Feasibility and efficacy of video-assisted thoracoscopic surgery for the surgical stabilization of rib fractures: a single-center retrospective cohort study.

PURPOSE: Surgical fixation of traumatic multiple rib fractures is becoming more common; video-assisted thoracoscopic surgery (VATS) is reportedly useful in such cases. Therefore, we aimed to explore the feasibility and effectiveness of VATS for surgical stabilization of rib fractures (SSRF). METHODS: We conducted a single-center, medical record-based retrospective cohort study including 52 patients with traumatic multiple rib fractures who underwent SSRF with or without VATS. All patients were admitted to our hospital between January 2017 and March 2024. Patient characteristics and perioperat

Simultaneous Bilateral Video-Assisted Thoracoscopic Surgery in the Prone Position for Resection of a Large Middle Mediastinal Tumor: A Case Report.

Schwannoma is the most common type of neurogenic tumor in the thorax and is typically benign. This article describes the case of a 64-year-old asymptomatic woman with abnormal findings on a chest X-ray examination. Preoperative enhanced computed tomography showed the presence of a middle mediastinal tumor measuring 7.4 cm protruding into both thoracic cavities. The mass was adjacent to the posterior aspect of the trachea. The esophagus was compressed to the right, indicating the potential appearance of symptoms in the future. Consequently, we performed simultaneous bilateral video-assisted tho

Uniportal video-assisted thoracoscopic surgery for retained shrapnel in the thoracic cavity: An update surgical approach for removal of foreign body.

Uniportal video-assisted thoracoscopic surgery commonly used for removal of bullet, tumours and foreign bodies in the chest cavity. However, shrapnel fragments related war can injure many organs and life-threatening condition. Furthermore, standard treatment for removal foreign bodies retained in the chest is conventional thoracotomy. Alternatively, single port video-assisted thoracoscopic surgery approach can be used in patients hemodynamically stable. In addition, retained foreign bodies after a penetrating thoracic injury remain challenge for u-VATS surgeons. Here we present A 35-year-old m

Recommendation for management of patients with their first episode of primary spontaneous pneumothorax, using video-assisted thoracoscopic surgery or conservative treatment.

International guidelines do not recommend surgery for the first episode of primary spontaneous pneumothorax (PSP), except in cases of persistent air leak, hemopneumothorax, bilateral pneumothorax, or occupations at risk. However, these recommendations have been challenged because of a significant reduction in the recurrence rate in emerging studies. We evaluated the rationale of recommendations by systematically reviewing RCTs and observational studies by using the Grading of Recommendations, Assessment, Development, and Evaluations (GRADE) system. We searched articles in PubMed, EMBASE, and C

Single port video-assisted thoracoscopic surgery for removal of a retained bullet in the lung parenchyma; A case report with literature review.

INTRODUCTION AND IMPORTANCE: For many years, thoracotomy has been considered as the standard approach for thoracic injuries. The aim of the current paper is to report a successful use of video assisted thoracoscopic surgery (VATS) as the first line approach for the removal of a bullet retained deep in the lung parenchymal tissue. CASE PRESENTATION: A 46-year-old male presented with a vague pain in the chest at the site of a penetrating bullet injury 26 years prior. Chest and native chest Computed Tomography scan (CT scan) showed a 22 mm foreign body inside the substance of the left lower lobe

The efficacy of video-assisted thoracoscopic surgery lung biopsies in patients with Interstitial Lung Disease: a retrospective study of 66 patients.

BACKGROUND: Diagnosing a specific type of Interstitial Lung Disease (ILD) is a challenging process and often necessitates that a Video-assisted Thoracoscopic Surgery (VATS) Lung Biopsy be performed. By analysing the proportion of patients who have their treatment changed after undergoing a VATS lung biopsy, this study aimed to determine the utility of performing this procedure in patients with ILD. METHODS: The clinical data from sixty-six patients with suspected ILD, who underwent VATS lung biopsies at the New Royal Infirmary of Edinburgh (NRIE) in the period of 16th May 2011 - 11th February

[Current data and perspectives in minimally invasive video-assisted head and neck surgery: example of endoscopic exploration of the submandibular region].

The purpose of this study was to review the current status of research in this field and to describe the endoscopic technique for resection of the submandibular gland: a 15-mm incision is sufficient for the endoscopic material and gland resection without insufflation. The wound is minimal and the dissection is not traumatic. This innovating technique enables a magnified view of the structures providing an excellent teaching method.

Video-Assisted Thoracoscopic Surgery for Catamenial Pneumothorax: A Report of Five Cases Treated by Diaphragm Reefing with and without Chemical Pleurodesis.

BACKGROUND: Catamenial pneumothorax (CP), the most common manifestation of thoracic endometriosis syndrome, is a rare form of primary spontaneous pneumothorax that occurs in women of reproductive age. Although CP is usually treated surgically or with hormonal therapy, there is no standard treatment and recurrence is common. We performed video-assisted thoracoscopic surgery (VATS) for five patients with CP from 2014 to 2023. METHODS: Our surgical basic procedure (BP) was as follows. VATS was performed by using one port site for the camera and a 5-cm mini-thoracotomy. Perforated holes or bluebe

Effective application of indocyanine green for visualization of the thoracic duct during video-assisted thoracoscopic surgery for chylothorax: a case report.

BACKGROUND: Chylothorax is an accumulation of typically milky-appearing lymphatic fluid within the pleural cavity. High-output chylothorax may lead to a severe risk of death if not treated promptly. Several studies have reported the effectiveness of indocyanine green (ICG) for chylothorax following lung cancer surgery, esophagectomy, and congenital and recurrent idiopathic chylothorax. Here, we report a case of a successful treatment of idiopathic chylothorax in an adult by identifying and ligating the thoracic duct (TD) using fluorescent thoracoscopy with inguinal lymph node injection of ICG.

Devising the guidelines: the techniques of pulmonary nodule localization in uniportal video-assisted thoracic surgery-hybrid operating room in the future.

Pulmonary nodules beneath the pleura can be hard to visualize or palpate, especially during the uniportal thoracoscopic surgery. Conventionally, thoracic surgeons would use adjuvant modalities to localize the lesion preoperatively, of which computed tomography-guided hookwire implantation has been adopted most widely due to its feasibility and high success rate. However, procedure-associated complications such as pneumothorax and wire dislodgement can cause patient discomfort or localization failure. Occasionally more healthy tissue is resected than needed to guarantee the lesion is removed an

The diagnostic role of video-assisted thoracoscopic surgery in exudative pleural effusion and follow-up results in patients with nonspecific pleuritis.

OBJECTIVES: One of the unfortunate sequelae in obstetric brachial plexus palsy (OBPP) is upper limb length discrepancy. However, the influence of primary nerve reconstruction remains undetermined. In this study, the resultant discrepancy in children with OBPP who underwent primary reconstruction was analyzed in relation to the severity of the lesion, the timing of surgery, and the functional outcome following surgery. METHODS: Fifty-four patients that met the inclusion criteria were included in this study. Preoperative and postoperative bilateral scanograms were obtained to document the effec

Posterior uniportal video-assisted thoracoscopic surgery for resection of the apical segment of the right lower lobe followed by completion lobectomy.

Uniportal thoracoscopic resections offer less pain and better cosmetic results. They are usually performed through an antero-lateral incision. Posterior uniportal approach has not been described yet. A 65-year-old female was admitted to our clinic for the treatment of an adenocarcinoma, located in the apical segment of the right lower lobe. Owing to the ideal location and size, anatomical segment resection and radical lymphadenectomy was planned. Fissureless video-assisted thoracoscopic resection of the apical segment of the right lower lobe, using a uniportal posterior approach was performed,

Dexamethasone as an adjuvant to continuous erector spinae plane block for postoperative analgesia after video-assisted thoracoscopic surgery for pulmonary nodule surgery: a randomized controlled trial.

BACKGROUND: While dexamethasone is proven to enhance single-shot erector spinae plane block (ESPB), its role as an adjuvant in continuous ESPB catheters is unclear. This randomised controlled trial evaluated whether adding dexamethasone to ropivacaine improves analgesia after video-assisted thoracoscopic surgery (VATS). METHODS: 85 patients undergoing VATS with continuous ESPB were randomised to receive postoperative infusion of either 0.2% ropivacaine(C-ESPB group) or ropivacaine with 10 mg dexamethasone(D + C-ESPB group). The primary outcome was resting pain visual analog scale (VAS)at 12 h

A Serratus Posterior Superior Intercostal Plane Block for Postoperative Analgesia in Minimally Invasive Pectus Excavatum Repair Surgery with a Video-Assisted Thoracic Surgery Technique: Case Report.

Video-assisted thoracic surgery (VATS) is currently used for the repair of pectus excavatum. Analgesia after thoracic surgery can be provided with nerve blocks, intravenous drugs, or neuraxial techniques. Serratus posterior superior intercostal plane block (SPSIPB) is a novel interfascial plane block and it is performed between the serratus posterior superior muscle and the intercostal muscles at the level of the second and third ribs. In this case, we present our successful analgesic experience with SPSIPB in a patient who underwent minimally invasive pectus excavatum repair with a VATS techn

Trans-axillary retro-mammary gland route approach of video-assisted breast surgery can perform breast conserving surgery for cancers even in inner side of the breast.

BACKGROUND: The endoscopic surgery for inner-side cancer of the breast is usually performed by periareolar approach, but it often makes deformation or malposition of nipple and areola. The trans-axillary approach is favorable without making any injuries on breast skin. Furthermore, we devised a new approach of retro-mammary route without subcutaneous exfoliation, from axillary skin incision, to preserve skin touch sensation. METHODS: We have performed video-assisted breast surgery (VABS) on 200 patients since December 2001. The newly devised trans-axillary retromammary-route approach (TARM) w

Effect of liposomal bupivacaine thoracic paravertebral block on postoperative analgesia in patients undergoing video-assisted thoracoscopic surgery: a randomized controlled trial.

BACKGROUND: Thoracic paravertebral block (TPVB) can provide effective analgesia for patients undergoing video-assisted thoracoscopic surgery (VATS). However, the duration of analgesia achieved with conventional local anesthetics combined with dexamethasone remains limited. This study aimed to determine if liposomal bupivacaine (LB) is superior to bupivacaine combined with dexamethasone (BD) with respect to duration of postoperative analgesia in VATS patients. METHODS: Adults scheduled for VATS were randomized to treatment with LB or BD. The primary outcome was duration of analgesia. Secondary

Video-assisted thoracoscopic surgery versus open thoracotomy for thoracic trauma in hemodynamically stable patients: An updated systematic review and meta-analysis.

BACKGROUND: Video-assisted thoracoscopic surgery (VATS) is increasingly used in thoracic trauma, yet high-certainty comparative evidence against thoracotomy remains limited. We updated the only prior comprehensive meta-analysis (Wu et al., 2015) and re-derived all estimates from source-verified study-level data. METHODS: Six databases were searched through December 2025. Studies comparing VATS with thoracotomy in hemodynamically stable adult patients with thoracic trauma were eligible. Random-effects models with restricted maximum likelihood estimation and Knapp-Hartung adjustment were used;

High-dose glucocorticoids in the treatment of postoperative pain after video-assisted thoracoscopic surgery-protocol for systematic review and meta-analysis.

BACKGROUND: Video-assisted thoracoscopic surgery (VATS) is widely used in lung cancer surgery, as this technique causes less pain and faster recovery than open thoracotomy. However, significant postoperative pain persists in a number of patients, often leading to increased opioid use and opioid-related adverse events in addition to prolonged admission times. Perioperatively administered glucocorticoids have been demonstrated effective in reducing pain after other types of surgeries, but the effect in VATS remains unclear. METHODS: This systematic review will assess the impact of glucocorticoi
