Off-Label Use of Janus Kinase Inhibitors in Inflammatory Cutaneous Diseases.

Dysregulation of Janus kinase (JAK) pathways from uncontrolled cytokine signaling comprises the pathological basis for many complex inflammatory cutaneous disorders. Oral JAK inhibitors, upadacitinib, tofacitinib, and baricitinib targeting JAK 1 and JAK 1/3, respectively, are currently US Food and Drug Administration (FDA)-approved for several rheumatic conditions. However, studies have shown that JAK-mediated signaling pathways are involved in many immune-related dermatologic conditions. As a result, for recalcitrant diseases, JAK inhibitors are potential alternative therapies due to their br

Indomethacin induced vasculitis.

We present a case of a 22 year old male with a personal history of ankylopoyetic spondylitis who has presented in the last year cutaneous lesions characterized by papulo erythematous (palpable purpura) localized in both distal inferior extremities in 3 occasions. These 3 episodes have always coincided with the administration of indomethacin due to increasing articular discomfort. The anatomical pathology of the lesions demonstrated the typical finding of leukocytoclastic vasculitis. The pathogenesis of the cutaneous process is discussed, considering the intervening immunocomplex wherein indome

Panitumumab-Induced Periorbital Dermatitis: A Case Report.

Panitumumab is a recombinant, fully humanized immunoglobulin G2 monoclonal antibody targeting the epidermal growth factor receptor (EGFR). It is approved for the first- and second-line treatment of advanced wild-type KRAS colorectal cancer. Although common cutaneous side effects include acneiform dermatitis, folliculitis, and xerosis, ocular toxicities have occasionally been reported. Herein, we report the case of an 81-year-old Thai female with chemorefractory advanced stage sigmoid colon cancer who developed isolated periorbital dermatitis following treatment with panitumumab plus modified F

Cutaneous Methotrexate-Related Epstein-Barr Virus-Positive Diffuse Large B-Cell Lymphoma in a Patient with Granulomatous Cutaneous T-Cell Lymphoma: A Case Report and Literature Review.

Methotrexate-related lymphoproliferative disorders (MTX-LPDs) are immunodeficiency diseases following methotrexate (MTX) administration, mainly occurring in rheumatoid arthritis patients. Although uncommon, MTX-LPDs have been reported in some patients with psoriasis, dermatomyositis, and cutaneous T-cell lymphoma (CTCL) who received MTX. Granulomatous mycosis fungoides (GMF) is a rare subtype of cutaneous T-cell lymphoma, where MTX is one of the treatment options in recalcitrant cases. Herein, we report a case of a 72-year-old female patient with GMF who additionally developed cutaneous Epstei

Cutaneous epitheliotropic lymphoma in a ferret.

An 8-year-old spayed female ferret was examined for diffuse generalized alopecia, erythema, erosions, crusts, and ulcerated plaques that were nonresponsive to long-term administration of corticosteroids. Cutaneous epitheliotropic lymphoma was diagnosed on the basis of histologic examination of skin biopsy specimens. Neoplastic cells were determined to be of T-lymphocytic origin by results of immunohistochemical staining with a rabbit anti-CD3 monoclonal antibody. Additional laboratory abnormalities detected included anemia, azotemia, isosthenuria, pyuria, and bacteriuria. Treatment included is

Anifrolumab for the treatment of cutaneous lupus erythematosus: a 10-case series.

Anifrolumab is a monoclonal antibody that targets the type I interferon receptor subunit 1. It was approved for the treatment of systemic lupus erythematosus by the US Food and Drug Administration in 2021 and by the European Medicines Agency in 2022. Although phase III clinical trials reported a statistically significant improvement in skin lesions, anifrolumab has not yet been approved for the treatment of cutaneous lupus erythematosus. We present a case series that includes 10 patients with cutaneous lupus erythematosus who demonstrated a rapid and sustained response to anifrolumab.

[Fatal Toxic Epidermal Necrolysis Induced by Diclofenac Re-Administration in the Teaching Hospital of Bouaké (Côte d'Ivoire)].

Lyell's syndrome or toxic epidermal necrolysis is an acute and severe cutaneous adverse drug reaction with a significant morbidity and mortality. It is a very rare condition but a vital emergency with a poor prognosis. The diagnosis is clinical and confirmed by a cutaneous biopsy showing a necrosis of the epidermis. It can be due to many drugs including non-steroidal anti-inflammatory drugs. We report a case of fatal Lyell's syndrome after oral re-administration of diclofenac in a patient with a previous history of diclofenac-related Stevens-Johnson syndrome, four years back.

Cutaneous leishmaniasis reactivation 2 years after treatment caused by systemic corticosteroids - first report.

American tegumentary leishmaniasis (ATL), an endemic anthropozoonosis in various countries in the world, is caused by parasites of the genus Leishmania. Despite reports on ATL reactivation as a result of immunosuppression, to the best of our knowledge, this paper describes the first case of ATL reactivation in its localized form (cutaneous leishmaniasis) associated with the administration of systemic corticosteroids. The possible action of corticosteroids on the host immune response to the parasite in patients with localized cutaneous leishmaniasis is discussed. This report demonstrates the po

Cutaneous toxicities of cancer therapy.

This review highlights the cutaneous side effects associated with the administration of chemotherapy and discusses the management of these conditions. Rapidly growing cells are the targets of chemotherapy, so the skin, hair follicles, and nail matrix are frequently affected by chemotherapy. Chemotherapy skin reactions are more likely toxic than allergic reactions. The most common cutaneous reactions are alopecia, hyperpigmentation, hand--foot syndrome, radiation recall, hypersensitivity, extravasation injuries, and nail dystrophies. While these side effects are generally not life threatening,

Primary cutaneous nocardiosis with multiple, subcutaneous abscesses in a patient with sarcoidosis.

A case of multiple subcutaneous nocardial abscesses is reported in a patient who received systemic corticosteroids over a period of about 4 years for the treatment of visceral sarcoidosis. The diagnosis of subcutaneous abscess caused by Nocardia asteroides was made based on bacteriological examination. It could not be determined before treatment whether the abscesses represented primary or secondary nocardiosis. Surgical drainage and systemic administration of minocycline hydrochloride dramatically resolved the cutaneous lesions. To the authors' knowledge, this is the first reported case in Ja

Tapentadol-induced urticarial rash in a patient with acute pancreatitis.

Tapentadol, a dual-action opioid, is increasingly used for pain management in acute pancreatitis (AP). Although its common side effects are well-documented, cutaneous reactions are rare. We report a 30-year-old gentleman with AP who developed generalized urticaria following oral tapentadol use. The patient had no prior history of atopy or drug allergies. The rash resolved after discontinuation of tapentadol and administration of a short course of corticosteroids and antihistamines. Cutaneous reactions to tapentadol are extremely rare but should be recognized promptly to ensure timely managemen

Generalized atrophic sarcoidosis with ulcerations.

A 53-year-old man presented to the Manhattan (NY) Veterans Administration Hospital with a 20-year history of generalized atrophic lesions associated with lower extremity ulcerations. Cutaneous biopsy specimens and other laboratory data confirmed the diagnosis of sarcoidosis involving the skin, lungs, eyes, joints, bones, pituitary gland, gonads, and liver. To our knowledge, generalized atrophic sarcoidal lesions have not been described previously in the English literature. Ulcerated cutaneous sarcoidal lesions, though recognized, are only rarely seen in the disease. The ulcerations, but not th

Primary cutaneous mucormycosis caused by Mucor irregularis in an elderly person.

Primary cutaneous mucormycosis is a rare but often lethal severe fungal infection, which usually occurs in immunocompromised patients. We report a case of primary cutaneous mucormycosis caused by Mucor irregularis in an elderly patient. Seven months after the surgical dissection of the involved skin, cutaneous mucormycosis recurred at the peripheral edge of the skin graft. Shortly subsequent to the administration of liposomal amphotericin B, the remaining skin lesion was excised again. M. irregularis is rarely but increasingly reported as a cause of mucormycosis in immunocompetent individuals,

Manifestation of cutaneous polyarteritis nodosa during interferon therapy for chronic hepatitis C associated with primary biliary cirrhosis.

Interferon alpha-2b was administered to a 50-year-old Japanese woman with chronic hepatitis C associated with primary biliary cirrhosis. Two months after the beginning of the interferon alpha-2b therapy a systemic nodular, erythematous rash developed. Histological analysis of the skin revealed typical features of necrotizing arteritis. Because there was no microhematuria, and no microaneurysms were detected on abdominal angiography, a diagnosis of cutaneous polyarteritis nodosa was made. A good outcome was achieved after interferon alpha-2b was discontinued and prednisolone was administered in

An 82-year-old man with new skin lesions after COVID-19 vaccination.

Background: During the height of the SARS CoV-2 (severe acutre respiratory syndrome coronavirus disease 2019 [COVID 19]) pandemic, there have been numerous case reports of cutaneous reactions shortly after COVID-19 vaccine administration. Most reported cases are local injection-site reactions, whereas persistent or delayed cutaneous reactions have not been as common. Methods: We present the case of an 82-year-old man with persistent rash after the second COVID-19 vaccination. Results: A specific diagnosis was confirmed after the third skin biopsy. Conclusion: Patients are frequently referred t

Imatinib mesylate-induced lichenoid drug eruption.

Imatinib mesylate (imatinib) is a tyrosine kinase inhibitor initially approved by the US Food and Drug Administration in 2001 for chronic myeloid leukemia (CML). Since then, the number of indicated uses for imatinib has substantially increased. It is increasingly important that dermatologists recognize adverse cutaneous manifestations of imatinib and are aware of their management and outcomes to avoid unnecessarily discontinuing a potentially lifesaving medication. Adverse cutaneous manifestations in response to imat-inib are not infrequent and can include dry skin, alopecia, facial edema, and

Optimizing Voriconazole Dosing in Extremely Premature Infants With Aspergillus Infections.

BACKGROUND: Primary cutaneous aspergillosis can disseminate in premature infants. Voriconazole, dosed every 12 hours, is the preferred antifungal agent. Prior studies show difficulty achieving therapeutic targets with this regimen. We report the safety and attainment of therapeutic targets with voriconazole dosed q8h with careful therapeutic drug monitoring. METHODS: Retrospective review of 4 extremely premature infants with primary cutaneous aspergillosis given voriconazole divided every 8 hours with therapeutic drug monitoring. Demographics, voriconazole administration, treatment duration,
