From a pool of 624 patients with idiopathic chronic urticaria and angioedema, 90 patients had evidence of associated thyroid autoimmunity (TA). Since the number expected by chance alone is 37, given that less than 6% of normal subjects have TA, the association is significant (p less than 0.01; chi-square test). Age and sex distribution was typical of patients with TA. Clinically, most patients suffered relentless and severe urticaria and/or angioedema. With the exception of thyroid function and thyroid antibody tests, other laboratory tests were not rewarding. In most cases, treatment with 1 t…

In the present study, we examined 102 patients with chronic urticaria and angioedema. The incidence of immune complexes (CIC)-mediated chronic urticaria with complement activation (C3b+) was 11.7% (12/102 patients). The 12 patients with CIC and C3b was divided in three diagnostic groups: with drug adverse reactions; with systemic disorders; without apparent associated pathologies. On the basis of data obtained it is remarkable the necessity of a careful etiologic diagnosis in presence of CIC mediated-chronic urticaria particularly when it is associated with arthralgies and/or elevated erythroc…

Hereditary angioedema (HAE) is a genetic disorder characterised by recurrent cutaneous or submucosal oedema. Most cases are caused by pathogenic SERPING1 variants (types I and II), while a smaller subset presents with normal C1-inhibitor (C1-INH) levels and function (HAE-nC1INH). Despite its potentially life-threatening attacks, HAE remains under-diagnosed owing to inconsistent screening practices, low clinical awareness, poor patient acceptance, and resource limitations, resulting in diagnostic delays and preventable morbidity. Although international guidelines recommend family screening to i…

Chronic urticaria is a common mast-cell-driven inflammatory disorder characterized by recurrent wheals, angioedema, or both. Angioedema is associated with higher disease burden, impaired quality of life, and greater difficulty in clinical assessment, yet the overall research structure of chronic urticaria in the context of angioedema has not been systematically mapped. Publications were retrieved from the Web of Science Core Collection on July 14, 2026. English-language Articles and Reviews published between 1947 and 2026 were included after independent screening by two researchers, with disag…

Intestinal angioedema is a rare but severe manifestation of hypersensitivity that mimics acute abdomen. This first reported case of promethazine-induced intestinal angioedema underscores the importance of recognizing drug-induced etiologies to prevent unnecessary surgery and enable conservative management.

BACKGROUND: Patients with hereditary angioedema (HAE) exhibit emotional distress and a deteriorated quality of life. Emotional distress is associated with impaired emotional regulation, including alexithymia, which is characterized by difficulties in recognizing and expressing emotions. Our study assessed the presence of alexithymia in patients with HAE to determine whether patients with this disease experience difficulties and specific learning in emotion regulation. Thirty-nine adult patients with HAE answered a self-report questionnaire measuring: 1) alexithymia (Toronto Alexithymia Scale-2…

BACKGROUND: Hereditary angioedema (HAE) is a rare genetic disease manifesting as recurrent painful, burdensome, and potentially life-threatening swelling attacks. This noninterventional, cross-sectional, web-based survey of adult (aged ≥ 18 years) participants with HAE from Argentina, Brazil, Colombia, Croatia, Denmark, Germany, Hungary, Ireland, Norway, Poland, Portugal, Romania, and Sweden sought to deepen the understanding of HAE burden. Individuals were eligible if they had a self-reported physician diagnosis of HAE, ≥ 1 HAE attack or prodromal symptom within the last year, and received HA…

INTRODUCTION: Hereditary angioedema (HAE) is a rare genetic disorder characterized by recurrent swelling caused by dysregulation of the kallikrein-kinin pathway. Although current therapies effectively reduce attack frequency, treatment remains lifelong. Lonvoguran ziclumeran (Lonvo-z; NTLA-2002) is the first systemically administered in vivo CRISPR/Cas9 gene-editing therapy designed to provide durable suppression of plasma kallikrein through permanent disruption of the KLKB1 gene.
AREAS COVERED: This review summarizes the pathophysiology and current management of HAE, the development of Lonvo…

BACKGROUND: Angiotensin-converting enzyme inhibitor-induced angioedema (AE-ACEI) is a life-threatening adverse event; globally, it is the most common cause of emergency presentations with angioedema. Several genome-wide association studies (GWASs) have found genomic associations with AE-ACEI. However, despite African Americans having a 5-fold increased risk of AE-ACEI, there are no published GWASs from Africa.
OBJECTIVE: The aim of this study was to conduct a GWAS of AE-ACEI in a South African population and perform a meta-analysis with an African American and European American population.

Hereditary angioedema (HAE) is a rare genetic condition caused by deficient or dysfunctional C1 inhibitor protein (C1INH) resulting in episodic angioedema of the skin, upper airway, and gastrointestinal tract. HAE most often presents within the first two decades of life and may cause severe abdominal pain, nausea, diarrhea, and emesis, making it an important diagnosis for pediatric gastroenterologists to consider. Here, we report the case of an 11-year-old boy with a history of Crohn's disease who presented with recurrent episodes of acute epigastric and periumbilical pain despite evidence of …

OBJECTIVES: Individuals with angioedema (AE) are at high risk for airway compromise and often require endotracheal intubation. Patient factors predisposing one to airway compromise are not well described. The objective of this study is to examine whether substance use disorder (SUD) in patients with AE is associated with need for airway intervention.
METHODS: This population-based retrospective cohort study compared AE patients with SUD versus propensity-matched control groups. Outcomes were hospitalization, intubation, and tracheotomy. Using the TriNetX National Database, this study included…

INTRODUCTION: Angioedema, a swelling of the subcutaneous or submucosal layers of the skin or gastrointestinal tract, is a potential complication to thrombolytic therapy in the treatment of acute ischemic strokes. In these cases, angioedema develops due to increased levels of bradykinin as a result of the activation of the fibrinolytic pathway and contact activation system. Angioedema can involve the tongue, larynx, and vocal cords, leading to occlusion of the airway and death due to asphyxiation. It is vital for the emergency physician to know that this complication can occur to ensure appropr…

Angioedema is a potentially life-threatening condition that can have an allergic origin, usually mediated by histamine or a non-allergic origin, mediated by bradykinin. The distinction between these origins may present a clinical challenge at first approach, especially in cases that appear as an emergency and the outcome is time dependent. The authors describe a rare case of bradykinin angioedema associated with airway obstruction and discuss the right approach and therapeutic options. A 46-year-old patient under ACE inhibitor, renin-angiotensin-aldosterone blocker and beta blocker presented w…

Hereditary angioedema (HAE) is an autosomal dominant disorder characterized by recurrent episodes of swelling of the skin, larynx, gastrointestinal tract, genitals, and extremities that can be disruptive to patient quality of life. Dysregulation of plasma kallikrein activity leads to increased production and accumulation of bradykinin in HAE and causes attacks of angioedema. Plasma kallikrein is a serine protease essential for the formation of bradykinin. Berotralstat is a potent, highly selective, orally bioavailable small-molecule plasma kallikrein inhibitor that has been approved to prevent…

Background: The coronavirus disease 2019 (COVID-19) pandemic has greatly affected health-care provision across the globe. Management of chronic ailments has become challenging because of the strained health-care resources and social distancing measures that prevent on-site clinical visits and treatments. Hereditary angioedema (HAE) is a debilitating, chronic disease characterized by unpredictable swelling attacks in various parts of the body. Controlling HAE symptoms often requires long-term prophylactic medication use and regular medical care; however, limited scientific information has been …

BACKGROUND: Hereditary angioedema (HAE) is caused by low levels of or defects in C1 inhibitor. Although disease activity may be modified by prophylaxis, emergency treatment, treatment for comorbidities, and oral contraceptives, the extent of their use is unclear.
OBJECTIVE: To investigate trends in the use of disease-specific and interfering drugs in patients with HAE compared with the general population in Sweden.
METHODS: In a nationwide, longitudinal study, 239 patients with HAE and 2 383 controls were compared with the Prescribed Drug Register (2005-2019). These data reflect rates of dis…

Episodic angioedema with eosinophilia (EAE) (Gleich's syndrome) is a rare disease characterized by hypereosinophilia (up to 95 × 109 cells/L), recurrent episodes of angioedema, urticaria, weight gain, and fever, that occur at periodical intervals (usually every 3-4 weeks). The exact etiology of EAE is still unclear, but both eosinophils and abnormalities of cytokines homeostasis seem to play a pivotal role in the pathogenesis of the disease. In particular, the cyclic elevation of serum interleukin-5 before the increase in eosinophil count has been reported. Herein, we performed a broad literat…
