The role of neuroplasticity and cognitive reserve in aging with HIV: recommendations for cognitive protection and rehabilitation.

By and large, the immune systems of people infected with HIV are being protected and maintained by advances in highly active antiretroviral therapy; as such, this is extending the lives of people into old age. Unfortunately, for many living with this disease, HIV is associated with neuroinflammation, co-morbidities, and accelerated aging which can compromise brain function, resulting in cognitive deficits. The purpose of this article is to highlight how to interpret these deficits within the framework of neuroplasticity and cognitive reserve for this clinical population. We suggest several rec

Accuracy of the Delirium Observational Screening Scale (DOS) as a screening tool for delirium in patients with advanced cancer.

BACKGROUND: The Delirium Observation Screening Scale (DOS) was developed to facilitate early recognition of delirium by nurses during routine clinical care. It has shown good validity in a variety of patient populations, but has not yet been validated in hospitalized patients with advanced cancer, although the DOS is commonly used in this setting in daily practice. The aim of this study was to evaluate the accuracy of the DOS in hospitalized patients with advanced cancer using the revised version of the Delirium Rating Scale (DRS-R- 98) as the gold standard. METHODS: Patients with advanced ca

Podcast on Cross-speciality Perspectives on Practical Management of Atopic Dermatitis-Associated Ocular Surface Diseases.

There is a growing awareness among dermatology providers of ocular comorbidities in patients with the chronic inflammatory skin disease atopic dermatitis (AD). For example, the prevalence of ocular surface diseases (OSD) such as conjunctivitis is higher in patients with AD than in the general population, and the use of some AD treatments may be associated with OSD. In a recent review published in the Journal of the American Academy of Dermatology, dermatologists and ophthalmologists provided an overview of the different types, etiology, pathophysiology, and practical management of OSD associat

Interstage Home Monitoring for Infants With Single Ventricle Heart Disease: Education and Management: A Scientific Statement From the American Heart Association.

This scientific statement summarizes the current state of knowledge related to interstage home monitoring for infants with shunt-dependent single ventricle heart disease. Historically, the interstage period has been defined as the time of discharge from the initial palliative procedure to the time of second stage palliation. High mortality rates during the interstage period led to the implementation of in-home surveillance strategies to detect physiologic changes that may precede hemodynamic decompensation in interstage infants with single ventricle heart disease. Adoption of interstage home m

Nurse-led intensive care follow-up interventions in low- and middle-income countries: A scoping review.

INTRODUCTION: Post-intensive care syndrome affects more than half of intensive care unit (ICU) survivors, resulting in long-term physical, psychological, and cognitive impairments. Structured post-ICU recovery programs, including nurse-led follow-up, have improved patient outcomes in high-income countries. However, little is known about such models in low- and middle-income countries (LMICs), where post-ICU care is often lacking. OBJECTIVE: The aim of this scoping review was to identify and map the existing nurse-led follow-up care interventions for ICU survivors in LMICs to inform future dev

Venous thromboembolism prophylaxis in adults hospitalised for psychiatric illness: an evidence-based clinical practice guideline developed using GRADE.

INTRODUCTION: Venous thromboembolism (VTE) is the leading cause of preventable hospital deaths. Adults hospitalised with psychiatric illness vary in their risk of VTE, and therefore in their likelihood of benefiting from thromboprophylaxis. There is a paucity of evidence-based practice guidelines addressing VTE prophylaxis for this population despite recognition of additional VTE risk factors in this population. AIM: To develop an evidence-based guideline on VTE prophylaxis for patients hospitalised with psychiatric illness using Grading of Recommendations, Assessment, Development and Evaluat

Substitution of doctors by nurses in primary care.

BACKGROUND: Demand for primary care services has increased in developed countries due to population ageing, rising patient expectations, and reforms that shift care from hospitals to the community. At the same time, the supply of physicians is constrained and there is increasing pressure to contain costs. Shifting care from physicians to nurses is one possible response to these challenges. The expectation is that nurse-doctor substitution will reduce cost and physician workload while maintaining quality of care. OBJECTIVES: Our aim was to evaluate the impact of doctor-nurse substitution in pr

Tumor Treating Fields: An Innovative Therapy for Glioblastoma and Other Solid Tumors.

Tumor Treating Fields (TTFields) therapy specifically disrupts cellular processes necessary for cancer cell viability and tumor progression through the delivery of electric fields from a portable medical device. Patients with solid tumors such as glioblastoma experience significantly improved overall survival when receiving TTFields therapy concomitant with other standard-of-care therapies. TTFields therapy is also well tolerated and allows patients to maintain their quality of life. It is currently approved for the treatment of newly diagnosed and recurrent glioblastoma, pleural mesothelioma,

A holistic approach to managing a patient with heart failure.

Despite varied and complex therapeutic strategies for managing patients with heart failure, the prognosis may remain poor in certain groups. Recognition that patients with heart failure frequently require input from many care groups formed the basis of The British Society of Heart Failure Annual Autumn Meeting in London (UK), in November 2012, entitled: 'Heart failure: a multidisciplinary approach'. Experts in cardiology, cardiac surgery, general practice, care of the elderly, palliative care and cardiac imaging shared their knowledge and expertise. The 2-day symposium was attended by over 500

Nurses' functional status assessments using the International Classification of Functioning, Disability, and Health across hospital types in Japan: A cross-sectional study.

BACKGROUND: In Japan, patients receiving treatment in advanced acute care hospitals often cannot return home immediately and require ongoing hospitalization. Sequential support is crucial to facilitate long-term recovery and community reintegration. Therefore, functional assessments should extend beyond physical function to include daily activities and participation. However, how nurses across different hospital types in Japan evaluate patients' functional status using a standardized framework remains unclear. OBJECTIVE: This study examined how nurses in hospitals with varying care functions
