A European survey on attitudes towards pain and end-of-life issues in locked-in syndrome.

OBJECTIVES: Patients with locked-in syndrome often self-report a higher quality of life than generally expected. This study reports third-person attitudes towards several salient issues on locked-in syndrome. METHODS: Close-ended survey among conference attendees from 33 European countries. Analysis included chi-square tests and logistic regressions. RESULTS: From the 3332 respondents (33% physicians, 18% other clinicians, 49% other professions; 47% religious), 90% agreed that patients with locked-in syndrome can feel pain. The majority (75%) disagreed with treatment withdrawal, but 56% did

Advance Care Planning in a Geriatric Primary Care Clinic: A Retrospective Chart Review.

PURPOSE:: Advance care planning (ACP) is theorized to benefit both the patient and their family when end of life is near as well as earlier in the course of serious illness. However, ACP remains underutilized, and little is known about the nature of ACP documentation in geriatrics practices. The study investigated the prevalence and nature of ACP documentation within a geriatric primary care clinic. METHODS:: A retrospective chart review was conducted on a randomly selected sample of electronic medical record (EMR) charts. The sample consisted of patients aged 65 and older who were seen in th

A Novel Tool for Assessing the Content and Quality of Goals of Care Documentation.

BACKGROUND: Addressing goals of care (GOC) serves a crucial role throughout the course of a patient's illness, but for it to be useful and e … important for GOC notes: participants, prognosis, patient goals and values, treatment options, and advance directives. As part of an internal quality improvement initiative at our quaternary academic referral center in the Northeast United States, a novel note assessment tool was constructed and iteratively adjusted to achieve 80% interrater reliability. We then us …

How Volume of Neurosurgical Interventions Impacts Patient Outcomes: An Analysis of 30,000 Craniectomies and Craniotomies.

OBJECTIVE: To determine the association between neurosurgical intervention (NSI) rates-defined as craniectomy or craniotomy, across trauma c … ed adult trauma patients with SDH >10 mm. Patients with penetrating injuries, nonsurvivable trauma, advance directives limiting care, or ED death were excluded. TCs were stratified into tertiles based on the NSI volume as low, middle, and high volume TCs. Outcomes measured were in-hospital mortality and the rate of favorable discharge. Multivariable regression analy …

BACKGROUND: Recently, the news media have reported on the discovery of covert awareness and the establishment of limited communication using … type of research shakes their faith in diagnostic accuracy, to clarify the legal rules relating to advance directives in this context, and to address the manner in which public messaging might help to alleviate any indirect impact on confidence in the organ donation system.

Refractory Hypoxemia and Use of Rescue Strategies. A U.S. National Survey of Adult Intensivists.

RATIONALE: The management of severe and refractory hypoxemia in critically ill adult patients is practice based. Variability across individu … variability was partly explained by providers' expertise with particular rescue strategies (77.7%), advance directives (70.1%), the training of allied health staff (62.3%), and institutional availability (53.8%). CONCLUSIONS: U.S. adult critical care physicians predominantly employ lung-protective ventilation for severe hypoxemia. A wide variation in other rescue s …

Treatment goal limitations in acute hospital care : A retrospective empirical ethics study of documentation and recorded reasoning.

BACKGROUND: Treatment goal limitations, such as "do not resuscitate", "do not escalate" and "comfort terminal care" are frequent in acute hospital care for patients with advanced illness, multimorbidity or impending death. Their ethical defensibility depends on transparent documentation of medical indications, proportionality and, where relevant, patient will. OBJECTIVE: To examine whether documentation made medical rationale, patient will, advance directives, relatives' involvement or ethics consultation visible. METHODS: We retrospectively analyzed documentation in an Austrian internal med…

Rapid Neurocognitive Deterioration and Mortality in a Healthcare Professional With Spongiform Encephalopathy: Implications for Neurologic and End-of-Life Care.

Spongiform encephalopathy (SE) is a rare prion disorder characterized by progressive cognitive dysfunction and mortality. Affected patients can observe a wide variety of neurological symptoms, such as myoclonus, dementia, cerebellar signs, and others. We present a case of laboratory-confirmed SE in an otherwise healthy 57-year-old medical professional who initially presented with nonspecific and unique "head in a fish-bowl" dissociation and cognitive decline. No social risk factors were ever identified other than his healthcare career, but subsequent neuroimaging, serology, and lumbar puncture

[IC triage in patients with an acutely worsening condition; challenges, considerations and decisions].

Acute intensive care (IC) triage involves a challenging decision-making process. Physicians are required to make life or death decisions about an unfamiliar patient within a short time frame. An 84-year-old female was admitted to the stroke unit following an extensive cerebral infarction. The intensive care unit (ICU) physician was consulted because of a suspected severe abdominal sepsis even though ICU treatment had never previously been discussed. A 77-year-old female with a previous history of myocardial infarction and severe COPD developed acute respiratory failure on the ward, and was adm

[Advance refusal of treatment in case of loss of autonomy due to persistent brain disease].

A short overview is given of the current debate on ethics and legal clarification of the range and binding force of so-called living wills demanding interruption of treatment in case of loss of autonomy due to persistent or progressive brain disease. Using the examples of dementia and persistent vegetative states - conditions with growing significance for psychiatrists - the binding force of living wills is examined for cases in which the irreversibility and extent of consciousness loss cannot be predicted with certainty. The range of living wills' authority appears also unclear. Legal proposa

Advance care planning for patients with advanced neurology diseases.

BACKGROUND: Advanced neurology diseases including motor neuron disease (MND) are usually progressive life-limiting illness and could be devastating for patients, families and caregivers. Although medical technologies, such as enteral feeding and non-invasive ventilation, may prolong life expectancy of the patients, their utilization prompts important ethical questions in regard to their quality of life (QoL). Little attention had been paid on how ACP practice would practically help with patients suffering from different neurology diseases. We are unaware of any published studies on ACP practic

APOE Genotype Disclosure Influences Decisions About Future Planning but not Adoption of Healthy Lifestyle Changes in Cognitively Unimpaired Individuals.

BACKGROUND: Studies have shown apolipoprotein E (APOE) genotype disclosure to be safe and well-tolerated in cognitively unimpaired (CU) older adults. This study aimed to examine the effect of the disclosure process on decisions about future directives and health behaviors in community-dwelling CU older adults from the Butler Alzheimer's Prevention Registry (BAPR). METHODS: CU APOE E4 non-carriers (n = 106) and carriers (n = 80) aged 58-78 completed in-person psychological readiness screening to undergo APOE disclosure. Follow-up assessments were completed online 3 days, 6 weeks, and 6 months

A medical ethics assessment of the case of Terri Schiavo.

The social, legal, and political discussion about the decision to stop feeding and hydration for Terri Schiavo lacked a medical ethics assessment. The authors used the principles of medical indications, quality of life, patient preference, and contextual features as a guide to medical decision-making in this case. Their conclusions include the following: (a) the use of a feeding tube inserted directly in to the stomach constituted artificial treatment; (b) the treatment prolonged biological life but did not lead to a cure and did not restore health; (c) quality of life was absent for the patie
