COVID-19 revisiting inflammatory pathways of arthritis.

Coronavirus disease 2019 (COVID-19) is an infectious disease, caused by severe acute respiratory syndrome coronavirus 2, which predominantly affects the lungs and, under certain circumstances, leads to an excessive or uncontrolled immune activation and cytokine response in alveolar structures. The pattern of pro-inflammatory cytokines induced in COVID-19 has similarities to those targeted in the treatment of rheumatoid arthritis. Several clinical studies are underway that test the effects of inhibiting IL-6, IL-1β or TNF or targeting cytokine signalling via Janus kinase inhibition in the treat

The use of biologic therapies in uveitis.

PURPOSE: Non-infectious uveitis has been long controlled with the use of corticosteroids with many side effects and poor control in some cases. The purpose of this paper was to assess the different biologic agents (in this case infliximab and adalimumab) and to compare their efficacy in the treatment of uveitis. RESULTS: Adalimumab has been proven very successful in replacing or aiding corticosteroid therapy in different autoimmune mediated uveitis (Juvenile Idiopathic Arthritis, Rheumatoid arthritis, sarcoidosis) whereas infliximab has been used intravenously and recently intravitreally with

A case of rheumatoid arthritis with unilateral knee synovial hypertrophy in hemiplegia.

A 64-year-old woman suffering right hemiplegia came in with pain and swelling on her left knee, general weakness and poor oral intake for 2 months. On physical examination we were able to palpate a mass with irregular margin around the left suprapatellar area. From the results of the magnetic resonance imaging (MRI), synovial proliferative disease, infectious arthritis, or gouty arthritis was suspected. We performed a blood laboratory test to detect rheumatologic diseases, knee joint aspiration, and bone scan for differential diagnosis, and were able to diagnose rheumatoid arthritis (RA) from

Toxic shock syndrome toxin 1 contributes to the arthritogenicity of Staphylococcus aureus.

Although enterotoxins have been implicated in disease states such as food poisoning and toxic shock syndrome, their role in infectious arthritis is not known. To study the arthritogenic properties of toxic shock syndrome toxin 1 (TSST-1), two pairs of S. aureus strains isogenic for TSST-1 production were injected intravenously into healthy Swiss mice. Mice injected with TSST-1-secreting staphylococcal strains developed more frequent and more severe arthritis than did mice inoculated with the isogenic TSST-1-deficient counterparts. Immunohistochemical analysis of arthritic joints revealed an eq

[Differential diagnostics of febrile conditions in an emergency care clinic].

We undertook the analysis of 157 cases of fever at the stages of polyclinic - admission department - hospital treatment for the purpose of elucidating the structure of febrile syndrome. Pneumonia developed in 45 patients, infectious endocarditis in 34, chronic alcoholic hepatitis in 21, rheumatoid arthritis (pseudoseptic variant) in 2, systemic lupus erythematosus in 1, polymyositis in 2, acute pyelonephritis (exacerbation of chronic pyelonephritis) in 15, tumours of different localization in 37 patients. We evaluated the informative value of some acute-phase blood characteristics obtained, re

Hazards of steroid injection: Suppurative extensor tendon rupture.

Local steroid injections are often administered in the office setting for treatment of trigger finger, carpal tunnel syndrome, de Quervain's tenosynovitis, and basal joint arthritis. If attention is paid to sterile technique, infectious complications are rare. We present a case of suppurative extensor tenosynovitis arising after local steroid injection for vague symptoms of dorsal hand and wrist pain. The progression of signs and symptoms following injection suggests a natural history involving bacterial superinfection leading to tendon rupture. We discuss the pitfalls of local steroid injecti

Subacute Bacterial Cellulitis With a Subacute Clinical Course With Difficulty in Distinguishing From Sjögren's Syndrome: A Case Report.

Differentiating between infectious and autoimmune diseases regarding inflammatory changes in the soft tissues around the joints that develop in a subacute course is often difficult. Herein, we present the case of a 65-year-old woman who presented with a chief complaint of the tarsal metatarsal joint (Lisfranc joint) pain in the second, third, and fourth toes of her right foot that had persisted for several days. The patient was initially treated with non-steroidal anti-inflammatory drugs, colchicine, and prednisolone 10 mg for pseudogout and Sjogren's syndrome arthritis; however, there was lit

Primary cutaneous Nocardia otitidiscaviarum infection in a patient with rheumatoid arthritis treated with infliximab.

Anti-tumor necrosis factor-a (anti-TNF-a) therapy strategies result in significant clinical benefits in patients with rheumatoid arthritis, but with an increased rate of serious infectious diseases. We describe a patient receiving infliximab who developed a primary cutaneous Nocardia otitidiscaviarum infection after a skin injury.

Reactive arthritis induced by bacterial vaginosis: prevention with an effective treatment.

We report a 42-year-old woman with reactive arthritis induced by bacterial vaginosis who presented with oligoarthritis with an additive form, arthralgia, and enthesitis. She hasn't had a history of diarrhea or dysuria or vaginal secretion, or sexually transmitted infections (STIs). The laboratory tests were normal except for a high erythrocyte sedimentation rate (ESR). Her pelvic examination revealed homogeneous white grey and malodorous vaginal discharge on the vaginal wall and Pap smear and Gram-stained smear of vaginal swab was consistent with bacterial vaginosis. She responded to metronida

The MHC class I-related FcRn ameliorates murine Lyme arthritis.

The identification of the neonatal FcR (FcRn) as an IgG homeostasis regulator has led to research aimed at delineating a role for FcRn in humorally mediated disease. FcRn is a class I-related molecule that prolongs the half-life of serum IgG by preferentially binding IgG at low pH and inhibiting its degradation. Its role in protective immunity to infectious organisms is unknown. We investigated the function of FcRn in the murine model of Lyme arthritis, caused by infection with Borrelia burgdorferi. We infected FcRn(-/-) and wild-type mice with B. burgdorferi and monitored the development of a

[The cricoarytenoid joint in chronic polyarthritis].

The cricoarytenoid joints are frequently asymptomatically involved in rheumatoid arthritis with a severe course. At least the symptoms recede into the background compared with the other complaints. Signs of obstructive laryngeal changes are often falsely interpreted. Dangerous respiratory tract obstructions may be initiated by bacterial infections of the larynx and mechanical laryngeal changes or by the acute exacerbation of the arthritis. The clinical diagnosis of CA-arthritis is supplemented by laryngoscopic, radiologic and computertomographic techniques. Simple clinical examination methods

Septic arthritis due to Clostridium ramosum.

OBJECTIVES: Clostridium species are anaerobic bacilli that are rarely reported as etiologic agents of infectious arthritis. Previous cases of arthritis caused by Clostridium ramosum have not been reported. We describe the first 2 cases of C. ramosum arthritis. METHODS: We reviewed the etiology of arthritis in our hospital during the previous 15 years. RESULTS: Both patients had underlying immunocompromising conditions and their infections involved a joint with preexisting disease: patient 1 had rheumatic arthritis and a prosthetic joint; patient 2, chronic renal failure on dialysis and hip o

[Arthropathies related to HLA-B27].

Spondyloarthropathies represent an important problem within the field of chronic childhood arthropathies. Nosology and differential diagnosis are yet unclear. It is important to distinguish spondyloarthropathies from JCA because biological aspects of affected patients, clinical findings, extraarticular manifestations and prognosis are very different. Ankylosing spondyloarthritis is the prototype of spondyloarthropathies: at the beginning, axial involvement is rare; it may develop during the following years or it may not occur. Enthesopathy is an important finding of spondyloarthropathies. Dise

The Seasonality of Childhood Bone and Joint Infection with Focus on Kingella kingae: A Systematic Review.

BACKGROUND: Seasonal trends in hospitalization for childhood bone and joint infection (BJI) are reported inconsistently. True seasonal variation would suggest an element of disease risk from environmental factors. This review evaluates all reported seasonal variations in childhood BJI, with additional analysis of seasonal trends for diseases secondary to Kingella kingae. METHODS: A systematic review of the literature was undertaken from January 1, 1980, to August 1, 2024. Data were extracted on the hospitalization rate by season and/or month. Pathogen-specific studies for BJI secondary to K.

[Osteoarticular infections in children].

We presented a retrospective review of osteoarticular infections diagnosed at Sant Joan de Deu Hospital from Barcelona, in the last 10 years (1981-1990). A total of 127 arthritis and 113 osteomyelitis were recorded in children aged from a few days to 15 years. This represents a 3.5% of all infectious diseases patients during the study period. The most common microorganism was S. aureus (52.3%) followed by Brucella (8.2%) and Salmonella as well as Streptococcus agalactiae (7.3% each). The most common involved joint were hip and knee in arthritis cases, but long bones (tibia and humerus) in oste

[Meningeal and Guillain-Barrè syndrome in a patient with rheumatoid arthritis receiving adalimumab therapy].

Adalimumab is a recombinant human monoclonal antibody that blocks the effects of tumor necrosis factor-alpha, and is presently used for treatment of rheumatoid arthritis, with demyelination being a potential adverse effect. A 31 year-old male with seropositive rheumatoid arthritis presented with diarrhea after the second injection of adalimumab. He was treated with ciprofloxacin. In a few days he developed a Guillain-Barrè syndrome confirmed by electromyography, and his cerebrospinal fluid was compatible with meningeal syndrome or partially treated bacterial meningitis. Adalimumab may be assoc

Skin disorders with arthritis.

Many inflammatory, metabolic and infectious diseases affect the skin and joints. Most of these, such as rheumatoid arthritis and systemic lupus erythaematosus, are considered to be rheumatic conditions with secondary skin involvement. However, several primary cutaneous diseases are associated with arthritis and may even present with joint symptoms prior to cutaneous lesions. Common skin disorders, such as acne and psoriasis, have well-known musculoskeletal manifestations. Other less common conditions, such as dermatomyositis, multicentric reticulohistiocytosis, pyoderma gangrenosum, Sweet's sy

Clinical characteristics of peripheral spondyloarthritis without psoriasis, inflammatory enteropathy or preceding infection, from a single rheumatology clinic in northern India.

The objective of this study was to characterise the peripheral arthritis of spondyloarthritis (pSpA) excluding psoriatic-, inflammatory enteropathy-related, post-infectious reactive-SpA and arthritis associated with axial SpA/ankylosing spondylitis (axSpA/AS). Patients presenting with the clinical features of the broad spectrum of SpA were screened for the presence of peripheral arthritis using the Assessment of SpondyloArthritis International Society (ASAS) criteria. The other conditions that could cause pSpA, e.g. psoriasis, inflammatory enteropathy, reactive arthritis and axSpA/AS, were exc
