Simultaneous lateral bone augmentation and implant placement using a particulated synthetic bone substitute around chronic peri-implant dehiscence defects in dogs.

AIM: To determine the histological outcomes when using a highly porous synthetic bone substitute and a cross-linked collagen membrane for lateral bone augmentation of peri-implant dehiscence defects. METHODS: In eight dogs, three treatment groups were randomly allocated at each peri-implant dehiscence defect (mean height × depth = 3 × 1 mm) as follows: (i) synthetic bone substitute covered by a cross-linked collagen membrane (test group), (ii) deproteinized bovine bone mineral covered by a natural collagen membrane (positive control), and (iii) no treatment (negative control). Two healing per

Comparison of intraoral harvest sites in the edentulous versus dentate specimen.

OBJECTIVE: The aim of this study was to compare the edentulous vs dentate specimen intraoral bone harvest sites. We wished to identify if there were any sites that yielded similar quantities of bone regardless of the status of the dentition. STUDY DESIGN: There were 59 cadavers in the study. Three continuous outcomes (area, thickness, and volume) were measured for each cadaver at 4 sites (zygoma, symphysis, ramus, and coronoid). RESULTS: Status of the dentition was not a factor in the quantity of harvested bone in regard to surface area and volume. The only difference noted between the denta

Application of the "All-on-Four" concept and guided surgery in a mandible treated with a free vascularized fibula flap.

In this article, we describe the first case in the literature in which 3D computer-assisted treatment planning and guided surgery enabled a patient affected by extreme paraphysiologic mandibular bone atrophy to be treated with a free vascularized fibula flap and, after a period of healing, the flapless installation of 4 immediately loaded dental implants. The computer-fabricated surgical guide allowed placement of the implants according to the "All-on-Four" concept in a proper spatial preplanned position and by tilting 1 implant, avoiding an area of bone discontinuity. Additionally, this techn

Use of the coronoid process as a donor site for sinus augmentation: a case report.

In a sinus lift procedure, bone grafting is usually performed concomitantly. The mandibular coronoid process is an advantageous donor site for autogenous bone grafts. A clinical case of sinus bone grafting using the coronoid process of the mandible is presented here, along with the surgical technique to accomplish this procedure. A successful outcome was obtained in the patient, who experienced no functional problems.

Treatment planning of implants in the aesthetic zone.

Aesthetic restoration of anterior teeth with implant supported restorations is one of the most difficult procedures to execute. Bone resorption following anterior tooth extraction often compromises gingival tissue levels for the implant restoration. In the last 10 years the focus has shifted from osseointegration, to creation of an implant borne restoration which is in harmony with the surrounding hard and soft tissue. Complete reconstruction of tooth and gingival related aesthetics remains the primary objective and in some instances can be very difficult to achieve.

A method of sealing perforated sinus membrane and histologic finding of bone substitutes: a case report.

To augment the atrophic posterior maxilla, a sinus bone graft has been widely used for sinus floor augmentation. Various bone substitutes have been developed and grafted in the maxillary sinus with and without membranes perforation, although autogenous bone is recommended as a gold standard of grafting materials. Membrane perforation is the most common complication associated with sinus bone graft. To repair a perforation, various methods have been developed. This case report is focused on histologic findings of 1 bovine hydroxyapatite (Bio-Oss; Geistlich Pharma AG, Wolhusen, Switzerland) and

Nonvascularized autogenous bone grafts for craniofacial skeletal augmentation and replacement.

Autogenous nonvascularized bone grafts play an important role in the reconstruction of complex craniomaxillofacial defects. Experimental animal data have demonstrated that grafts from membranous bone donor sites tend to undergo less resorption than grafts from endochondral donor sites, probably because of the different bony architecture of each of these types of grafts. Of all the potential donor sites, the harvest of bone graft from the calvarium is associated with the least overall morbidity. Surgeons should be aware of the biologic basis for the successful application of free autogenous bon

[Clinical evaluation of natural coral implantation in osseous periodontal defects. Results after one year].

17 intrabony periodontal defects on 14 patients were surgically treated by a alloplastic graft using micro-granules of natural coral. Criteria of evaluation were standardized probing and reproducible radiographs. The measures, made at day 0 and one year post-implantation showed a reduction in probing pocket depth (4.38 mm +/- 0.43) assessed more through a gain of clinical attachment (3.38 mm +/-0.30) than an increase of the gingival recession (1.00 mm +/- 0.20). These results are statistically significant.

Evaluation of the Stress Levels of Different Implant Treatment Options in Extremely Atrophic Maxillae by Finite Element Analysis Method.

BACKGROUND: Bone resorption progresses in the alveolar ridge after tooth loss. In extremely athropic maxillae, treatment options include quad-zygoma implants, iliac bone augmentation with intraosseous implants, (the All-on-4 concept), Group-SUB periosteal implant. In this study, extremely atrophic maxilla was defined as an edentulous ridge with 3 mm crest thickness, 6 mm nasal base-crest distance, and 5 mm sinus floor-crest distance. AIM: This study aimed to compare stress distribution of these protocols using finite element analysis. METHODS: In this study, analyses were performed using the

Horizontal vascular-stalked split osteotomy (HVSO): a bone augmentation technique for the atrophic jaw- a retrospective cohort study in 29 patients.

BACKGROUND: Implant therapy in the advanced atrophic jaw remains challenging in oral and maxillofacial surgery. Hence, a plethora of different augmentation procedures to increase bone volume in the maxilla and mandible have been published. Horizontal vascular-stalked split osteotomy (HVSO) represents a safe and effective approach for the three-dimensional jaw augmentation since it combines maximum vascularization through lingual or palatinal periosteal stalking with reduced grafting morbidity. OBJECTIVE: To analyze the efficacy of HVSO for implantation therapy in atrophic jaws by assessing ve
