elicited by the Dix-Hallpike test. Treatment involves a physical maneuver designed to mobilize the free calcium particles from the semicircular canal to the utricle. …

A retrospective study was conducted on 90 patients with episodic vertigo that could be related to migraine as the most probable pathomechanism. Since the majority of the patients did not fulfill the criteria of the International Headache Society (IHS) for basilar migraine, the diagnosis was substantiated by disease course, medical efficacy in treating (ergotamines) and preventing (metoprolol, flunarizine) attacks, ocular motor abnormalities in the symptom-free interval, and careful exclusion of the most relevant differential diagnoses, such as transient ischemic attacks, Menière's disease, and…

Between October 1974 and August 1997 in our Dizziness Clinic (n = 15,233), 2,523 patients were found to suffer from BPPV. All patients were assessed and followed up by the author. Patients (n = 337) having other ear or neurological diseases were excluded from this retrospective study. In 1644/2186 (75.21%) patients, the type of nystagmus was clinically identified in two opposite directions of gaze in the provocative head position. These patients were divided into two groups: i) idiopathic (n = 1,490) (no apparent cause); ii) post-traumatic (n = 154) (time of onset related to accident). It was

The position of the jugular bulb is extremely variable. A high jugular fossa with a diverticulum of the jugular bulb can alter the inner ear function with sensorineural hearing loss, vertigo and tinnitus. Nine cases of jugular bulb diverticulum with vertigo mimicking Menière's disease were operated on and followed up from 3 months to 4 years. Eight patients were treated surgically. The jugular bulb was approached through a mastoïdectomy and the diverticulum was compressed downwards using bone wax. The vertigo disappeared after surgery in all cases. These observations suggest that an abnormally…

An otoneurological expert system (ONE) was developed to help collect data and diagnose the work-up of vertigo of both central and peripheral diseases causing vertigo. Patient history and otoneurological and other examination results are used in the reasoning process. The history is interactively collected and is complemented with clinical examination results. The case history data can be either mandatory or supportive. Mandatory questions are used to confirm a diagnosis, and conflicting answers are used to reject an unlikely disease. Supportive questions support or suppress a diagnosis, but th…

The approach to drug treatment of vertigo is almost exclusively symptomatic. There are 3 major goals for drug treatment of vertigo. The first one is to eliminate the hallucination of motion. Drugs with vestibular 'suppressant' properties are used for this purpose. The major vestibular suppressants are anticholinergic and antihistamine drugs. The second goal is to reduce the accompanying neurovegetative and psychoaffective signs (nausea, vomiting, anxiety). Antidopaminergics are used for this purpose. The third goal is to enhance the process of 'vestibular compensation' to allow the brain to fi…

The steps of the examination procedure applied for a correct diagnosis of benign paroxysmal positional vertigo (BPPV) are reviewed. A precise diagnosis is important in view of treatment of this type of vertigo by rehabilitation therapy. Clinical experience supports the concept that the diagnosis has to be based not only on a typical history, but also on the presence of a reproducible vertigo and paroxysmal positioning nystagmus. In the procedure applied in the department, this nystagmus can be reproduced by the Dix-Hallpike maneuver under Frenzel's glasses, during electronystagmography (ENG) r…

The influence of vestibular dysfunction upon the vestibulospinal reflex (VSR) in two common peripheral syndromes was investigated by two types of posturographic examination: "static" posturography, recording and analyzing the postural sway in stance, and "kinetic" posturography, recording the stepping in place test. The influence of the dysfunction was examined outside the attacks, ie, between attacks and not during or immediately after an attack, in patients with Meniere's disease and in others with a sudden vestibular loss syndrome, "neuronitis." However, in both syndromes the influence of t

Differential diagnosis of the disease entities of vertigo are important in clinical investigations. Many concepts and procedures have been proposed for available diagnostic tests. Recent basic and clinical studies proved the efficacy and reliability of galvanic tests, including galvanic nystagmus test (GNT) and galvanic body sway test (GBST), emphasizing that the effects of galvanization are: 1) on the action potential in vestibular nerve and 2) on the resting discharge of the vestibular nerve. These suggest further that galvanic nystagmus reaction (GNT) is mainly a manifested reaction of the …

Auditory nerve action potential (AP), cochlear microphonics (CM) and summating potential (SP) were recorded from 6 patients with low-tone hearing loss without vertigo. All these cases showed high AP and -SP amplitude and satisfactory CM response. These findings resembled the electrocochleographic findings of type 1, which is an early stage of Ménière's disease. Hearing returned to normal range in half of the cases but remained hardly changed in the other half. The difference between these two groups could not be clarified electrocochleographically. On the other hand, low-tone hearing loss due …

Three hundred and twenty-one head injury patients investigated at the Workmen's Compensation Board Hospital and Rehabilitation Centre were studied. The patients were classified into two groups, minor and moderate according to the duration of post-traumatic amnesia. Post-traumatic vertigo was a significant symptom in 34 per cent and 50 per cent of the minor and moderate groups respectively. Based on the findings of full otoneurologic and vestibular examination, objective vestibular disorder was noted in 40 per cent and 65 per cent of the two vertiginous groups respectively. An approach to the i…

This paper describes a method of endolymphatic sac decompression and drainage. The method emphasizes widely exposing dura, avoiding skeletonization of the posterior semicircular canal and draining the sac via a T tube. The results of this procedure in 75 patients, including statistics in 46 ears are discussed. In patients with unilateral disease good results were obtained in control of vertigo in 94 percent of patients and significant improvement in cochlear function in over 30 percent.
