Seven cases with the syndrome of thalassemia minor and pauciarticular, non-erosive, seronegative arthritis are described. Other known causes of seronegative arthritis had been excluded. There was a predilection for middlesized joints such as ankles, wrists and elbows. Usually the synovitis was asymmetrical from onset, with one to eight joints affected. Finger or toe joint were usually spared. Extraarticular synovitis such as tenosynovitis or bursitis were not observed, nor were nodules, signs of vasculitis or visceral involvement. The course of this arthritis showed chronicity and mild, persis…

In order to recognize courses with high and low progression in rheumatoid arthritis deviating from former approach the values of a locomotor function test were used which do not belong to the dispersion region for the adequate classes of duration of a disease. With the help of a material of an electronic data processing study of 1,000 cases with ascertained diagnosis correlations between high progression and, among others, following criteria could be ascertained: male sex, old age, early beginning of the disease, manual trades, unfavourable functional values and very much changed laboratory va…

Sepsis is an unusually common cause of illness and death in RA. All sorts of infections occur, but pyarthrosis produces exceptional problems. Clinically, pyarthrosis, empyema, and purulent pericarditis mimic bland rheumatoid effusions. Aspiration of the attendant effusions is the only reliable diagnostic procedure. Subcutaneous nodules on the sacrum and back are easily overlooked. Necrosis and ulceration of these nodules may provoke septicemia. Those with Felty's syndrome do not uniformly have problems with recurrent infection. Splenectomy may not benefit such patients. The belief that cortico

In patients with recent inflammatory arthritis HL-A27 was seen in 6 out of 7 patients with ankylosing spondylitis, in 15 out of 18 patients with Reiter's disease, in 9 out of 12 patients with reactive postinfectious arthritis, in 22 out of 45 patients with rheumatoid arthritis, and in 2 out of 14 patients with other types of inflammatory arthritis. In two control series HL-A27 occurred in 46 out of 326 persons and in 3 out of 34 persons. The high frequency of HL-A27 in RA patients is in disagreement with previous results by other investigators. The explanation for this may be either that the d…

The origin of rheumatoid arthritis (RA) is in our opinion a bacterial infection. The infection gives rise to changes in the macrophages, with release of enzymes, etc., and secondarily abnormal immune processes occur. In favor of this opinion is, among other things, the similarity with rheumatic fever, which is caused by streptococci group A, as well as experience gained in connection with experimentally provoked arthritis. In experimental arthritis, produced by streptococci group B (Svartz), there appears in rats the same type of joint disease as in human RA and, besides, a rheumatoid factor (…

A brief review of bacterial arthritis as seen at the Mayo Clinic in the years 1958 through 1973 has been presented with a selected literature review. Once considered a disease of children and adolescents, this disease now is seen more often in adults, especially in those who are elderly, are chronically ill, or are receiving medications that lower the host resistance in invading bacteria. The diagnosis must be made early, with rapid isolation and identification of the organism. Proper antibacterial drugs should be selected on the basis of in vitro susceptibility tests. The joint should be eith…

Seven patients with myasthenia gravis developed clinical signs of arthropathy. In two patients, the symptoms were due to a deforming rheumatoid arthritis and the myasthenic symptoms appeared as a transitory phase during the course of the disease. Muscle antibodies of IgG class were demonstrated with sera from both patients. Autoreactivity between muscle antibodies and rheumatoid factor was detected in one patient. Both patients died from sudden cardiac failure. Necropsy was performed in one and revealed a spotty myocardial necrosis. One patient had juvenile rheumatoid arthritis. Two patients h…
