Detection of Local Anesthetic Ingredients Including Lidocaine, Tetracaine, and Prilocaine in Illegally Distributed Drugs Through Gas Chromatography-Time-of-Flight Mass Spectrometry and Gas Chromatography-Triple Quadrupole Mass Spectrometry.

Local anesthetic compounds may be mixed with counterfeit products and distributed as legitimate pharmaceuticals. In this study, gas chromatography-time-of-flight mass spectrometry (GC-TOF MS) and gas chromatography-triple quadrupole mass spectrometry (GC-QQQ MS) methods are used simultaneously to determine 18 local anesthetics and identify illegal products. First, GC-TOF MS is performed to determine the fragmentation patterns and structures of the 18 compounds and qualitatively screen the samples. GC-QQQ MS is used for quantitative analysis and thoroughly validated for specificity, linearity,

The thermodynamics of general and local anesthesia.

General anesthetics are known to cause depression of the freezing point of transitions in biomembranes. This is a consequence of ideal mixing of the anesthetic drugs in the membrane fluid phase and exclusion from the solid phase. Such a generic law provides physical justification of the famous Meyer-Overton rule. We show here that general anesthetics, barbiturates, and local anesthetics all display the same effect on melting transitions. Their effect is reversed by hydrostatic pressure. Thus, the thermodynamic behavior of local anesthetics is very similar to that of general anesthetics. We pre

Spinal opioids and the treatment of the obstetric patient with cardiac disease.

The administration of narcotics into the spinal or epidural space has proven to be efficacious in the relief of mild to moderate pain. Unlike local anesthetics, intraspinal opioids are not associated with a decrease in systemic vascular resistance. Recent research has shown that opioids alone, or combined with local anesthetics, offer distinct advantages over the use of local anesthetics, alone when providing analgesia for the pregnant patient with cardiac disease.

A Comprehensive Review of the Use of Alpha 2 Agonists in Spinal Anesthetics.

BACKGROUND: Spinal Anesthesia was the first regional anesthetic technique to be performed. It was performed by Dr. August Bier, known for the Bier block, and his colleagues on August 16, 1898. Dr. Bier opted for, what he referred to at the time as "cocainization of the spinal cord" by introducing 15 mg of cocaine intrathecally prior to the operation. The surgery was largely uneventful and painless. The patient only experienced some vomiting and a headache postoperatively. Dr. Bier's use of neuraxial anesthesia aimed to directly inject local anesthetics in and around the central nervous system

Comparative effectiveness of nerve block strategies for preventing postherpetic neuralgia in thoracic herpes zoster: a network meta-analysis.

BACKGROUND: Shingles is a common global disease, with the chest region being one of its most frequently affected areas. Postherpetic neuralgia (PHN) is one of the most common and debilitating complications of this disease, characterized by high incidence, prolonged duration, and treatment resistance, severely affecting patients' daily life and quality of life. Currently, research on the prevention of PHN remains limited. Nerve block, as a promising intervention, has been widely applied in clinical pain management. However, there is still no consensus on the efficacy and safety of different ner

Permanent neurologic symptoms after prolonged epidural analgesia.

OBJECTIVE: Although transient neurologic symptoms may occur after neuraxial anesthetics with lidocaine, permanent neurologic symptoms (PNS) are unusual after uncomplicated continuous epidural analgesia (CEA). This report demonstrates that PNS may follow CEA uncomplicated by paresthesias, dural puncture, spinal hemorrhage, or intravascular local anesthetic injection; and performed with bupivacaine alone administered at high dosages with vasoconstrictors over a prolonged time period. CASE REPORT: An obstetrical patient developed fixed left L5-S1 sensorimotor radiculopathy following L5-S1 level

Efficacy of low-dose hypobaric anesthetics in spinal anesthesia for cesarean delivery: systematic review and meta-analysis.

OBJECTIVE: Many studies have compared the anesthetic effects of different doses of hypobaric local anesthetics (without glucose) in spinal anesthesia for cesarean sections. Therefore, a meta-analysis of these trials is warranted. METHODS: Systematic review and meta-analysis of eligible randomized controlled trials comparing the efficacy of low-dose spinal hypobaric anesthetics (Bupivacaine < 10 mg, levobupivacaine < 10 mg, ropivacaine < 15 mg) with conventional dose (Bupivacaine/Levobupivacaine ≥ 10 mg, ropivacaine ≥ 15 mg) in elective and subemergency Cesarean sections in data sources (PubMe

Locoregional anesthesia in patients with Brugada syndrome. A retrospective database analysis.

BACKGROUND: The use of local anesthetics (LA) in individuals with Brugada syndrome (BrS) remains a subject of debate due to the lack of large-scale studies confirming their potential risks. This study primarily aimed to evaluate the incidence of new malignant arrhythmias or defibrillation events in patients diagnosed with BrS during the perioperative period, following the administration of local anesthetics, and within 30 days postoperatively. The secondary objective was to analyze the occurrence of adverse effects during hospitalization, as well as 30-day readmission and mortality rates. MET

Lumbar epidural blocks: a case report of a life-threatening complication.

A case of life-threatening complication resulting from a lumbar epidural block is presented. A 70-year-old woman with spinal stenosis developed cardiac and respiratory arrest 5 minutes after receiving a lumbar epidural block containing 80 mg of triamcinolone acetonide and 6mL of 1.5% lidocaine. The patient received cardiopulmonary resuscitation and recovered without any sequelae. It is suggested that this complication was caused by subdural or intravascular injection of local anesthetics. It might be preventable by injecting a test dose of local anesthetics before injecting a full dose of loca

Pain management strategies for lumbar puncture in infants ≤6 months: A systematic review and meta-analysis.

BACKGROUND: Lumbar puncture (LP) is a painful procedure performed in infants. Both national and international organizations recommend the use of local anesthetics for LP pain management. However, recommendations are not based on systematic review of the literature. The primary objective of this systematic review and meta-analysis was to synthesize the literature on local anesthetics and opioids for pain management in infants undergoing LP. METHODS: Databases were searched for randomized trials from inception to March 2024. The literature search was updated in June 2025 and no additional eligi

Comparison of Clinical Efficacy of Epidural Injection With or Without Steroid in Lumbosacral Disc Herniation: A Systematic Review and Meta-analysis.

BACKGROUND: Epidural injection is performed for treatment of back and radicular pain in patients with lumbosacral disc herniation (LDH). Steroids are usually administered to effectively remove inflammatory mediators, and local anesthetics or saline also contribute to pain reduction by washing out chemical mediators or blocking the nociceptor activity. Controversy exists regarding whether steroids produce superior clinical effects compared with local anesthetics or saline. OBJECTIVES: This study investigated whether epidural injection of steroids produces better clinical effects than local ane

New therapeutic uses for an alpha2 adrenergic receptor agonist--dexmedetomidine in pain management.

Dexmedetomidine was initially approved for clinical use as a sedative. Its development in pain management has been limited. Dexmedetomidine has analgesic effects and analgesic-sparing properties, especially for patients with obstructed airways. Mixing dexmedetomidine with local anesthetics is a promising new avenue to enhance local anesthetics' effectiveness. Peripheral, spinal and supraspinal α(2A)-ARs are responsible for the analgesic function of dexmedetomidine. Animal studies have shown that antinociceptive synergism results from co-application of dexmedetomidine and opioids or local anest

Methemoglobinemia presenting in a circumcised baby following application of prilocaine: a case report.

INTRODUCTION: Local anesthesia with prilocaine has become a routine part of ambulatory circumcision procedures. Methemoglobinemia is a rare but potentially lethal complication of local anesthetics. CASE PRESENTATION: We report the case of a 40-day-old Turkish boy who presented with cyanosis after receiving local anesthesia with prilocaine. His methemoglobin level revealed severe methemoglobinemia (methemoglobin = 44%). His cyanosis resolved after intravenous administration of methylene blue. CONCLUSION: Although the association between prilocaine use and methemoglobinemia has generally restr

Methemoglobinemia after topical anesthesia with lidocaine and benzocaine for a difficult intubation.

Methemoglobinemia is an uncommon cause of cyanosis; however, rapid diagnosis is critical to avoid potentially fatal consequences. Several local anesthetics can precipitate methemoglobinemia in susceptible patients. This case report of acute methemoglobinemia occurred during fiberoptic intubation in an awake patient without a past medical history of adverse reactions to local anesthetics.
