Comparison of visual acuity and complications between primary IOL implantation and aphakia in patients with congenital cataract younger than 2 years: a meta-analysis.

To analyze the visual acuity and complications between primary intraocular lens (IOL) implantation and contact lens wearing, this literature search was performed with data on patients with congenital cataract younger than 2 years published in March 2019. Seven identified studies enrolling 675 eyes were selected for analysis. Patients with primary IOL implantation owned better visual acuity than those with aphakia who wore the contact lens (weighted mean difference = 0.161; 95% CI, 0.108-0.214). For visual axis opacification (VAO), primary IOL implantation increases the incidence of VAO compare

Modified surgical technique for improving donor adherence in DSAEK in the aphakic vitrectomized eye.

We describe a modified technique for performing Descemet stripping automated endothelial keratoplasty (DSAEK) in a patient with aphakia and vitrectomized eye. A 72-year-old woman presented with corneal decompensation after a complicated cataract surgery and a subsequent vitreoretinal procedure. Before commencing DSAEK, a pars plana infusion cannula was inserted 3 mm from the limbus. The infusion fluid in the vitrectomized posterior segment helped circumvent potential complications, such as recurrent globe collapse, posterior dislocation of the donor disc, and loss of air tamponade. Increasing

[Artificial iris].

The flexible iris prosthesis according to Koch with a customized iris design serves for anterior segment reconstruction in patients with partial or complete aniridia. It is designed for implantation in the ciliary sulcus and is recommended only for pseudophakic or aphakic eyes. The multilayered implant consists of a core of a silicone matrix with incorporated color pigment coated with another layer of medical grade silicone. As a design variant the implant is available with or without an additionally embedded tissue layer. Without tissue the implant is more flexible, can be easily folded and p

Sutured intraocular lens placement in aphakic post-vitrectomy eyes via small-incision surgery.

A technique for placing a sutured intraocular lens (IOL) in post-vitrectomy eyes is described. A foldable acrylic IOL (Akreos) is sutured to the sclera with 4-point fixation via a standard cataract incision, avoiding the large incision needed for nonfoldable IOLs, which would represent extra risk in eyes with no internal support. Postoperative results in 2 cases were significant for quick recovery and minimal complications.

Injection of an intraocular lens in an eye without capsular support.

We describe a technique that can be used to facilitate posterior chamber intraocular lens (IOL) placement with iris fixation in cases with compromised capsular support. This procedure allows injection of an IOL through an unenlarged clear corneal incision. A safety net suture is temporarily fixed in the posterior chamber to act as a surrogate capsule. This suture supports the IOL while the surgeon injects it and secures it to the iris. The addition of this single step facilitates the remainder of the procedure and potentially makes it safer.

Long-term reversibility of epikeratophakia.

PURPOSE: To assess the long-term reversibility of epikeratophakia. METHODS: Three human epikeratophakia lenticules (from 3 patients) were removed 7-14 years after refractive keratoplasty for aphakia (n = 1) and myopia (n = 2). Reasons for removal were irregular astigmatism (n = 1), opacities in the graft and host cornea, and progressive myopia (n = 2). After removal, 2 patients underwent cataract extraction and 1 underwent secondary implantation of AC-IOL. Visual acuity, refraction, keratometry, and corneal topography were assessed before and after removal of the lenticule, as well as after t

Use of the aphakic refraction in intraocular lens (IOL) power calculations for secondary IOLs in pediatric patients.

PURPOSE: Calculating the power of a secondary intraocular lens (IOL) requires axial lengths and keratometry measurements. In children, these measurements often need to be done under either sedation at the time of surgery or additional sedation. We devised a method of calculating IOL power using only the aphakic refraction and a standard keratometry value. PATIENTS AND METHODS: This retrospective study included children undergoing a secondary IOL procedure who had IOL calculations based on either a measured axial length and a measured keratometry value (with sedation if needed) or a calculated

Aberrations of the human eye in visible and near infrared illumination.

PURPOSE: In most current aberrometers, near infrared light is used to measure ocular aberrations, whereas in some applications, optical aberration data in the visible range are required. We compared optical aberration measurements using infrared (787 nm) and visible light (543 nm) in a heterogeneous group of subjects to assess whether aberrations are similar in both wavelengths and to estimate experimentally the ocular chromatic focus shift. METHODS: Ocular aberrations were measured in near infrared and visible light using two different laboratory-developed systems: laser ray tracing (LRT) an

LASIK after epikeratophakia.

PURPOSE: To present a case of successful laser in situ keratomileusis (LASIK) performed on a patient with residual hyperopic astigmatism and symptomatic anisometropia after previous epikeratoplasty (epikeratophakia). METHODS: LASIK was used successfully to treat hyperopic astigmatism after regression of epikeratophakia performed 16 years earlier. RESULTS: Visual acuity improved to 20/30 without correction, and best-corrected visual acuity was 20/25 at the 3-month postoperative visit. CONCLUSIONS: LASIK can be used successfully as an alternative treatment to correct residual refractive error

Longitudinal study on visual outcome and spectacle use after intracapsular cataract extraction in northern India.

PURPOSE: More than 3 million cataract extractions are undertaken in India annually. Almost 60% of these operations are intracapsular Cataract Extractions. The subsequent optical correction is provided by aphakic spectacles. The aim of this study is to assess visual outcome and perceived benefits of post-operative use of aphakic spectacles. METHODS: One hundred and sixty-seven persons who had undergone intracapsular cataract extraction and had been given best corrected aphakic spectacles were evaluated one year following prescription of the best corrected aphakic spectacles. Out of these, 82.6

The critical period for surgical treatment of dense congenital unilateral cataract.

PURPOSE: Early treatment of dense congenital unilateral cataract is associated with better acuity outcomes. It is unclear whether there is a gradual worsening of prognosis with delay of treatment from the time of birth (linear model) or whether there exists an early window of time during which treatment is maximally effective, followed by declining success (bilinear model). The aim of the current study was to determine which model better describes the response to treatment. METHODS: A maximum likelihood procedure that permits statistical comparison between linear and bilinear models was appli

[Effects of the duration of cataract extraction and eyeglass wear in patients with aphakia on the development of changes in the macular area].

Forty-eight patients with aphakia were examined to elucidate the effect of cataract extraction duration on subsequent development of Irwin-Hass syndrome. A total of 229 patients were examined 6 months and more after cataract extraction to find out whether sun light could induce development of dystrophic changes in the central retina in aphakia. The results indicate that operation duration of more than an hour increased the risk of subsequent development of Irwin-Hass syndrome. Aphakia patients wearing no eyeglasses or contact lenses or intraocular lenses may develop maculodystrophies, probably

Ultraviolet sensitivity of three cone types in the aphakic observer determined by chromatic adaptation.

Sensitivities at 5.5 deg off-fovea from the aphakic observer superimposed on orange, purple and blue fields were obtained to estimate short-, middle- and long-wavelength cone spectra respectively. The middle-wavelength mechanism had a visible wavelength maximum resembling a nomogram plus an UV sensitivity fitting a cis-peak. The short (and, to a lesser extent, the long) wavelength cone sensitivities are higher in the UV than expected.
