In 1901 Eugene L. Opie, MD, a Dean of Washington University School of Medicine (Figure 1) described the histologic presence of amyloid within the pancreatic islets of patients with type 2 diabetes mellitus. Although amyloid is present in at least 70 percent of type 2 diabetic patients, the subject has not been clinically emphasized. This paper reviews the history of Opie's discovery and current clinical implications of islet amyloid.

Many patients with psychiatric disorders are refractory to current treatments, prompting a search for novel therapies. Based on the efficacy of electroconvulsive therapy, there is interest in developing other brain stimulation methods in psychiatry. Alternatives include vagal nerve stimulation, repetitive transcranial magnetic stimulation and deep brain stimulation. This paper reviews these methods, highlighting strategies currently being used and developed at Washington University and Barnes-Jewish Hospital in St. Louis.

Based on the claims that transcutaneous electrical nerve stimulation is effective in myoclonic dystonia and essential tremor, we evaluated its acute effects in 5 patients with essential tremor and 2 patients with tremor attributed to peripheral neuropathy using as parameters the Washington Heights-Inwood Genetic Study of Essential Tremor rating scale, self-reported impression, and recording of electromyographic activity. We found no significant improvement in any of the parameters tested.

In December 1980, a 26-year-old black man came to the Washington Hospital Center Eye Clinic with a unilateral decrease in visual acuity, vitreous inflammation, optic disk pallor, and a degenerated retinal pigment epithelium. A motile subretinal worm was subsequently found. Unlike an earlier series of 18 patients with diffuse unilateral subacute neuroretinitis reported by other authors, this patient exhibited eosinophilia and a highly positive ELISA for Toxocara.

A patient with an 8-month history of chronic progressive visual loss was referred to the Neuro-ophthalmology Consult Office of Washington University with a diagnosis of chronic optic neuritis. Previous diagnostic studies, including CAT scan, had been negative. Because chronic progressive visual loss is considered a hallmark of mass lesion, more vigorous investigation, including repeat CAT scans, was undertaken, leading to the diagnosis and removal of a 2 cm meningioma.
