End-tidal CO2 responses and ventilatory correlates during voluntary hyperventilation: a repeated-measures pilot study.

BACKGROUND: Controlled voluntary hyperventilation is widely used to study the ventilatory control of end-tidal CO2 (PetCO2), but the breath-by-breath determinants of PetCO2 during an externally paced maneuver remain incompletely characterized. This repeated-measures pilot study aimed to describe PetCO2 changes and to quantify associations with ventilatory timing and expired gas output during paced voluntary hyperventilation. METHODS: Thirty healthy males (age range 19.2-39.8) performed a metronome-paced voluntary hyperventilation maneuver at 30 breaths/min intended to last up to 3 min. Breath

Cerebral cortex and respiratory muscles perfusion during spontaneous breathing attempts in ventilated patients and its relation to weaning outcomes: a protocol for a prospective observational study.

INTRODUCTION: In addition to the well-documented factors that contribute to weaning failure, increased energy demands of the respiratory muscles during spontaneous breathing trials (SBTs) might not be met by sufficient increases in energy supplies. This discrepancy may deprive blood and oxygen of other tissues. In this context, restrictions in perfusion of splanchnic organs and non-working muscles during SBT have been associated with weaning failure. However, alterations in perfusion of the brain during the weaning process are less well understood. OBJECTIVE AND HYPOTHESIS: To investigate whe

The degree of breath methane production in IBS correlates with the severity of constipation.

BACKGROUND: Recent work has demonstrated that among irritable bowel syndrome (IBS) subjects, methane on lactulose breath test (LBT) is nearly universally associated with constipation predominance. This work has been based on subjective constipation outcomes. In this study, methane is compared to constipation in another population of IBS subjects with constipation being determined both subjectively and objectively. METHODS: A nested study was conducted in subjects enrolled in a double-blind randomized placebo-controlled study. After consent, subjects were asked to complete a stool diary for 7

Exercise Intolerance in Heart Failure: Central Role for the Pulmonary System.

We propose that abnormalities of the pulmonary system contribute significantly to the exertional dyspnea and exercise intolerance observed in patients with chronic heart failure. Interventions designed to address the deleterious pulmonary manifestations of heart failure may, therefore, yield promising improvements in exercise tolerance in this population.

Respiratory cycle-related EEG changes: response to CPAP.

STUDY OBJECTIVES: Respiratory cycle-related EEG changes (RCREC) quantify statistically significant synchrony between respiratory cycles and EEG spectral power, vary to some extent with work of breathing, and may help to predict sleepiness in patients with obstructive sleep apnea. This study was designed to assess the acute response of RCREC to relief of upper airway obstruction by positive airway pressure (PAP). DESIGN: Comparison of RCREC between baseline diagnostic polysomnograms and PAP titration studies. SETTING: Accredited academic sleep disorders center. PATIENTS: Fifty adults referre…

Intratracheal pressure: a more accurate reflection of pulmonary airway pressure in pediatric patients with respiratory failure.

OBJECTIVES: Peak inflation pressure (PIP) on many ventilators (P(vent)), measured distal to the exhalation limb or Y-piece of the breathing circuit, is assumed as the pressure applied to the airways and lungs. However, in vitro studies show P(vent) data are spurious. There are no studies evaluating the accuracy of P(vent) data for pediatric patients with acute respiratory failure. We hypothesized that intratracheal airway pressure (P(T)) is more accurate than P(vent) and that by using P(vent), abnormally increased imposed resistive work of breathing (WOBi) may go undetected. DESIGN: Prospecti…

Dynamic hyperinflation during bronchoconstriction in asthma: implications for symptom perception.

OBJECTIVE: The objective of this study was to examine the relationship between respiratory symptom intensity and quality and dynamic lung hyperinflation (DH) during induced bronchoconstriction in asthma. PATIENTS AND METHODS: Subjects with asthma (n = 116) underwent baseline spirometry and lung volume measurement followed by high-dose methacholine challenge testing (MCT) [maximum decrease in FEV(1) of 50% from baseline]. Dyspnea intensity (Borg scale) was measured after each dose of methacholine. Qualitative descriptors of breathlessness and functional residual capacity (FRC) were measured at

Noninvasive ventilation for acute respiratory failure: a prospective randomised placebo-controlled trial.

The aim of the present study was to clarify whether the known effects of noninvasive positive-pressure ventilation (NPPV) in patients with respiratory failure are real or due to placebo effects and whether early application of NPPV in the emergency department leads to rapid improvement of the patients condition and outcome. A prospective randomised placebo-controlled study was conducted in 20 patients with severe acute respiratory failure (ARF) secondary to an acute exacerbation of chronic obstructive pulmonary disease (COPD) or acute pulmonary oedema, not improving under conventional medical

CT analysis of lung density changes in patients undergoing total body irradiation prior to bone marrow transplantation.

Sequential changes in lung density measured by CT are potentially sensitive and convenient monitors of lung abnormalities following total body irradiation (TBI). Methods have been developed to compare pre- and post-TBI CT of lung. The average local features of a cross-sectional lung slice are extracted from three peripheral regions of interest in the anterior, posterior, and lateral portions of the CT image. Also, density profiles across a specific region may be obtained. These may be compared first for verification of patient position and breathing status and then for changes between pre- and

Improving patient preparation and comfort in voluntary DIBH - Single-centre quality improvement evaluation.

INTRODUCTION: Voluntary deep-inspiration breath-hold (vDIBH) is commonly used in left breast cancer patients during radiotherapy to reduce cardiac dose and improve setup accuracy. However, some patients have trouble maintaining a consistent, adequate, breath-hold during radiotherapy. This service evaluation assessed the impact of the RESPIRE tool-an online resource offering instructional videos and audio guides-introduced prior to simulation to improve patient compliance and preparedness. METHODS: Fifteen eligible breast cancer patients received RESPIRE information during a clinic appointment

Highly selective and sensitive online measurement of trace exhaled HCN by acetone-assisted negative photoionization time-of-flight mass spectrometry with in-source CID.

Exhaled hydrogen cyanide (HCN) has been extensively investigated as a promising biomarker of the presence of Pseudomonas aeruginosa in the airways of patients with cystic fibrosis (CF) disease. Its concentration profile for exhalation can provide useful information for medical disease diagnosis and therapeutic procedures. However, the complexity of breath gas, like high humidity, carbon dioxide (CO2) and trace organic compounds, usually leads to quantitative error, poor selectivity and sensitivity for HCN with some of existing analytical techniques. In this work, acetone-assisted negative phot

Relation between various chromium compounds and some other elements in fumes from manual metal arc stainless steel welding.

For the years 1987-1990 160 individual samples of manual metal arc stainless steel (MMA/SS) welding fumes from the breathing zone of welders in four industrial plants were collected. Concentrations of soluble and insoluble chromium (Cr) III and Cr VI compounds as well as of some other welding fume elements (Fe, Mn, Ni, F) were determined. Concentration of welding fumes in the breathing zone ranged from 0.2 to 23.4 mg/m3. Total Cr amounted to 0.005-0.991 mg/m3 (including 0.005-0.842 mg/m3 Cr VI). Total Cr content of fumes varied from 0.1 to 7.4%. The distribution of particular Cr compounds was:

Expiratory lung MRI: a simple, sensitive method to quantify and visualise regional gas trapping in cystic fibrosis.

BACKGROUND: Computed tomography is the gold standard for visually assessing gas trapping, a hallmark of early lung disease in people with cystic fibrosis (pwCF), but its use is limited in children. Lung proton magnetic resonance imaging (¹H-MRI) offers a simple, non-ionising alternative. This work aimed to use breath-hold 1H-MRI to visualise and quantify gas trapping in pwCF. METHODS: 24 normal controls (9 adults, 15 children) and 27 pwCF underwent breath-hold 1H-MRI at residual volume (RV) and total lung capacity (TLC). For each participant, a threshold was defined from the TLC image and app

Volatile organic compounds in exhaled breath as non-invasive biomarkers for Gastrointestinal motility disorders: an exploratory study.

BACKGROUND AND AIMS: Gastrointestinal (GI) motility disorders are characterized by abnormalities in the motor functions of the GI tract. The diagnostic evaluation of these disorders frequently involves invasive and time-consuming examinations, for which access may be limited. Volatile organic compounds (VOCs) could serve as non-invasive alternative. Therefore, the aim of this study was to explore the potential of exhaled breath VOCs as biomarkers in patients with GI symptoms and potential GI motility disorders. METHODS: In this exploratory, prospective study, breath samples were obtained from

What is central sleep apnea?

Central sleep apnea (CSA) describes a group of conditions in which cessations in air flow occur without respiratory effort. In contrast, obstructive sleep apnea patients have ongoing respiratory effort during respiratory events. However, considerable overlap exists in the pathogenesis and clinical presentation of obstructive sleep apnea and CSA. A good working knowledge of the mechanisms underlying CSA is important for optimal clinical care. In general, CSA can be classified into those with excessive drive (eg, Cheyne-Stokes breathing) versus those with inadequate drive (eg, sleep hypoventilat

Respiratory muscle strength pre- and post-maximal apneas in a world champion breath-hold diver.

Maximal static dry, that is, on land, apneas (breath-holds) result in severe hypoxemia and hypercapnia and have easy-going and struggle phases. During the struggle phase, the respiratory muscles involuntarily contract against the closed glottis in increasing frequency and magnitude, that is, involuntary breathing movements (IBMs). IBMs during maximal static apnea have been suggested to fatigue respiratory muscles, but this has yet to be measured. Thus, the purpose of this study was to quantify respiratory muscle strength pre- and post-apneas in an elite, world champion, world record-holding ap

A comparison of internal target volume definition by limited four-dimensional computed tomography, the addition of patient-specific margins, or the addition of generic margins when planning radical radiotherapy for lymph node-positive non-small cell lung cancer.

AIMS: Radical radiotherapy for stage II/III non-small cell lung cancer (NSCLC) includes the primary tumour and positive mediastinal lymph nodes in the clinical target volume (CTV). These move independently of each other in magnitude and direction during respiration. To prevent a geographical miss, a generic margin is usually added to the CTV to create an internal target volume (ITV). Previous studies have investigated the use of additional breath-hold computed tomography to generate patient-specific ITVs for primary tumours alone. We used a similar technique to investigate the generation of pa
