Vitiligo: Pathogenesis, clinical variants and treatment approaches.

Vitiligo is a common chronic acquired disease of pigmentation whose etiology is unknown, which usually occurs with asymptomatic whitish patch or macule. Although several hypotheses have been proposed in the literature, the leading theory is still the auto-immune etiology linked to specific genetic mutations. Vitiligo can also be associated with several autoimmune diseases, including autoimmune thyroid diseases, alopecia areata, and halo nevi. Sensorineural hearing loss was reported in several vitiligo patients due to a reduction in the number of melanocytes contained in the membranous labyrint…

Vitiligo--Part 2--classification, histopathology and treatment.

In an unprecedented effort in the field of vitiligo, a global consensus resulted on a suggested new classification protocol for the disease. The main histopathological finding in vitiligo is the total absence of functioning melanocytes in the lesions, while the inflammatory cells most commonly found on the edges of the lesions are CD4+ and CD8+ T lymphocytes. Physical and pharmacological treatment strategies aim to control the autoimmune damage and stimulate melanocyte migration from the unaffected edges of lesions and the outer hair follicle root sheath to the affected skin; moreover, surgica…

Ultraviolet-based therapy for vitiligo: what's new?

Vitiligo is an ancient disease in which depigmented and hypopigmented macules appear on the skin. It is a disfiguring condition that may lead to severe psychological trauma. Among the many treatment modalities available for use in vitiligo, those using light therapy, and in particular ultraviolet (UV) light, are some of the most effective treatments. UV-based therapy includes phototherapy (narrowband UVB), photochemotherapy (psoralens with UVA), and targeted phototherapy (excimer laser and excimer lamp). It is important for any practitioner of UV-based therapy to understand the efficacy of eac…

Koebner's phenomenon in vitiligo: European position paper.

Koebner's phenomenon (KP) has been observed in a number of skin diseases, including vitiligo. Its clinical significance in vitiligo with respect to disease activity and course is still debatable, while its relevance for surgical techniques has been demonstrated in some reports. We present a literature review on the currently known facts about KP in vitiligo, including details of clinical, experimental, and histopathological changes. The consensus view is that there are still no methods to define and assess KP in vitiligo. A new classification is proposed to allow an evaluation of KP in daily p…

Juvenile-onset hypopigmented mycosis fungoides mimicking vitiligo.

BACKGROUND: Mycosis fungoides (MF) is the most common form of cutaneous T-cell lymphoma and typically affects older adults. It is estimated … tocks, which have evolved over the past 7 years. This condition had previously been misdiagnosed as vitiligo. Recent immunohistologic and molecular biology studies are consistent with MF. RESULTS AND CONCLUSIONS: Given that hypopigmented MF is an uncommon condition, it may not be clinically suspected in the pediatric population. Histopathologic, immunophe …

Vitiligo following a combined liver-kidney transplant.

We report an Afro-Caribbean male who developed vitiligo 10 days following a combined liver-kidney transplant from a Caucasian donor. Neither the donor nor the recipient had any previous history of vitiligo, nor of autoimmunity. The depigmentation gradually resolved by 8 weeks post-transplant with topical corticosteroids and standard maintenance immunosuppression. We propose that the skin depigmentation occurred due to the destruction of melanocytes by donor-derived alloreactive cytotoxic T-lymphocytes or antibody transferred during transplantation. Although vitiligo has been described in patie…

Vitiligo prevalence study in Shaanxi Province, China.

BACKGROUND: Recent publications, especially those based on population surveys, show that the presumed vitiligo prevalence of 1-2% is overestimated. OBJECTIVE: To obtain the vitiligo prevalence in Shaanxi Province, China, through a population survey. METHODS: Approximately one-thousandth of the 36.05 million people in Shaanxi Province, China, were selected through stratified four-stage cluster sampling. They lived in 180 investigation units and all were investigated in a door-to-door survey. Vitiligo and suspected vitiligo patients were marked in the basic questionnaire. They were encouraged …

Childhood vitiligo: a prospective hospital-based study.

Eighty-eight consecutive children presenting with vitiligo to a hospital dermatology clinic in a 1-year period were assessed clinically, and for autoimmune and endocrine disorders. Vitiligo started between 8 and 12 years of age in 51% of our patients. A positive family history was obtained in 27.3% of the patients. Vitiligo vulgaris was the most common clinical type seen. Three patients, though clinically asymptomatic, showed antithyroid antibodies. Our results are compared with those published previously on the subject.
