[The role of video-nystagmography in the diagnosis of acoustic neuroma].

AIM: Schwannoma of the vestibular nerve represents 75% of all expansive processes affecting the pontocerebellosum angle. Hearing loss is the most frequent symptom at diagnosis (86%) with or without tinnitus, in the intracanalicular tumors. Vestibular symptoms are described in 60% of patients. MATERIAL AND METHOD: We study twenty cases of acoustic neuromas diagnosed between years 2000 and 2002 in both Otolaryngology and Neurosurgery Departments in our hospital. Videonystagmography (VNG) was performed in all of them. Videonystagmographic findings were analyzed statistically together with the tu

Histopathology of the inner ear in unoperated acoustic neuroma.

Although hearing loss is the most common presenting symptom in patients with acoustic neuroma, the pathophysiology of hearing loss associated with acoustic neuroma is unknown. Although primary dysfunction of the auditory nerve is intuitively logical, available histopathologic and clinical data suggest that although neural degeneration is common, it alone does not adequately account for hearing loss in many cases. The purpose of this study was to evaluate 11 cases of unoperated unilateral acoustic neuromas. Temporal bones were identified by means of a search mechanism provided by the National T

Expression of angiogenic growth factors in acoustic neurinoma.

OBJECTIVES: Acoustic neurinoma (AN) can grow to a large size, but the growth-promoting molecular pathways remain unknown. As angiogenesis has been described as being activated in many cancers, we undertook this study in order to examine the microvascular network of AN and the expression of angiogenic growth factors and their cognate receptors in AN. The aim was to draw conclusions regarding the underlying mechanisms and potential benefit of a pathway-specific anticancer therapy. MATERIAL AND METHODS: Surgical specimens from 34 patients with AN were analysed immunohistochemically for the expre

Surgery combined with radiosurgery of large acoustic neuromas.

UNLABELLED: The treatment of acoustic neuromas has been improved by advancements in microsurgical techniques and in radiosurgery. To further elucidate the degree of clinical improvement, we evaluated the treatment results of a combination of surgery and radiosurgery for large acoustic neuromas. METHODS: From January 1994 through December 2000, we treated 14 patients with large acoustic neuromas using a combination of surgery and radiosurgery. Of these, 8 were male and 6 were female patients, with an average age of 47 years (range, 18-64). The average maximum diameter of the tumor was 42 mm (r

Handheld cellular telephones and risk of acoustic neuroma.

The hypothesis that intracranial energy deposition from handheld cellular telephones causes acoustic neuroma was tested in an epidemiologic study of 90 patients and 86 control subjects. The relative risk was 0.9 (p = 0.07) and did not vary significantly by the frequency, duration, and lifetime hours of use. In patients who used cellular telephones, the tumor occurred more often on the contralateral than ipsilateral side of the head. Further efforts should focus on potentially longer induction periods.

Orofacial paraesthesia: an unusual presentation of acoustic neuroma.

Patients with acoustic neuromas commonly present with unilateral hearing loss, tinnitus and unsteadiness. An uncommon initial feature is involvement of the trigeminal nerve. The unusual feature in the case reported here was orofacial paraesthesia, which preceded hearing loss by 3-4 years. Diagnosis was made following a referral by the patient's dentist.

Multicystic acoustic neurinoma: report of two cases.

A report of two unusual and remarkably similar cases of multicystic acoustic neurinoma in 46 and 60 year old patients is presented. The clinical presentation mimicked a regular acoustic neurinoma. The tumours in each case were similar in their radiological appearance. The entire tumour had a honeycomb appearance with well defined and multiple cysts. Each cyst contained xanthochromic fluid. Like in any other cystic acoustic neurinoma, the presence of multiple cysts made internal decompression of the tumour relatively easy but the dissection of the tumour from the adjoining cranial nerves was di

Susceptibility to acoustic trauma in young and aged gerbils.

The effect of age on susceptibility to noise-induced hearing loss (NIHL), the effect of gender on the interaction of age-related hearing loss (ARHL) and NIHL, and the relative contributions of ARHL and NIHL to total hearing loss are poorly understood. The issues are difficult to resolve empirically in human subjects because of lack of control over extrinsic variables and for ethical reasons. Accordingly, these issues were examined in a well-studied animal model of both ARHL and NIHL, the Mongolian gerbil. Animals were exposed to an intense tone (3.5 kHz, 113 dB SPL, 1 h) either as young adults

Miniature acoustic guidance system for endotracheal tubes.

Ensuring that the distal end of an endotracheal tube (ETT) is properly located within the trachea, and that the tube is not obstructed by mucus deposition, is a major clinical concern in patients that require mechanical ventilation. A novel acoustic system was developed to allow for the continuous monitoring of ETT position and patency. A miniature sound source and two sensing microphones are placed in-line between the ventilator hose and the proximal end of the ETT. Reflections of an acoustic pulse emitted into the ETT lumen and the airways are digitally analyzed to estimate the location and

Acoustic neuroma with malignant transformation. Case report.

The authors describe the case of a 57-year-old woman who had a right-sided hearing disturbance that had remained untreated for 1 year. The diagnosis was of a right cerebellopontine angle tumor, and the patient underwent its removal via retrosigmoid approach. Pathologically, the tumor was a typical benign neuroma. Growth of residual tumor was detected 4 years after the initial operation, and it was treated with gamma knife surgery (GKS). Six months later, the tumor had grown, and the patient underwent surgery via a combined retrosigmoid-translabyrinthine approach. Abnormal mitotic figures were

Otolith function after acoustic neuroma surgery.

The response of the vestibular system after acoustic neuroma surgery was investigated in nine patients. The otolith system was studied by means of ocular counterrolling, assessed by video oculography. Horizontal vestibulo-ocular reflex (VOR) function was tested by the sinusoidal harmonic acceleration test using electronystagmography. The results were compared with those obtained from a normal control population. The response to slow rotation tests was symmetric, but the gain was significantly reduced when compared to the normal population. Phase lag was significantly increased. No difference i

XML transcription method for biomedical acoustic signals.

We have proposed applying MIDI technology to coding of biomedical acoustic signals such as heart sounds used in medical diagnosis for constructing medical audio databases. According to our several experiments, we could correspond encoded MIDI event data or musical notes with heart sound components used for clinical diagnosis. However, precise discrimination among abnormal heart sounds and murmurs was difficult owing to insufficiency of analysis precision in our proposed method. Then we have improved its coding precision and supported harmonic or formant analysis in order to extend our applicat

Effect of acoustic clutter on prey detection by bats.

Bats that capture animal prey from substrates often emit characteristic echolocation calls that are short-duration, frequency-modulated (FM) and broadband. Such calls seem to be suited to locating prey in uncluttered habitats, including flying prey, but may be less effective for finding prey among cluttered backgrounds because echoes reflecting from the substrate mask the acoustic signature of prey. Perhaps these call designs serve primarily for spatial orientation. Furthermore, it has been unclear whether the acoustic image conveyed by FM echoes enables fine texture discrimination, or whether

[Our current results with acoustic neurinoma surgery].

Out of 130 CPA tumors operated on between 1993 and 1997, 91 were vestibular schwannomas less than 25 mm with preoperative hearing and normal facial function. The pre and postoperative facial and hearing functions were analyzed prospectively. The surgical technique applied was the complete tumor removal via retrosigmoid approach under facial nerve monitoring and CPA endoscopy. Clinical features were analyzed to determine postoperative facial grading (House-Brackmannn) and tonal and vocal audiometrical datas (PTA-SDS) at 8 days, 90 days and 1 year. We achieved 96 % of good facial results (Grade
